A Narrative Review of Perioperative Peripheral Nerve Injuries After Major Surgery: Clinical Recognition, Electrodiagnostic Evaluation, and Rehabilitation Implications.

Perioperative peripheral nerve injuries (PPNIs) are complications of surgical and procedural care and may cause neuropathic pain and motor or sensory deficits that delay functional recovery. This narrative review summarizes common patterns of PPNI involving the extremities, practical bedside recognition, the role and timing of electrodiagnostic testing, and key rehabilitation considerations. Upper-limb involvement most often affects the brachial plexus, ulnar nerve, and median nerve. In contrast, lower-limb presentations commonly involve the lateral femoral cutaneous, sciatic, and common fibul

Spinal Cord Stimulation and Improved Hand Function in a Traumatic Upper Limb Replantation: A Case Report.

BACKGROUND: Spinal cord stimulation (SCS) is commonly used to treat chronic pain that is refractory to conservative management. SCS has demonstrated efficacy in reducing paresthesia and pain, but functional motor improvements after this type of intervention are reported less often. CASE REPORT: A 70-year-old man sustained a traumatic left forearm amputation in 2015, which was followed by surgical reattachment and persistent neuropathic pain. The patient tried several interventions, including nonsteroidal antiinflammatory drugs, muscle relaxants, gabapentin, pregabalin, opioids, duloxetine, an

Preoperative risk factors associated with patient outcomes following arthroscopic elbow contracture release.

BACKGROUND: Elbow contractures can occur due to several causes including age-related degenerative disease, inflammatory conditions, and post-traumatic injury. The resulting pain and reduced range of motion can significantly interfere with activities of daily living and impede patient functionality. One treatment option for elbow contractures is arthroscopic contracture release. The purpose of this study is to evaluate the preoperative risk factors associated with inferior clinical outcomes following arthroscopic elbow contracture release. METHODS: This retrospective cohort study included pati

Hitting A Nerve: T1 Nerve Root Injury Secondary to Open Heart Surgery.

INTRODUCTION: Patients who undergo cardiothoracic surgery and sternotomy can experience cardiovascular complications from manipulation of the great vessels or artificial pump bypass of the heart, raising the fear of stroke in the setting of focal neurological deficit. However, this study reveals a less common but often as focal and debilitating complication of these surgeries from direct injury of the peripheral nerves under prolonged physical tension. CASE STUDY: This case details the presentation of a 72-year-old woman who developed acute left upper extremity weakness following elective ope

Retroperitoneal Hematoma Following Lumbar Injection in a Patient Receiving Clopidogrel: A Case Report.

Lumbar spine injections are commonly used to manage spinal and radicular pain and have included techniques with varying anatomic depth and bleeding risk. Although hemorrhagic complications are rare, lumbar injections in patients receiving antiplatelet therapy-particularly when multiple deep lumbar procedures are performed-may result in clinically significant bleeding and neurologic injury. We report a case of a 56-year-old man receiving clopidogrel who underwent a lumbar medial branch block and a psoas compartment-related block during the same session at an outside hospital and subsequently de

Distribution of Infralesional Lower Motor Neuron Injury in Traumatic Cervical Spinal Cord Injury Candidates for Upper Limb Reconstruction: A Single Center Experience.

INTRODUCTION/AIMS: Patients with cervical spinal cord injury (SCI) prioritize recovery of upper extremity function. Upper extremity reconstruction can restore function in carefully selected individuals. Infralesional cord damage, observed below the neurological level of injury (NLI), may affect postoperative outcomes. We characterized the extent of infralesional lower motor neuron (LMN) injury in patients with traumatic cervical SCI. METHODS: Each patient underwent electromyography (EMG) of potential donor and recipient muscles based on preoperative clinical evaluation and specific candidacy

Holding on to bypass in endovascular times? A narrative review on the role of surgical LSA revascularization in zone 2 TEVAR and how to achieve optimal results.

Thoracic endovascular aortic repair (TEVAR) has become the preferred treatment for most descending thoracic aortic pathologies. In Zone 2 procedures, intentional coverage of the left subclavian artery (LSA) is frequently required to achieve an adequate proximal seal, but this carries a risk of stroke, spinal cord ischemia, and upper extremity ischemia. Preventive LSA revascularization is therefore recommended in current guidelines, although the level of supporting evidence remains limited. This narrative review summarizes the indications, timing strategies, techniques, and outcomes of surgical

The Clean Pass: Upper Limb Impalement with Complete Neurovascular Preservation.

