Temporal development of peripheral neuroinflammation in whiplash-associated disorder grade II and its role in chronicity.

Whiplash injuries cause considerable pain and disability. Most individuals are diagnosed with whiplash-associated disorder grade II (WADII), which is defined by the absence of frank nerve injury. However, studies indicate possible peripheral neuroinflammation in some individuals with WADII that may contribute to symptoms. The temporal changes of peripheral neuroinflammation in WADII remain unclear. This study aimed to investigate the course of peripheral neuroinflammation from acute to chronic stages and assess whether neuroinflammation in the acute stage predicts recovery 6 months postinjury.

[Recommendations and Protocols for the Safe Use of Tourniquets in Upper- and Lower-Extremity Surgeries].

The tourniquet (TNQ) interrupts blood flow to a given anatomical segment and has critical applications in orthopedic surgeries by providing a blood-free operating field. The risks and complications attributed to its use are increased pain, reperfusion injury, edema, deep venous thrombosis, and peripheral nerve injury. The main recommendations for TNQ use and to reduce the occurrence of complications include adequate limb padding; TNQ inflation to pressures of 50 mmHg and 100 mmHg above the perfusion pressure for the upper and lower limbs respectively; avoid TNQ use in children and patients wit

Brachial Plexus Injuries Associated with Extracorporeal Membrane Oxygenation : A Report of 3 Cases.

CASE: We present 3 cases of brachial plexus injuries (BPI) secondary to compressive hematomas during extracorporeal membrane oxygenation (ECMO), with follow-up ranging from 18 months to 12 years. Patients exhibited profound upper extremity deficits and underwent variable interventions, including surgical decompression. All demonstrated meaningful neurologic recovery. CONCLUSION: BPI is a rare but serious complication of ECMO. These cases highlight the importance of early recognition and hematoma evacuation, emphasizing the need for clinical vigilance to optimize neurologic outcomes following

Feasibility and Safety of Brachial Plexus Blocks in Patients With Cervical Spinal Cord Injury Undergoing Upper Extremity Surgery: Case Series.

Upper extremity reconstructive surgeries in patients with cervical spinal cord injury (SCI) can significantly improve function, but anesthesia planning and postoperative pain management are complex. Continuous peripheral nerve blocks (CPNBs) may offer intraoperative anesthesia and postoperative analgesia benefits, yet literature on their safety and patient tolerance in those with cervical SCI is limited. This case series reviewed the records of 18 patients with cervical SCI who underwent upper extremity tendon transfer, nerve transfer, or reconstructive surgery and received an ultrasound-guide

Risk Factors of Positional Peripheral Nerve Injury in Robotic Laparoscopic Radical Prostatectomy.

BACKGROUND AND OBJECTIVES: One of the responsibilities of the anesthesiologist is to maintain the physiologic anatomic position during surgery. Postoperative positional peripheral nerve injury (PPPNI) inevitably may occur during robot-assisted laparoscopic radical prostatectomy (RARP) in steep-Trendelenburg-lithotomy positioning. The primary aim of the study was to identify incidence and risk factors for the development of PPPNI in the RARP and the secondary aim was to identify the most common types and duration of PPPNI. METHODS: After ethics committee and patients approval, patients who und

Trans-Ulnar Single Incision Fasciotomy for Decompression of Forearm Compartment Syndrome: A Cadaveric Study.

OBJECTIVES: Acute compartment syndrome (ACS) is a medical emergency that requires timely intervention, and delays in treatment can lead to severe complications such as nerve injury, muscle necrosis, amputation, and even death. Definitive treatment of ACS requires a fasciotomy. Currently, there is no consensus on the best approach for a forearm fasciotomy; compartment release is most commonly done through volar or combined volar and dorsal incisions. A trans-ulnar single incision approach has been demonstrated to be effective in a case report. This study investigates if a trans-ulnar single inc

Tourniquet-related complications in extremity injuries: a scoping review of the literature.

BACKGROUND: Tourniquets are crucial for controlling life-threatening hemorrhage and, therefore, in preventing avoidable deaths in both military and civilian settings. Its increased use since the launch of the Stop the Bleed campaign, however, has raised concerns regarding possible complications associated with its application, including limb ischemia and amputation. The objective of this study was to synthesize the existing evidence regarding complications associated with the use of tourniquets for extremity injuries and identify gaps in knowledge to guide future research on this topic. Do Gunshot and Explosion Injuries Differ in Severity and Management? A Multicenter Study of Upper Extremity Trauma in the 2023 Israel-Gaza Conflict.

