Insights into Cervical Phrenic Nerve Variations Based on Cadaveric Dissection and Intraoperative Electromyography.

BACKGROUND: In complex cervical nerve surgeries, ensuring the integrity of the phrenic nerve (PN) adjacent to the anterior scalene muscle is critical. However, we have encountered a clinical paradox: Postoperative diaphragmatic dysfunction occurred even when the main PN trunk was preserved intraoperatively. This suggests the potential involvement of PN variations (PNVs), such as the accessory PN, which merges with the main trunk, and separate branches that maintain an independent course. METHODS: Using contralateral C7 nerve transfer (CC7) as a surgical model, this study aimed to characterize

Iatrogenic Nerve Injuries Affecting Upper Limb Function: Medicolegal Implications.

PURPOSE: Nerve injuries are among the commonest iatrogenic complications and most frequently are the result of direct intraoperative damage causing serious distress and long-term morbidity. Additionally, they can add a considerable financial burden to health care systems, while they constitute a well-known source of medicolegal implications. There are few published articles addressing the medicolegal consequences of the iatrogenic nerve injuries affecting upper limb function. Our aim was to identify malpractice claims related to iatrogenic nerve injuries affecting upper limb function and to de

Pediatric digit replantation: A mixed methods outcomes study.

BACKGROUND: Indications for pediatric digital replantation/revascularization (replant/revasc) are broader than those for adults. Pediatric patients suffer many of the same challenges seen in adult replant/revasc patients with similar rates of digit salvage. The purpose of this study was to review the incidence of and long-term outcomes after pediatric replant/revasc. METHODS: Data was collected from medical records of patients presenting to our Level 1 Trauma Pediatric Hospital Emergency Room between January 2011 and March 2017. All charts with ICD-9 codes for upper extremity trauma were quer

When Stroke Mimics Peripheral Nerve Injury: A Case Report of Isolated Wrist Drop.

Isolated wrist drop as a presentation of stroke is uncommon and may pose a diagnostic dilemma. We present a case of a 65-year-old man with hypertension and diabetes who presented with sudden-onset weakness of the left hand and inability to extend fingers, without any other focal neurological deficits. Noncontrast computed tomography of the head was normal. However, diffusion-weighted magnetic resonance imaging of the brain revealed an acute ischemic infarct involving the right frontoparietal and occipital lobes. A diagnosis of isolated left-sided wrist drop secondary to acute ischemic stroke w

Postoperative Shoulder Splint-Induced Palm Pressure Leading to Median and Ulnar Neuropathies: A Case Report.

INTRODUCTION: Postoperative positioning devices, such as shoulder splints, are commonly used to support and stabilize the upper limb following surgery. However, improper placement and prolonged pressure from these devices can lead to iatrogenic nerve injuries. CASE PRESENTATION: We present a case of severe axonal injury to the median and ulnar nerves at the proximal hand in a patient following shoulder surgery. This was attributed to palm pressure from a postoperative shoulder splint with a built-in palm stress ball designed to improve circulation and healing, along with impaired pain sensati

Management of Volkmann Ischemic Wrist Contracture: A Literature and Case-Based Review of Treatment Modalities with a Focus on Ilizarov Circular Frames.

Volkmann's ischemic contracture is a disabling consequence of compartment syndrome, characterized by irreversible muscle and nerve damage leading to joint deformities and functional impairment. Traditional surgical approaches such as tendon lengthening, neurolysis, and free muscle transfer are often limited in severe cases, especially when soft tissue is compromised. This article reviews current treatment options, focusing on the Ilizarov circular frame as a minimally invasive alternative for managing complex wrist contractures. A narrative review of the literature was conducted to assess trea

Discovery of Hyperelastic Constitutive Laws from Experimental Data with EUCLID.

Aseptic nonunion is prevalent in orthopedic practice, causing persistent pain and functional impairment. Humeral shaft fractures, accounting for 3-5% of all fractures, have nonunion rates of 2-33% in nonoperative and 5-10% in surgical management. This study, the largest case series on operative management of humeral shaft nonunion, examines surgical techniques and outcomes. This retrospective study, conducted at a referral center in eastern India, included 132 patients with aseptic humeral shaft nonunion from 2002 to 2020. Cases were from May 2002 to April 2012 and May 2015 to December 2020. P

Secondary reconstruction of tendons and nerves improves long-term function in electrical burns of the wrist: A retrospective cohort study.

BACKGROUND: High-voltage electrical burns can significantly damage the tendons and nerves, causing severe upper-limb dysfunction. Although early debridement and free flap coverage are proven techniques for limb salvage, the functional benefit of subsequent secondary tendon and nerve reconstruction has not been adequately investigated. Therefore, in this study, we assessed whether a systematic, staged reconstruction strategy improves long-term functional outcomes beyond those achieved with successful wound closure alone. METHODS: A single-center retrospective cohort study was conducted on 42 p

Late Presentation of Brachial Plexus Traction Injury After Fixation of Chronic Clavicle Nonunion Mimicking Brachial Plexitis: A Case Report and Literature Review.

