Digital three-dimensional planning combined with 3D-printed patient-specific guide-assisted step-cut osteotomy for correction of old cubitus varus deformity: a case report.

Cubitus varus is the most common long-term complication following supracondylar humeral fractures. Although traditional step-cut osteotomy yields reliable clinical outcomes, the intraoperative osteotomy direction and angle remain largely dependent on the surgeon's experience, resulting in suboptimal precision. Although digital three-dimensional (3D) planning and 3D-printed patient-specific guides have been shown to improve osteotomy accuracy, their application in adolescent patients with long-standing cubitus varus deformity has rarely been reported. We report an 18-year-old male patient who d

Outcomes of simultaneous microsurgical replantation of complete amputations at the distal third of the right forearm and left wrist: a case report and 22-month follow-up.

Microsurgical replantation of simultaneous bilateral upper extremity amputations is a complex procedure requiring a highly coordinated multidisciplinary team. Functional outcomes depend on anatomical level of injury, duration of ischemia, extent of crushing, degree of contamination, and quality of postoperative rehabilitation. We report a 45-year-old male industrial worker who sustained complete simultaneous amputations through the distal third of the right forearm and at the left wrist following an industrial paper-cutting machine accident. Total ischemia time was 8 h. Two microsurgical teams

Study of the Spread of Scarring along the Damaged Area of the Limb Nerve after Gunshot and Explosive Injuries.

BACKGROUND: Determining the true size or extent of the nerve defect after damage and excision of the posttraumatic neuroma is fundamental in the microsurgical treatment of the patient. It is assumed that the length of the scar in the nerve may depend on the nature of the combat wound of the limb, gunshot, or mine-explosive. Determining the extent of scarring in the nerve in cases of these two types of injury may provide an opportunity to establish a relationship with the type of injury or to disprove such a relationship. This should help in the surgical treatment tactics. METHODS: Fifty sampl

Post-cycle therapy after androgen abuse: what are we seeking to achieve?

Despite limited evidence supporting its efficacy, the use of post-cycle therapy (PCT) is widespread amongst men following cessation of androgen abuse. The proposed rationale for PCT is to accelerate recovery of hypothalamic-pituitary-gonadal (HPG) axis function following androgen-induced suppression. In this commentary, we consider the distinction between pharmacological response and sustained recovery of endogenous HPG axis function, the relationship between biochemical and symptomatic recovery, and the potential role of PCT as a harm-reduction strategy.

Surgical Management of Digital Neuromas: A Systematic Review of Techniques, Outcomes, and Pain Relief.

BACKGROUND: Painful digital neuromas are disorganized nerve proliferations that develop following traumatic or iatrogenic digital nerve injury, often resulting in debilitating allodynia, hypersensitivity, and impaired hand function. While conservative therapies such as desensitization, splinting, and pharmacological agents may offer temporary relief, over 50% of patients ultimately require surgical intervention. However, surgical strategies remain highly heterogeneous, and no consensus exists regarding the most effective operative technique. METHODS: A comprehensive search of 4 databases thro

Firearm-Related Upper-Limb Injuries in Children: An 8-Year Single Institution Analysis.

PURPOSE: Pediatric firearm-related injuries are rising, reflecting an ongoing public health concern and burden on the health care system. However, studies examining regional gunshot wound (GSW) trends among children are scarce. We evaluated upper-limb injuries from GSWs at our state's sole level-1 trauma center over 8 years. We hypothesize there would be an increase in upper limb ballistic injuries in our pediatric population and that understanding these trends may lead to improved supportive care and interventions aimed at mitigating future occurrences. METHODS: A retrospective chart review

Improving hand function in chronic incomplete tetraplegia by high-PAS intervention with alternative equipment: a case report.

INTRODUCTION: The reproducibility of a paired associative stimulation (high-PAS) protocol, shown to be beneficial for rehabilitation in incomplete spinal cord injury (SCI), was investigated in a different technical environment. Whereas previous studies relied on a specific technical setup for high-PAS, here the protocol was replicated using alternative devices. Differences in technical attributes could influence the replicability of specific output. As the necessary technology is available in many clinical and research settings, validating the protocol across device systems is important for br

A Case of Brachial Plexus Injury Induced by Intraoperative Positioning during Thoracoscopic Resection of a Mediastinal Tumor.