BACKGROUND: In 2023, the Israel-Gaza conflict escalated with the October 7, 2023, attack that has led to prolonged warfare. Upper extremity injuries, often resulting from explosions or gunshot wounds, have been a major cause of disability in this conflict. While previous studies have examined blast and ballistic trauma separately, there is a lack of direct, large-scale comparisons of their injury patterns, severity, and clinical management in a real-world conflict setting. It remains unclear whether gunshot wounds or explosion-related injuries result in more intensive surgical interventions or

Comparative Outcomes of Direct Versus Connector-Assisted Peripheral Nerve Repair.

Background: Peripheral nerve injuries affect a significant proportion of patients with upper extremity trauma, with transections frequently requiring surgical intervention. While direct repair (DR) remains the historical standard, connector-assisted repair (CAR) has been proposed to improve functional outcomes by addressing limitations inherent to DR, such as fascicular misalignment and tension at the repair site. Objectives: The purpose of this systematic review is to evaluate and compare the clinical effectiveness and complication rates of DR versus CAR in upper extremity peripheral nerve in

Peripheral Nerve Injuries in Patients Undergoing Congenital Cardiac Surgery and Cardiac Catheterization.

OBJECTIVES: Peripheral nerve injury (PNI) is a clinically significant but frequently under-recognized complication after cardiac surgery and catheterization, contributing to postoperative morbidity. While PNI has been well characterized in adult cardiac populations, evidence in children and adolescents remains limited. This study aimed to describe and review patients with documented PNI following congenital cardiac interventions, focusing on identifying potential risk factors, assessing differences between surgery and catheterization, and evaluating clinical outcomes. DESIGN: Retrospective st

External hinged fixation vs. internal joint stabilization for elbow instability: a systematic review and meta-analysis of functional outcomes and surgical complications.

BACKGROUND: Traumatic elbow instability can be managed with hinged external fixator (HEF) or internal joint stabilizer (IJS). While prior studies report device-related complications with both devices, a comprehensive analysis comparing range of motion (ROM), patient-reported outcome measures, and surgical complications is limited. This study aims to evaluate these outcomes to guide treatment decisions for complex elbow instability. METHODS: This systematic review and meta-analysis was conducted following Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. A comprehe

Successful arm replantation using traditional surgical techniques: a case report from a low-resource country.

INTRODUCTION: Upper limb replantation requires rapid intervention by a multidisciplinary team and appropriate equipment to rescue the limb. Note that the success of the operation and the decision to perform it depend on several factors. This case report presents an upper limb replantation performed using traditional surgical techniques. CASE PRESENTATION: A 12-year-old boy presented to our hospital with a crushing injury to the right upper arm, which was amputated at the lower third of the humerus, with the biceps brachii tendon remaining intact. The first aid included resuscitation and intra

Soft Tissue Reconstruction in the Extremities: Principles, Techniques, and Outcomes.

Soft tissue reconstruction in the extremities is complicated by the fact that injuries often involve composite defects of skin, muscle, tendon, nerve, and bone. In both the upper and lower limbs, achieving reliable soft tissue coverage, restoring function, and reducing complications are critical goals. The upper extremity requires precision and fine manipulation, whereas the lower limb should be reconstructed to support weight-bearing and mobility. Free flaps have become a routine part of the surgical armamentarium, offering flexible solutions tailored to different needs. Donor sites in the lo

Weber Clamp Dorsal Compression Technique for Distal Radius Fracture Open Reduction and Internal Fixation With Case Series.

Open reduction and internal fixation (ORIF) of distal radius fractures (DRF) is a common procedure performed by hand surgeons. Complications include, but are not limited to, failure/loss of reduction, improper positioning of hardware, and nerve or tendon injury. Although uncommon, these risks should be considered when choosing a surgical technique. In this article, we present a novel dorsal compression reduction technique (DCRT) using a Weber clamp and surgical towel for ORIF of DRFs. The DCRT maintains efficacy and efficiency and improves safety by minimizing the risks of penetrating skin or

Limb Salvage and Reconstruction of a Complex Upper Extremity Injury: A Case Report.

INTRODUCTION: Complex upper extremity trauma poses us with significant challenges in orthopedic and reconstructive surgery due to the complex anatomy and functional importance of the extremities. These injuries are often the result of high-energy mechanisms like motor vehicle collisions, which makes their management particularly demanding. Making the decision between limb salvage and amputation in such cases requires careful consideration of multiple factors such as functionality, risk of complications, and recovery. CASE REPORT: We describe the case of a 39-year-old male who had sustained an