We report the case of a healthy 56-year-old male who developed severe neuromotor deficits of his upper extremity after two surgeries for a chronic clavicle fracture nonunion. Initial reconstruction with plate fixation achieved anatomic alignment. Twelve days later, diffuse numbness and weakness developed in his ipsilateral hand. A second (and final) clavicle surgery was then performed to increase the subclavicular space, after which his neuromotor deficits dramatically worsened. Because brachial plexopathy, specifically Parsonage-Turner syndrome (PTS), was considered the most likely diagnosis,

Neurologic Complications in Shoulder Surgery: Anatomy, Mechanisms, and Surgical Recommendations.

Intraoperative nerve injuries during orthopaedic shoulder procedures can cause pain, paresthesia, and muscle weakness. Iatrogenic nerve injuries are often transient or subclinical, and it is believed that many neurologic injuries go undiagnosed. Several risk factors predispose patients to intraoperative nerve injury, including local tissue stiffness, revision surgery, low body mass index, medications, and decreased range of motion. Apart from patient-specific risk factors, there are direct and indirect mechanisms of injury related to surgical technique, including device implantation, nerve tra

Technical considerations and contemporary literature review of the Gore Thoracic Branch Endoprosthesis®.

Thoracic endovascular aortic repair (TEVAR) has become the preferred treatment for various thoracic aortic pathologies, including blunt thoracic aortic injury, aneurysms, and dissections. Involvement of Ishimaru Zone 2 occurs in up to 60% of TEVAR cases requiring extension across the distal aortic arch and left subclavian artery (LSA). Coverage of the LSA without revascularization increases the risk of complications such as upper extremity ischemia, stroke, and spinal cord injury. Therefore, the preservation of LSA perfusion during TEVAR is essential. Cervical debranching remains an effective

Safety and feasibility of paired vagus nerve stimulation with rehabilitation for improving upper extremity function in people with cervical spinal cord injury: a pilot randomized controlled trial.

BACKGROUND: Recovery of upper extremity function remains a major unmet need for individuals with chronic cervical spinal cord injury (SCI). Pairing vagus nerve stimulation (VNS) with task-specific rehabilitation has been shown to enhance neuroplasticity and motor recovery in stroke, but evidence in SCI is limited. OBJECTIVE: To evaluate the safety and feasibility of pairing VNS with upper extremity rehabilitation in individuals with chronic cervical SCI, and to explore preliminary effects on upper extremity function. METHODS: This single-site, triple-blind, randomized, sham-controlled pilot

Distal radial artery cannulation in the Hegu (LI4) region for perioperative invasive arterial blood pressure monitoring: two case reports.

BACKGROUND: Invasive arterial blood pressure monitoring is essential for perioperative hemodynamic management. While proximal radial artery cannulation is widely used due to its ease and low complication rate, it is unsuitable for patients with upper-limb trauma, wrist/forearm surgery, local soft-tissue injury, or prior vascular injury from puncture. Distal radial artery access at the Hegu (LI4) region-originally developed for cardiovascular procedures-offers a viable alternative: it lies superficially, is less affected by wrist motion, and benefits from collateral perfusion via the ulnar arte

Innovative intramedullary nail system with internally locking: A safe alternative fixation technique for humerus shaft fracture that reduces the risk of the iatrogenic radial nerve injury and surgical time.

PURPOSE: Humeral shaft fractures may be managed conservatively or operatively; however, increasing non-union and complications after non-operative treatment have renewed interest in intramedullary fixation. Innovative intramedullary nails with distal internally locking, requiring neither fluoroscopy nor external guidance, aim to reduce surgical time, simplify the procedure, and minimise the risk of iatrogenic radial nerve injury. This study evaluates the clinical and radiological outcomes of a humeral nail utilising distal internally locking. MATERIAL AND METHOD: A retrospective review includ

Glypican-3 (GPC3) mRNA and protein are expressed in >80% of human hepatocellular carcinomas (HCC) but not in normal tissues except for placenta and fetal liver. The oncofetal antigen GPC3 is a glycosylphosphatidyl inositol-anchored membrane protein and may be secreted. It is a novel tumor marker for human HCC: GPC3 protein was present in sera from 40-50% of HCC patients, but was not detected in sera from patients with liver cirrhosis or chronic hepatitis, or in sera from healthy individuals. alpha-Fetoprotein (AFP) and PIVKA-II (protein induced by vitamin K absence or antagonist-II), are well

Robot-assisted Axillary Lymph Node Dissection for Axillary Lymph Node Metastasis in Breast Cancer.

Current experience with robotic-assisted axillary lymph node dissection (R-ALND) is limited. Axillary lymph node dissection is essential for patients with pathological N2-3 disease, but conventional open surgery carries a risk of injury to the intercostobrachial nerve, leading to sensory deficits, pain, and reduced quality of life. R-ALND may enhance precision through minimally invasive techniques, potentially reducing complications. Patients with breast cancer who underwent R-ALND at our institution between March 2024 and March 2025 were enrolled and analyzed. The procedure used the da Vinci
