The Risk of Acute Carpal Tunnel Syndrome in Lunate and Perilunate Dislocations.

PURPOSE: Lunate and perilunate dislocations are uncommon wrist injuries that have a high reported association with acute median neuropathy, frequently referred to as acute carpal tunnel syndrome (ACTS). Previous investigations into the incidence of lunate and perilunate dislocation and acute median neuropathy have often been limited by small sample sizes. The purpose of our study was to report a relatively large cohort examining the incidence of ACTS in lunate and perilunate dislocations and assess predictive risk factors. METHODS: A retrospective review was performed of all lunate and perilu

Acromioclavicular Joint Injury and Repair.

The acromioclavicular (AC) joint is commonly injured in athletes participating in contact and overhead throwing sports. Injuries range from simple sprains to complete ligamentous disruption, and they are classified by the established Rockwood grading system. High-grade injuries are associated with fractures around the AC joint and disruption of the superior shoulder suspensory complex, a ring of osseous and ligamentous structures at the superior aspect of the shoulder. Radiographs are the mainstay of imaging of the AC joint, with magnetic resonance imaging reserved for high-grade injuries to a

Perilunate dislocation and fracture dislocation of the wrist: Outcomes and long-term prognostic factors.

INTRODUCTION: Perilunate dislocations and fracture-dislocations are severe injuries that often have serious functional sequelae. Our goal was to evaluate the long-term clinical and radiological results of these perilunate injuries, and to look for prognostic factors of a poor clinical outcome. HYPOTHESIS: All patients who suffered perilunate injuries in their wrist have functional sequelae and long-term radiographic changes despite optimal treatment with anatomical surgical reduction. MATERIALS AND METHODS: We did a single-center, retrospective study of 32 patients who had either an isolated

Patient Likelihood to Repeat Surgery After Highly "Successful" Hand Procedures.

BACKGROUND: Outcomes for routine hand procedures, such as carpal tunnel release, trigger finger release, and first dorsal compartment release, are typically reported as "highly successful" with "infrequent complications" based on classic literature. No contemporary study has utilized a large prospective registry to assess patient willingness to repeat surgery. We utilized a prospective hand registry to evaluate the proportion of patients that would repeat surgery after common hand procedures. METHODS: We utilized a prospective, hand surgery registry to collect postoperative patient likelihood

Comparison of pediatric performance athlete injuries presenting to emergency departments.

OBJECTIVE: Pediatric injuries in performance sports represent a significant healthcare burden and account for over 50,000 annual Emergency Department (ED) visits in the United States. The objective of this study was to characterize and compare pediatric injury presentation across the most common performance sports. METHODS: The National Electronic Injury Surveillance System (NEISS) database was retrospectively analyzed for pediatric injuries (3-18 years) related to gymnastics, dance, or cheerleading from 2015-2019. Cases were categorized as children (˂11 years) or adolescent (≥11 years). Inju

Predictors of Reduced Frequency of Physical Activity 3 Months After Injury: Findings From the Prospective Outcomes of Injury Study.

BACKGROUND: Physical inactivity is a risk factor for noncommunicable diseases. Incurring an injury has been identified as a barrier to physical activity. However, it is not known which factors, if any, are associated with reduced activity among general injury populations. OBJECTIVE: The 2 study objectives were: (1) to investigate changes in physical activity frequency from before an injury to 3 months after the injury and (2) to examine the association of preinjury, injury-related, and postinjury factors with reduced physical activity frequency in participants with a range of injury types. Carpal tunnel syndrome caused by space occupying lesions.

PURPOSE: To evaluate the diagnosis and treatment of the carpal tunnel syndrome (CTS) due to space occupying lesions (SOL). MATERIALS AND METHODS: Eleven patients and 12 cases that underwent surgery for CTS due to SOL were studied retrospectively. We excluded SOL caused by bony lesions, such as malunion of distal radius fracture, volar lunate dislocation, etc. The average age was 51 years. There were 3 men and 8 women. Follow-up period was 12 to 40 months with an average of 18 months. The diagnosis of CTS was made clinically and electrophysiologically. In patients with swelling or tenderness o

Tears of the subscapularis tendon in athletes--diagnosis and repair techniques.