INTRODUCTION: Impalement injuries of the upper limb represent a rare and striking form of penetrating trauma, often associated with significant risk of neurovascular and tendinous damage due to the compact anatomical arrangement of the forearm. Despite their alarming external appearance, actual morbidity is determined by the trajectory relative to critical structures. Effective management requires careful pre-hospital stabilization, thorough pre-operative assessment, appropriate imaging, and meticulous surgical extraction under direct visualization to minimize complications. CASE REPORT: A 29

Non-surgical casting versus surgical reduction for children with severely displaced distal radial fractures (the CRAFFT Study): a multicentre, randomised, controlled non-inferiority trial and economic evaluation.

BACKGROUND: Severely displaced distal radial fractures are among the most common and controversial injuries in children. Despite observational evidence of reliable remodelling with growth in younger children, their alarming radiographic appearance-particularly when completely displaced (off-ended)-has driven routine surgical reduction and fixation. The Children's Radius Acute Fracture Fixation Trial (CRAFFT) aimed to evaluate the clinical and cost-effectiveness of surgical reduction compared with non-surgical casting. METHODS: This pragmatic, multicentre, randomised, non-inferiority trial inc

[Comparative efficacy of island latissimus dorsi myocutaneous flap versus lower trapezius myocutaneous flap for repair of posterior neck skin and soft tissue defects].

OBJECTIVES: Posterior neck skin and soft tissue defects often result from electrical burns, post-tumor resection, infections, or trauma. The region has complex anatomy, high mobility, and proximity to the cervical spine, major vessels, and critical nerves; thus, reconstruction must balance functional restoration and aesthetic outcomes. The island latissimus dorsi myocutaneous flap, with its reliable blood supply, large harvestable area, and long rotation arc, is widely used for trunk and limb reconstruction and shows particular advantage for large defects in the thoracic, dorsal, and posterior

Sensory Recovery Following Digital Nerve Repair Using Conduits Versus Autologous Nerve Grafts: A Systematic Review and Meta-Analysis.

BACKGROUND: Peripheral nerve injuries (PNI), particularly digital nerve injuries, can lead to significant loss of function, pain, and sensory deficits. Autologous nerve grafting (ANG) has been the gold standard for nerve gap reconstruction, but it is associated with donor site morbidity and other complications. Nerve conduits, including collagen-based, chitosan-based, and muscle-in-vein conduits, have emerged as potential alternatives with the advantage of avoiding donor site morbidity. The objective of this systematic review and meta-analysis was to compare the efficacy and safety of nerve co

Concurrent Nerve and Tendon Transfers in a Tetraplegic Patient: Long-Term Functional Recovery and Late Hardware-Associated Flexor Pollicis Longus Rupture.

Tetraplegia is a significant impairment of mobility in all four limbs, often following a cervical spinal cord injury. Recent studies in the management of tetraplegia describe the use of nerve transfer in addition to tendon transfer for upper extremity rehabilitation. While the popularity of these techniques is growing, there are limited studies describing the long-term outcomes and functional recovery following such procedures. This case describes a patient who sustained a C7 spinal cord injury, right olecranon fracture, and left distal radius fracture following a fall. Surgeons performed open

Subdeltoid ultrasound-guided brachial plexus block: A technical description with anatomical correlation and a preliminary case series.

Regional anaesthesia in the emergency department requires reliable techniques with minimal complications. We describe a subdeltoid ultrasound-guided brachial plexus approach targeting a potential "nerve funnel" at the surgical neck of the humerus, bridging infraclavicular and axillary approaches. Cadaveric dissection explored its anatomical basis and correlated it with clinical sonographic findings. This retrospective case series included six adult patients with traumatic upper-limb injuries. Cadaveric observations suggested terminal nerve clustering and proximity of the musculocutaneous and i

Intraoperative Monitoring of the Diaphragm Function During Cervical Dorsal Root Entry Zone-Lesioning Illustration With a Case Report.

BACKGROUND AND IMPORTANCE: Microsurgical DREZotomy (MDT) involves lesioning the dorsal root entry zone at the cervical spinal level, carrying a potential risk of iatrogenic spinal cord injury. Misidentifying the C4 root can lead to impaired diaphragmatic contractions and respiratory compromise. This report is the first MDT procedure in our Central Asian region and introduces the simultaneous analysis of 3 ventilator waveforms-capnography, pressure time, and flow time-as a novel, rapid-response intraoperative neurophysiological monitoring for definitive C4 root identification.