INTRODUCTION: Brachial plexus injury is a rare but serious complication of thoracic surgery performed in the lateral position. Improper positioning of the upper limb or neck can increase the risk of nerve damage. Risk factors include excessive abduction of the upper limb and hyperextension of the neck. Preventive measures-such as preoperative simulation of surgical positioning, regular intraoperative reassessment of limb and neck alignment, and close collaboration among surgeons, anesthesiologists, and nursing staff-are critical for ensuring patient safety. CASE PRESENTATION: A 40-year-old fe

Impact of Microscope Versus Loupes on Sensation Disturbances After Digital Nerve Repair: A Propensity Score-Matched Analysis.

BACKGROUND: Digital nerve repair is a procedure performed to restore sensation in fingers. While surgical microscopes are used to achieve optimal outcomes, their cost may limit accessibility. Loupes, in contrast, offer a more affordable alternative, particularly in resource-limited settings. This study compares the risk of developing skin sensation disturbances in patients who underwent digital nerve repair using microscopes versus loupes. METHODS: The TriNetX database was queried to identify patients who underwent digital nerve repair. Patients were classified into two cohorts: microscope an

Hand Dysfunction After Intervention via Distal Versus Conventional Transradial Access: A Meta-Analysis of Randomized Trials.

BACKGROUND: Percutaneous coronary intervention (PCI) is an important treatment for coronary artery disease (CAD). Distal transradial access (dTRA) has gained popularity as an alternative to conventional transradial access (TRA) in cardiovascular interventional procedures. However, some patients may experience hand dysfunction after undergoing TRA or dTRA procedures. METHODS: In this study, the literature from the PubMed and Web of Science (WOS) databases was reviewed and 11 randomized controlled trials (RCTs) were selected. Meta-analysis and narrative synthesis were used to evaluate the hand

Upper Limb Peripheral Nerve Injuries Associated With Subdermal Contraceptive Implants: A Case Series, Systematic Review, and Proposed Evidence-based Treatment Algorithm.

BACKGROUND: Subdermal contraceptive implants (SCIs) are a widely used form of long-acting reversible contraception (LARC) with high efficacy (Pearl Index 0.00, 95% CI: 0.00-0.14). However, upper limb neurovascular injuries related to SCI insertion and removal are increasingly reported. The true incidence of these complications cannot be reliably estimated from the current literature, which is largely limited to case reports and small case series, although sporadic cases are being described with increasing frequency. These injuries may result in significant morbidity, including chronic neuropat

Transient peripheral neuropathy following autologous breast reconstruction: a case series and evidence-based protocol for risk reduction.

Free flap reconstruction is the gold standard following mastectomy, yet peri-operative peripheral neuropathies remain underrecognized complications. This study aimed to identify peri-operative risk factors for upper limb peripheral nerve injury in patients undergoing autologous breast reconstruction and propose modifications to minimize risk. Out of 155 patients undergoing autologous abdominal breast reconstruction performed by a single surgeon between 2014 and 2022, eight developed transient postoperative upper limb neuropraxia. All affected patients were overweight/obese and classified as Am

Peripheral nerve schwannomas: Anatomical distribution, biopsy risks and surgical outcomes in a contemporary large-volume case series.

BACKGROUND: Schwannomas are benign peripheral nerve sheath tumours that may cause pain, sensory disturbance and functional deficits. Despite their typically indolent nature, management decisions carry the risk of neurological morbidity. This study presents the largest single-centre series of schwannomas reported to date, evaluating clinical presentation, imaging, management and outcomes. METHODS: A retrospective review was conducted of solitary schwannomas treated at a tertiary peripheral nerve injury unit between 2017 and 2020. Data collected included anatomical location, imaging features, p

A Narrative Review of Perioperative Peripheral Nerve Injuries After Major Surgery: Clinical Recognition, Electrodiagnostic Evaluation, and Rehabilitation Implications.

Perioperative peripheral nerve injuries (PPNIs) are complications of surgical and procedural care and may cause neuropathic pain and motor or sensory deficits that delay functional recovery. This narrative review summarizes common patterns of PPNI involving the extremities, practical bedside recognition, the role and timing of electrodiagnostic testing, and key rehabilitation considerations. Upper-limb involvement most often affects the brachial plexus, ulnar nerve, and median nerve. In contrast, lower-limb presentations commonly involve the lateral femoral cutaneous, sciatic, and common fibul

Preoperative risk factors associated with patient outcomes following arthroscopic elbow contracture release.

BACKGROUND: Elbow contractures can occur due to several causes including age-related degenerative disease, inflammatory conditions, and post-traumatic injury. The resulting pain and reduced range of motion can significantly interfere with activities of daily living and impede patient functionality. One treatment option for elbow contractures is arthroscopic contracture release. The purpose of this study is to evaluate the preoperative risk factors associated with inferior clinical outcomes following arthroscopic elbow contracture release. METHODS: This retrospective cohort study included pati
