A review of the literature reveals that with conventional treatment alone or in combination with secondary muscle/tendon transfer, about 4-43% of cases show incomplete recovery with severe functional and/or aesthetic impairment (group III). If these patients undergo early microsurgical brachial plexus revision, regeneration without significant functional and/or aesthetic impairment (shift from group III to group II) can be achieved in 80-90% of cases. Moreover, microsurgical reconstruction of the brachial plexus increases the possibilities of secondary muscle/tendon transfers. Therefore, provi

Elbow flexion plays a key role in the overall function of the upper extremity. In the case of unilateral complete brachial plexus lesion, restoration of elbow flexion will dramatically increase the patient's chances of regaining bimanual prehension. Furthermore, depending on the type of reconstruction, stability of the glenohumeral joint as well as some supination function of the forearm can be restored to a varying degree at the same time. Depending on the level of brachial plexus lesion and/or reinnervation, different reconstructive procedures are available. In order to select the best treat

The field of MR imaging of the postoperative shoulder continues to evolve. As the indications expand for obtaining postoperative shoulder studies, there arises a need for delineation and definition of criteria associated with these imaging studies. Further investigations are necessary to analyze these data in greater detail, and to correlate these data to clinical situations.

We report 17 cases of abnormal pinch due to anterior interosseous nerve (AIN) palsy. The AIN lesion was complete with palsy of flexor pollicis longus (FPL) and Flexor digitorum profondus of the second digit (FDP2) in nine cases. A partial lesion with FPL palsy was observed in four cases, and FDP2 in four other cases. Three of this eight cases were initially considered to be tendon ruptures. Electrodiagnosis assessed AIN lesion and respect of the main median trunk in all cases. Pronator quadratus examination provided the diagnosis in 14/17 cases and FPL or FPD2 examination established the diagn

PURPOSE OF THE STUDY: The authors have reviewed 67 axillary nerve lesions from 1987 to 1993. 35 times the lesion was isolated; 20 times it was associated to other nerve lesions (9 times supra scapular nerve, 3 times musculocutaneous nerve, 8 times posterior cord) and 12 times lesions were associated to rotator cuff injury.
DISCUSSION: Injury corresponded 9 times to stretching mechanism in upper limb traction and came 58 times with osteoarticular lesions (30 anterior dislocations and 28 shoulder fractures). In 11 cases, shoulder active abduction was normal, inspite of deltoïd's complete palsy

Hand and upper extremity injuries to the professional or serious amateur musician may cause significant disability and time away from one's instrument. This article reviewed 222 instrumentalists; 201 were followed to an end result of their injuries. 80% played strings or keyboard instruments. Sports or a direct fall were the most common causes of injury. The spectrum of diagnoses made was typical of hand trauma in general; fractures, sprains, muscle strains and lacerations were the most numerous. Final results after treatment in 201 patients included complete relief of symptoms in 137 (68.2%)

Anatomical and clinical research has shown that the entire lateral epicondylar region is innervated only by radial nerve branches. Based on these investigations we have developed a surgical procedure for complete denervation which is indicated only in resistant cases of tennis elbow. Only one nerve pathway calls for careful exposure, otherwise denervation is accomplished blindly by disinsertion of certain muscles. The result of this procedure also depends on simultaneous indirect decompression of the posterior interosseous nerve. Excellent or good results were obtained in 90% on average. Resul

Tendon transpositions serve to restore functions which were lost through an injury of nerves or muscles. At the Department of Hand Surgery and Plastic Surgery at the Berufsgenossenschaftliches Unfallkrankenhaus Hamburg, 116 tendon transpositions were carried out for radial paralysis between 1960 and 1989. The follow-up for 85 patients is presented and the evaluation scheme developed by Haas (1991) further modified and our scoring system compared to others such as Zachary (1946) modified by Neumann and Partecke (1980). The results were good, also when evaluated according to Riordan (1959) and M

Spinal cord injury at the C5 and C6 level results in loss of hand function. Electrical stimulation of paralyzed muscles is one approach that has demonstrated significant capacity for restoring grasp and release function. One potential limitation of this approach is that key muscles for stimulation may have lower motor neuron damage, rendering the muscles unexcitable. We have used surgical modification of the biomechanics of the hand to overcome this limitation. Tendon transfer of paralyzed but lower motor neuron intact muscles can compensate for potential function lost owing to muscles with lo