Isolated injury to the subscapularis tendon is an uncommon - but potentially debilitating - injury among athletes. The most frequent injury pattern seen in younger athletes is an acute, traumatic tendon avulsion. In the older athletic population, chronic degenerative tearing of the tendon may precede sports-related rupture, particularly following shoulder dislocation. This article reviews how to diagnose this injury in athletes, and describes both conservative and operative treatment options and their outcomes.

[Bone disease with Pain. Colles' fracture].

The distal radius is one of the most commonly fractured long bone. Colles' fracture results from a fall on the dorsiflexed and pronated hand. The dinner-fork deformity is the typical deformity of the Colles' fracture. For patients with no or a little displacement, conservative treatment is applied. The non-bridge type external fixator is applied for patients without an intra articular fracture. For patients with a comminuted fracture, the locking plate (volar approach) is recommended. During the healing period, shoulder, elbow and finger exercise should be insisted.

Shoulder dislocation after infraclavicular coracoid block.

OBJECTIVE: We report a case of shoulder dislocation after a brachial plexus block with a double-injection technique using the infraclavicular coracoid approach. CASE REPORT: An obese woman with no previous episodes of joint dislocation developed an anterior dislocation of the head of the humerus after an infraclavicular coracoid block performed for hand surgery. Dislocation was probably because of a combination of unrecognized glenohumeral instability, paralysis of some muscles of the shoulder, and positioning of her arm on a board below her torso. Shoulder dislocation was recognized after su

Advanced ureteral cancer with complete remission achieved by taxan containing systemic chemotherapy.

BACKGROUND: Excess gestational weight gain (GWG) in pregnant adolescents is a major public health concern. Excess GWG increases risk of pregnancy complications as well as postpartum and offspring obesity and cardiometabolic disease. Prevention interventions for pregnant adults that target lifestyle modification (i.e., healthy eating/physical activity) show insufficient effectiveness. Pregnant adolescents have distinct social-emotional needs, which may contribute to excess GWG. From an interpersonal theoretical framework, conflict and low social support increase negative emotions, which in turn

[Ulnar nerve compression and pseudoarthrosis of the medial metacarpal bases].

A case of compression of the deep branch of the ulnar nerve associated with pseudarthrosis of the base of the fifth metacarpal and the upper shaft of the fourth metacarpal is reported. The delayed procedure consisted in volar decompression of the ulnar nerve by dividing the pisi-hamate ligament and plating and grafting of both fractures. The patient achieved marked improvement four weeks post-operatively and had complete functional recovery at follow-up of 16 years after injury without narrowing of the hamato-metacarpal joint despite synostosis of the bases of the medial metacarpals.

The biomechanics of a thumb carpometacarpal immobilization splint: design and fitting.

Splinting for the common osteoarthritis of the carpometacarpal (CMC) joint of the thumb is infrequently described in the literature, but the few splints that are described include one or both adjacent joints. This paper describes the design and biomechanics of a custom-molded thumb CMC immobilization splint that excludes the thumb metacarpophalangeal and wrist joints. The problem of the imbalance of extrinsic extensor/abductor forces against the intrinsic flexor/adductor forces is described. The accompanying weakening of the thumb CMC capsule allows dorsal shifting of the proximal end of the m

[Stabilization-interposition arthroplasty in peri-trapezial arthritic lesions. Apropos of a prospective series of 200 cases].

A group of 200 arthroplasties (180 patients) for basal thumb arthrosis is reported. The used technique is similar to that described by Burton and Pelegrini in 1986. At the last follow up (2 years minimum), 90% patients were without pain, 95% were very satisfied, the grip strength was improved in 70%, and the shortening of the thumb was between 2 and 9 mm (mean 6 m). The mobility of the first metacarpal is improved in only 47%. Complications were limited to two NAD for patients operated of carpal tunnel syndrome in the same time.

The hand complications of Colles' fractures.

Two hundred and thirty-five patients with displaced Colles' fractures were followed to union, and 209 patients to six months, specifically searching for hand pathology. The incidence of carpal tunnel syndrome was seventeen per cent at three months and twelve per cent at six months. The patients with the syndrome were significantly older and their fractures showed significantly greater residual dorsal angulation. Sixteen patients developed Dupuytren's disease between three and six months, when the incidence was eleven per cent. These patients were significantly older and had average fractures a
