Costo-Osteochondral Autograft for the Non-salvageable Proximal Scaphoid: A Systematic Review.

Background: Scaphoid non-union is a challenging condition that can lead to chronic wrist pain, decreased range of motion (ROM) and functional impairment. One promising approach involves the use of costo-osteochondral rib autografts, offering a non-vascularised and structurally supportive environment for bone regeneration. This systematic review aims to evaluate the current evidence regarding the management of non-salvageable scaphoid proximal poles and non-union using rib costo-osteochondral autografts. Methods: A comprehensive search of electronic databases was conducted. Data extraction was

Is Scaphoid Reconstruction Still an Option for Scaphoid Nonunion With Established Arthritis? A Retrospective Study of a Group of Patients.

PURPOSE: Although many studies have supported salvage procedures as the best option for scaphoid nonunions with established arthritis, there remains uncertainty regarding when procedures to gain union are favored over salvage procedures. The purpose of this study was to evaluate whether the short-term results from scaphoid nonunion with established arthritis treated by scaphoid reconstruction are acceptable. METHODS: We retrospectively studied the clinical outcomes of scaphoid nonunions with established arthritis treated by scaphoid reconstruction between January 2019 and December 2021. Eight

Impact of body mass index on fracture severity, clinical, radiological and functional outcome in distal radius fractures: a retrospective observational study after surgical treatment.

INTRODUCTION: Distal radius fracture (DRF) is one of the three most common fractures of the human body with increasing incidences in all groups of age. Known causes of increasing incidence, such as ageing of the population or increased obesity, have been described and discussed. So far, literature reports ambivalent effects of body mass index (BMI) on bone physiology. It is worthwhile to examine the influence of BMI on the outcome of fractures more detailed. This study aims to investigate the influence of an abnormal BMI on fracture severity and treatment, as well as clinical, radiological, an

Necrotizing Soft Tissue Infection Following a Closed Distal Radius Fracture: A Case Report and Literature Review.

Necrotizing soft tissue infections (NSTIs), characterized by extensive soft tissue destruction, are rare but life-threatening. We present a case of a NSTI in a healthy 65-year-old woman following a closed distal radius fracture. The patient presented with severe pain, fever, and lethargy 4 days after her index injury, with physical examination of the right upper limb revealing erythema and swelling to the mid-humeral level and blisters of the fingers and hand. Multiple surgical debridements were required to control the infection, which was caused by Streptococcus pyogenes. This case highlights

Etiology and Diagnostic Challenges of Ulnar Wrist Pain in Pediatric and Adolescent Patients.

PURPOSE: Diagnosing the cause of ulnar-sided wrist pain can be difficult in the pediatric and adolescent age group. While frequently used, the diagnostic accuracy of magnetic resonance image (MRI), as compared with intraoperative arthroscopic findings, is not well-described in this population. This study aimed to determine concordance rates between magnetic resonance and arthroscopic findings depending on the specific ulnar wrist pathology. METHODS: A retrospective review was performed to identify pediatric and adolescent patients who underwent operative treatment of ulnar wrist pain between

Do Social Deprivation and Surgical Volume Influence Outcomes Following Distal Radius Fracture Fixation?

BACKGROUND: Distal radius fractures account for nearly 25% of fractures in adults, with a trend toward operative fixation. The objective of this study was to assess the relationship between surgeon and hospital volume with complications following distal radius fixation. METHODS: A retrospective study was performed using the New York Statewide Planning and Research Cooperative System database from 2009 to 2015. Outpatient claims were identified for distal radius fractures and surgery. The facility and surgeon's identifier were used to calculate annual procedure volume. The risk for infection,

Non- or minimally displaced distal radius fractures in adult patients < 50 years of age : Three weeks of cast immobilisation versus one week of brace immobilisation: study protocol for a multicentre randomised controlled trial.

BACKGROUND: Currently, non- or minimally displaced distal radius fractures are treated by 3 to 5 weeks of cast immobilisation. Many patients with a distal radius fracture suffer from long-term functional restrictions, which might be related to stiffness due to cast immobilisation. Current literature indicates that 1 week of immobilisation might be safe; however, no level 1 evidence is available. This trial aims to compare 1 week of brace immobilisation with 3 weeks of cast immobilisation in patients with distal radius fractures that do not need reduction. METHODS: The aim of this trial is to

Round table discussion. Distal radioulnar joint instability after surgical treatment of distal radial fractures.

Distal radioulnar joint instability is commonly seen after surgical fixation of a distal radial fracture, and surgeons' ability to reliably determine stability on examination is poor. Debate remains regarding whether to fix the ulnar styloid or reinsert the triangular fibrocartilaginous complex. Four surgeons with Level 5 expertise were asked to respond to questions surrounding this debate.

Comparative Analysis of Surgical Approaches in the Treatment of Proximal Humerus Fractures: A Retrospective Study.

BACKGROUND: "Proximal humerus fractures [PHFs]" are common orthopedic injuries, often requiring surgical intervention for optimal outcomes. However, the choice of surgical approach remains controversial, with varying outcomes reported in the literature. METHODS: A retrospective research was overseen among subjects who had surgery for PHFs. Three surgical approaches were compared: "Open Reduction Internal Fixation (ORIF)", "Hemiarthroplasty [HA]", and "Reverse Shoulder Arthroplasty [RSA]". Demographic data, fracture characteristics, surgical details, and postoperative outcomes were analyzed.

Robot-assisted percutaneous scaphoid fixation: patient-reported outcomes and learning curve at two centres.

This study describes patient-reported outcomes of robot-assisted percutaneous scaphoid fracture fixation of 62 patients from two medical centres and the learning curve of this new technique. One attempt to place the guidewire was sufficient in 97% of cases. All fractures achieved radiographic union at a mean of 9 weeks. There were no complications observed. At a mean follow-up of 36 months (range 12-68 months), the mean patient-rated wrist evaluation (PRWE) was 2 (range 0-22) and the mean Mayo Wrist Score was 96 (range 70-100). After the initial ten to 20 cases, the learning phase was reasonab
