Preventing posterior interosseous nerve injury during a distal biceps tendon repair: an anatomic study comparing extensor digitorum communis and extensor carpi ulnaris muscle splitting approaches.

BACKGROUND: Dorsal forearm approaches are commonly used to repair both partial and complete distal biceps tendon tears. Either the extensor digitorum communis (EDC) or extensor carpi ulnaris (ECU) and the supinator muscles are longitudinally split to expose the radial tuberosity (RT). Despite the known risk of posterior interosseous nerve (PIN) injury, there is little information on the safety of each approach. The purpose of this study was to define the location of the PIN relative to the RT through both EDC and ECU muscle splitting approaches. Additionally, we aimed to find a reliable method

Tunnel-transposition with veno-venous or veno-arterial re-anastomosis versus skin-flap elevation for second-stage superficialization of brachiobasilic arteriovenous fistula.

IntroductionArteriovenous fistula is the preferred vascular access for patients with chronic kidney disease on hemodialysis. Brachiobasilic arteriovenous fistulas are valued for their durability and long-term patency in patients with unsuitable superficial venous anatomy. The basilic vein, located deeper within the arm, offers a larger diameter. However, due to its depth, surgical interventions like superficialization or transposition are necessary to make the vein accessible for routine cannulation. This study compares the outcomes of three different surgical techniques used during the second

Medial Approach Derotational Humeral Osteotomy in Patients with Brachial Plexus Birth Palsy.

BACKGROUND: Medial-approach derotational humeral osteotomy is indicated in patients with brachial plexus birth palsy (BPBP) who have internal rotation contracture, a condition that substantially limits upper-extremity function and creates cosmetic concerns as a result of excessive internal rotation1. This procedure enhances the range of motion of the arm by surgically externally rotating the humerus, thereby facilitating essential activities such as bringing the hand to the mouth and neck without the need for compensatory movements. In addition, the medial approach offers cosmetic benefits; th

Effect of time from injury to surgery on surgical technique and complication rate in distal biceps tendon repair.

BACKGROUND: Surgical treatment options for distal biceps tendon ruptures vary based on time from injury to surgery. While direct repair (DR) is preferred for acute injuries, high flexion angle (HFA) repair and allograft reconstruction (AR) are alternatives for chronic cases. This study examines the relationship between time to surgery, surgical technique selection, and complication rates. METHODS: A retrospective chart review was conducted on patients treated surgically for distal biceps tendon ruptures at a single center from January 2012 to June 2023. Cases were identified through electroni

Digital Nerve Coaptation vs Standard of Care for Partial and Ray Digital Amputations.

BACKGROUND: Traction neurectomy (TN) remains the standard of care for digital nerve management in partial and ray digital amputations but is associated with both neuroma formation and chronic postoperative pain. The purpose of this study is to evaluate if digital nerve coaptation (DNC) is associated with lower postoperative pain and complication rates compared to standard TN. We hypothesize that patients undergoing digital amputations with DNC will demonstrate lower postoperative complications, reduced rates of persistent pain, and fewer reoperations for nerve-related pain. Effect of controlled hypotension on postoperative awakening quality in shoulder arthroscopy with lateral decubitus rotator cuff repair.

BACKGROUND: Background: Shoulder arthroscopy is commonly performed to treat shoulder injuries, degenerative conditions, and chronic pain. The quality of postoperative emergence is critical to patient recovery and overall surgical outcomes. However, few studies have investigated whether controlled hypotension affects emergence quality during shoulder arthroscopy. Controlled hypotension can optimize the surgical field, reduce operative time, decrease the risk of intraoperative complications, and enhance the quality of postoperative recovery. These benefits highlight its clinical utility and supp

[Operations in patients with giant false aneurysms of upper-limb arteries (clinical case reports)].

False aneurysms of upper-limb arteries are rare pathological conditions in vascular surgery. They are typically caused by various injuries, more often iatrogenic, after endovascular interventions. These pseudoaneurysms can be complicated by ruptures, compression of nerves and nerve plexuses, as well as embolization of the distal arterial bed with the development of limb ischemia. In this article, we summarize the experience of surgical treatment of false aneurysms of upper-limb arteries and describe two clinical case reports of successful surgical management of giant pseudoaneurysms of the bra

Clinical outcomes of reverse shoulder arthroplasty in obstetric brachial plexus palsy with suprascapular nerve injury and glenohumeral osteoarthritis.

BACKGROUND: Untreated obstetric brachial plexus palsies (OBPP) present with various upper limb impairments, including muscle weakness, soft tissue contractures, and osteoarticular deformities. Reverse shoulder arthroplasty (RSA) medializes the center of rotation, optimizing deltoid function to compensate for deficits caused by OBPP. This study evaluates the effectiveness of RSA in patients with untreated OBPP involving the suprascapular nerve and end-stage glenohumeral osteoarthritis (OA), focusing on functional and quality-of-life improvements. METHODS: A retrospective analysis was performed

Axillary to Radial Nerve Transfer for Elbow Extension in the Tetraplegic Patient: A Case Series.

BACKGROUND AND OBJECTIVES: Recovery of upper extremity function is considered the top priority in the tetraplegic population. Midcervical injury often results in paralysis of the hand and loss of elbow extension. Elbow extension is essential for extending one's workspace and facilitating mobility (ie, self-transfer, wheelchair propulsion). Tendon transfers have traditionally been used to restore elbow extension, but the axillary to radial nerve transfer (ARNT) has emerged as a promising alternative. Our objective was to review a case series of single-surgeon outcomes for recovery of elbow exte

Efficacy of Suprascapular Versus Interscalene Block for Post-Operative Pain Management in Shoulder Surgeries: A Narrative Review.

PURPOSE OF REVIEW: Analgesic techniques which provide sufficient post-operative shoulder pain relief while minimizing complications is crucial for helping patients achieve optimal recovery. The interscalene brachial plexus block (ISB) has long been considered the gold standard in terms of providing patients with adequate pain control following shoulder operations. However, ISB's expansive neural blockade poses several potential risks, including delayed sensorimotor rehabilitation to the upper extremity, partial blockade of the cervical sympathetic chain, and, most notably, diaphragmatic paresi
