Since Roles and Maudsley's publication, in 1972, it has been admitted that the entrapment of the posterior interosseous nerve (PION) is a possible cause of lateral elbow pain. In the radial tunnel, at least 4 or 5 compressive structures have been described. The arcade of Frohse is a well known compressive cause while the medial edge of the extensor radialis brevis (ECRB) muscle is less frequently incriminated. An anatomic study of 45 supper limbs from 40 cadavers was performed to analyse the relations between the medial edge of the ECRB and the PION. The medial edge of the ECRB was a real fibr

In the last 15 years the methods of examination of the shoulder have intensively expounded. According to a well documented retrospective study we describe the value of the shoulder arthroscopy and its correlation to MRI and arthro-CT findings. MRI represents nowadays the best way of exploring rotator cuff lesions and of investigating an impingement syndrome. Associated with a contrast medium its ability to clear up an anterior or posterior shoulder instability is now superior to the arthro-CT's. The arthroscopy allows the dynamic exploration of the joint and can be used not only as a diagnosti

Permanent deficiency in abduction and external rotation of the arm seen after brachial plexus injury may be corrected by a levator scapulae transposition onto the supraspinatus, while the teres major, associated or not with latissimus dorsi, is transferred onto the infraspinatus muscle. Transposed in a bipolar manner, the latissimus may compensate the deltoid muscle. Associated measures have to be carried out in children with deformities caused by obstetrical palsy. A paralysed serratus anterior is best compensated by a transfer of the pectoralis major together with pectoralis minor muscle.

This is a retrospective review of 29 patients (33 hands) who underwent a palmaris longus transfer because of severe thenar atrophy secondary to median nerve entrapment at the wrist. The mean follow-up was 17 months. Ninety-four percent of our patients were satisfied because their thumb function improved. Twenty-six of the patients had the transfer at the time of initial release of the carpal tunnel, and three patients had the transfer when the carpal tunnel was released a second time. The transfer helps with thumb palmar abduction, and the palmaris longus is an expendable muscle for transfer.

Fourteen patients with epicondylitis humeri radialis, resistant to conservative therapy were surgically treated according to Wilhelm's procedure at the II. Department of Plastic Surgery, University of Vienna. The mean follow-up period was 7.6 years (1 to 14 years). This study demonstrates a marked improvement of the symptoms after operative treatment, hence it follows that Wilhelm's procedure can be recommended in patients resistant to conservative therapy.

Twenty-four cases of flexor carpi radialis tendinitis were diagnosed between 1985 and 1989 by SOS Main Strasbourg. This condition typically occurs in women (75%) with a mean age of 47 years with no particular occupational activity who present with spontaneous and induced pain along the distal part of the tendon. Examination may reveal disturbances of cutaneous sensation in the territory of the palmar cutaneous branch of the median nerve and associated signs: cysts (10/24 cases), scapho-trapezium osteoarthrosis (3 documented cases). Medical treatment provided good results in 7 out of 9 patients

Avulsion of the subscapularis tendon is caused by a combined abduction and external rotation trauma of the upper limb. Weakness of internal rotation and a positive apprehension test are clinical signs of the injury. A fracture of the lesser tuberosity in the x-ray film or a disruption of the subscapularis tendon documented in dynamic sonography of the shoulder clarify the diagnosis. Operative treatment is indicated of prevent weakness of internal rotation as well as anterior instability. This recommendation is supported by four cases with full range of motion and good anterior stability after

In established compartment syndrome discrimination between the different forms of flexion contracture, i.e., manifest Volkmann's contracture and intrinsic contracture is necessary. A combination of both is also possible. Classification is essential for determination of whether reconstruction is indicated and what procedure should be selected. Shortening osteotomies of the ulna and radius are now of historical interest only, as is carpalectomy. Lengthening of the flexor tendons is indicated only in mild and localized limited contracture of only some of the long fingers, but there is a danger of

Results of long-term follow up of 66 patients with ischemic contracture of the forearm and hand, all surgically treated in the "Unfallkrankenhaus Hamburg)) between 1961 and 1982, are presented. Whereas ischemic contracture of the forearm flexors resulted mainly from fractures, ischemic contracture of the intrinsic muscles of the hand was most often seen after pressure injuries. All patients in this study presented to us with fully established ischemic contractures. Muscle and nerve damage was retrospectively evaluated according to operative notes, and the degree of damage could be classified i

Avulsion amputations of the thumb are generally thought to have a worse prognosis after replantation than other amputations. We report the results of 17 thumbs that had an avulsion amputation and were replanted. Fourteen of the 17 survived (82 percent). Our experience indicated that the survival rate was improved by restoring continuity of at least two veins and two arteries, using a Y-shaped vein graft and the princeps pollicis artery for the source of arterial circulation. Nerve grafts were used to bridge defects in avulsed digital nerves. When possible, avulsed tendons were reattached to th

Muscle spasticity and contractures in the spinal cord injured are a big problem interfering with rehabilitation, leading to inconveniences and complications in these patients. Management is based on pharmacotherapy, physiotherapy and surgeries. The authors are against central neurosurgery except on rare occasions. They have been practicing peripheral surgeries chiefly on muscles and tendons with satisfactory results in selected cases. The guidelines and procedures are presented.