In order to find a direct approach to the antero-inferior third of the glenoid rim, an anatomic study was performed on a total of 89 shoulders (48 cadavers). To obtain defined reference points for the anterior inferior third of the glenoid cavity, it was compared with the hour markings on a clock face. The 4:30 position on the right shoulder and the 7:30 position on the left shoulder were defined as the relevant reference points. The average distance between the palpable end of the coracoid process and the 4:30 and 7:30 positions was 19 mm. The average distance to the point of intersection of

Magnetic resonance imaging provides clinically useful information in assessing the elbow joint. Superior depiction of muscles, ligaments, and tendons as well as the ability to directly visualize nerves, bone marrow, and hyaline cartilage are advantages of magnetic resonance imaging relative to conventional imaging techniques. Ongoing improvements in surface coil design and newer pulse sequences have resulted in higher quality magnetic resonance images of the elbow. Traumatic and degenerative disorders of the elbow are well seen with MR imaging. The sequelae of medial traction and lateral compr

Fifty cases in 48 patients of intractable medial tennis elbow tendinosis (medial humeral epicondylitis) were treated surgically from 1985 to 1990 with identification and excision of the injured tendon, while retaining and closing the resection defect. All patients had symptoms that were aggravated by repetitive upper extremity activities and had failed to improve with nonoperative therapy. At surgery, the flexor carpi radialis-pronator teres interval was involved in 28 cases. Histologic examination revealed angiofibroblastic tendinosis and fibrillary degeneration of collagen. Postoperative fol

An aberrant muscle is demonstrated by magnetic resonance (MR) imaging in a patient presenting with focal wrist swelling and compression neuropathy of median and ulnar nerves following 4 months of carpentry work. The muscle originated from the palmaris longus tendon and ulnar antebrachial fascia at the lower half of the forearm as a single belly, then diverged medially from palmaris longus tendon and bifurcated. Both portions of the split muscle extended into the distal ulnar tunnel or Guyon's canal. One segment joined with the abductor digiti minimi muscle and the other with the flexor retinac

Radial tunnel syndrome results from compression of the radial nerve by the free edge of the supinator muscle or closely related structures in the vicinity of the elbow joint. Despite numerous reports on the surgical management of this disorder, it remains largely unrecognized and often neglected. The symptoms of radial tunnel syndrome can resemble those of tennis elbow, chronic wrist pain or tenosynovitis. Reliable objective criteria are not available to differentiate between these pathologies. These difficulties are discussed in relation to 29 patients who underwent 30 primary explorations an

Five hundred forty-one patients with reflex sympathetic dystrophy (RSD) of the upper extremity were prospectively studied. One hundred fifteen patients complained of pain and/or limited range of motion in the shoulder. Shoulder complaints more often occurred in women (p = .01); age and etiology were not different from patients with RSD without shoulder complaints. Physical examination showed a tendinitis of one or both tendons of the biceps muscle in 109 patients. Seventy one patients were treated with local injection of bupivacaine followed by methylprednisolone. This resulted in permanent re

Free muscle transplantation with motor innervation is the only way to add contractile elements to upper extremities with extensive loss of musculature due to direct trauma or untreated compartment syndrome (Volkmann's contracture). The functional cross-sectional area and the mean resting fiber length determine the maximum power and the contracting amplitude of the donor muscle, respectively. Although considerably weaker than the finger flexors to be replaced, the gracilis muscle was the preferred donor muscle because of the consistent anatomy of its neurovascular pedicle and the minimal donor

Disturbances of the stabilizing musculature are an important factor in painful shoulders. The important changes in the rotator cuff muscles are well established, but less attention is put on possible disturbances of the deltoid muscle. This study investigated the effect of shoulder impingement on muscle fiber cross-sectional area and the Na,K-pump concentration of the deltoid muscle. The study included six patients with impingement syndrome. Biopsies were taken from the affected and unaffected sides. The muscle fiber cross-sectional area and the Na,K-ATPase concentration was measured. There we
