Distal Radioulnar Joint Instability in Distal Radius Fracture.

Distal radioulnar joint (DRUJ) instability following distal radius fracture (DRF) is an increasingly recognised entity, particularly with recent advances in the management of DRF. This review aims to provide a comprehensive appraisal of DRUJ anatomy, methods of assessing instability and current treatment strategies. Both non-operative and operative modalities are considered. In the acute setting, surgical management primarily involves open reduction and internal fixation (ORIF) of the distal radius, which can restore DRUJ stability through correction of translation and distraction of the dista

Association of Nontobacco Nicotine Dependence with Postoperative Complications After Distal Radius ORIF: A Retrospective Analysis.

PURPOSE: Distal radius fractures are common, with management varying by age, activity level, and fracture characteristics. Although operative treatment can improve early recovery, it may be associated with complications. Nontobacco nicotine dependence (NTND), through products like e-cigarettes, has increased, but its impact on postoperative outcomes remains underexplored. This study investigated the effect of NTND on complications following distal radius fracture open reduction and internal fixation (ORIF). METHODS: A retrospective cohort study using the TriNetX database categorized adult pat

How Should the Shape of the Volar Rim Be Taken into Account in the Treatment of Distal Radius Fractures? A Radiological Study.

BACKGROUND: In the management of articular distal radius fractures, achieving an accurate reduction of joint fragments is essential for a favorable prognosis. Fixation of the volar rim remains a surgical challenge, as volar plates may be improperly positioned despite adequate joint reduction. PURPOSE: This study aims to analyze anatomical variations in the distal radius, focusing on the distance between the watershed line (WL) and the radiocarpal articular surface. These anatomical variations may partially explain the difficulty in achieving optimal plate placement during distal radius fractu

Correlation of Radiographic and Arthroscopic Staging in Kienböck Disease: Impact on Treatment Decisions and Functional Outcomes.

Background: The prognosis of Kienböck disease (KD) depends upon the extent of damage to the articular surface of the lunate and its adjacent articulations. Arthroscopy offers valuable information for management and decision-making by revealing the status of the articular surface, which radiographs cannot visualise. Methods: This study on KD assessed 10 patients, correlating radiographic and arthroscopic evaluations to enhance diagnostic accuracy and determine the decision-making, which may result in improved functional outcomes for patients. Results: Findings have shown that arthroscopy often

Combined Vascularised Bone Graft Techniques for Displaced Coronal Fracture in Kienböck Disease: A Case Report.

Surgical treatment of Kienböck disease remains controversial and depends on patient age, activity and disease stage. We report an 18-year-old female badminton player who presented with wrist pain, limited motion and decreased grip strength. Imaging revealed a displaced coronal lunate fracture, with the palmar fragment locked against the volar rim of the radius, consistent with Lichtman Stage IIIC and Bain grade 2b. MRI showed low signal intensity, suggesting compromised vascularity. Arthroscopy confirmed intact cartilage except at the fracture site. Lunate reconstruction using the Kuhlmann tec

Impact of Bariatric Surgery Upon the Incidence of Distal Radius Fractures Among Patients With Severe Obesity.

PURPOSE: Bariatric surgery has been associated with weight loss, amelioration of obesity-related conditions, and improvements in quality of life and longevity. However, there are concerns that bariatric surgery may increase the risks of subsequent fractures through relative malnutrition, metabolic changes, and decreased bone mineral density. The purpose of this study was to determine if a history of bariatric surgery leads to an increased risk of distal radius fractures (DRFs). METHODS: The PearlDiver Patient Records Database was queried from 2010 to 2023 to identify 4 groups of patients aged

Is RAGE Expression in Flexor Tendon Synovium Associated With Carpal Tunnel Syndrome in Patients With Diabetes?

BACKGROUND: Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy and is more prevalent in patients with diabetes. However, the underlying mechanisms linking diabetes and CTS have not been fully elucidated. The advanced glycation end products (AGEs) and the receptor for AGEs (RAGE) are involved in chronic inflammation and diabetic complications, suggesting that this pathway may be relevant to the pathogenesis of CTS in patients with diabetes. QUESTIONS/PURPOSES: (1) Is RAGE expressed in the flexor tendon synovium in patients with CTS, and if so, is it expressed more in patient