Control of elbow motion was evaluated in 45 extremities of adults with spasticity resulting from traumatic brain injury with use of dynamic electromyography. Simultaneous recording of elbow motion was obtained using a double parallelogram goniometer. Thirty-four male and 9 female patients were studied. Mean elbow flexion was 85 degrees and mean extension was 20 degrees. The average time of elbow flexion was 1.8 seconds. Extension time was prolonged to a mean of 4.0 seconds. Dynamic electromyography revealed a consistent pattern of muscle activity. Severe spasticity was noted in the brachioradi

Ten patients with spastic wrist flexion deformities secondary to traumatic brain injury were evaluated for carpal tunnel syndrome. The angle of wrist flexion deformity averaged 75 degrees (range, 58 to 115 degrees). Nerve conduction studies demonstrated prolonged median motor and/or sensory latencies in all patients. Preoperative wick catheter measurements of carpal tunnel pressures in eight patients averaged 11 mm Hg in the resting position, 21 mm Hg in maximal wrist flexion, and 15 mm Hg in maximal extension. Each patient had carpal tunnel release with simultaneous wrist and finger flexor te

Sural nerve grafts were used in three patients to bridge gaps measuring 2 1/2 to 4 1/2 cm in previously transected but not repaired posterior interosseous nerves. The grafts were done at 4, 5, and 7 months after injury. Extension was weaker in all three than on the uninjured side, but the range of motion was complete, with the exception of a slight lag of index finger extension and partial return of extension of the extensor carpi ulnaris in the same patient. All three patients recovered full function of the involved hand 1 year after grafting. The properties of the posterior interosseous nerv

The latissimus dorsi myo-cutaneous flap has been used for restoration of elbow flexion in 8 cases. The indication for this transposition were brachial plexus injuries, significant trauma to the upper limb and restoration of arm contour. The surgical technique involved unipolar muscular transposition (4 cases) with tendon plication (one case) and bipolar transposition (three cases). The evaluation of results is based on the force and degree of elbow flexion, active pronation and supination, as cortical integration. We consider that we have obtained good results in 5 cases and moderate results i

The author analyses the results of treatment of 26 patients aged 5 to 15 with consequences of birth injury of the brachial plexus of lower and mixed types. The main task in the treatment of this category of the patients is the restoration of the hand function which is achieved by means of tendon--and myoplasty and derotational osteotomy of the humerus or the bones of the forearm or by their combination. Diversity of clinical manifestations determines a strictly individual approach to the treatment of every patient. In stage-by-stage treatment of the consequences of birth injury of the brachial

The author reports on a 42-year-old male patient who presented in our clinic with swelling over the left wrist bones after a fall. Dorsal and palmar wrist movement was painful, yet nearly equal on both sides. The X-ray showed an isolated osseous tendon rupture of the extensor carpi radialis brevis muscle at the base of the third metacarpal. This finding was confirmed upon surgical exploration, and a traction band was applied. After osseous healing of the fragments, the metal was removed early due to irritative synovialitis. Thereafter, the patient's wrist mobility soon became free and undistur

The authors analyse the treatment of 153 children aged 4.5-15 who were performed 213 tendon- and myoplasties for persistent lesions of the peripheral nerves. The peculiarities of rehabilitative therapy in this category of patients are described. Three groups of muscles used for grafting and requiring special approach during the rehabilitation period have been distinguished. The rehabilitative treatment is divided into three periods: preoperative preparation, working out of the new function and securing of the new motor act. The peculiarities of the rehabilitative treatment in each period have

Eleven people with spinal cord injury (SCI) (C4-T6) participated in a program of functional electric stimulation (FES) of their paralyzed leg muscles using the REGYS I system. Individualized protocols consisted of an initial phase of weight lifting, an intermediate phase of ergometer pedalling against 0 Kilopond (kp) alternated with 1/8kp for six two-minute runs separated by two-minute rest periods, and a final phase of 36 sessions of continuous ergometer pedalling against variable resistance. A metabolic analyzer measured exercise stress test parameters before and after each phase while subje
