Spasticity that interferes with upper extremity function is common in adults following stroke, brain injury, or anoxia. During the period of neurologic recovery definitive surgical procedures are avoided. Techniques to temporarily reduce spasticity include the implantation of a MicroPort reservoir and catheter for repeated branchial plexus blocks and phenol nerve blocks, which provide longer lasting relief of noxious muscle tone. Percutaneous blocks of the musculocutaneous and recurrent median nerves and motor point blocks of the pectoralis major, the brachioradialis, and forearm flexor muscle

Thirty-one patients who had transfer of the flexor digitorum superficialis tendons to the flexor digitorum profundus tendons en masse in thirty-four non-functional spastic hands were examined at an average of fifty months postoperatively. All of the patients had had a clenched-fist deformity preoperatively, with severe hygienic problems of the palmar skin and no active function of the hand. Postoperatively, all of the hands were in an open position, which allowed for good hygiene of the palmar surface. A minor wound infection developed in three patients. Neurectomy of the motor branch of the u

Palliative treatment of palsy of intrinsic muscles of the four fingers classically aims at suppressing claw. But functional restoration is mainly ensured in setting up again the volar flexion, the stage preparing prehension. Three hundred and thirty seven hands have been operated. Each one of the utilized interventions has advantages and drawbacks that have to be known in order to select the best adapted technics, the more or less important stiffness of fingers being the most important criteria. Finally a simple choice is easy and possible, particularly for inexperienced surgeons in the treatm

A 7-year-old boy who suffered from carpal tunnel syndrome caused by aberrant lumbrical muscles associated with cystic degeneration of the tenosynovium of the left hand is described. Judging from the literature studied, this seems to have a very rare origin. Symptoms subsided, and no recurrence has been noticed after operative removal of these structures.

Nonsurgical problems and complications of orthotist-prosthetist techniques are not addressed in this article. There are a number of complications, such as decubiti, joint contracture or damage, tendon injury, wound healing, etc., which are not discussed because they are similar in cause, identification, and treatment to the same adult problems. Though it is possible to list the more common complications seen for each congenital anomaly, the tedious repetitiveness of such an approach dissuaded the author. General principles can always be specifically applied when the occasion requires. It is al

A supination contracture of the forearm is a severe handicap in paraplegics as well as in brachial plexus pareses, e.g. in operating wheelchairs, supporting the upper part of the body and other daily tasks. For restoration of the pronation of the forearm, Zancolli et al. have suggested transfer of the biceps tendon. Since 1976, we have carried out this operation on 16 patients. These cases involved plexus pareses due to birth trauma, high transverse lesions of the cord with paraplegia. In two patients, there was spastic hemiparesis. The results are presented and discussed. Transfer of the posterior deltoid muscle to the triceps insertion for elbow extension provides improved function in patients with C5 and C6 level tetraplegia. We have modified the surgical technique using the tibialis anterior tendon as a graft. The length-tension characteristics and available amplitude of the posterior deltoid muscle were determined with intraoperative electrical stimulation. The excursion of the posterior deltoid muscle was 7.31 cm with a standard deviation of 1.23 cm. Postoperative mean torque measurements of elbow extension power were 36.4 kg cm with a standard deviation o

Shoulder pain is probably the most frequent complication of hemiplegia. In this study 219 hemiplegia patients were regularly followed up after their cerebrovascular accident (CVA) for one year (166 men, 53 women, with a mean age of 47 years). Criteria and parameters for evaluation of these shoulders were established at the outset. Distinction was made between flaccid and spastic hemiplegia. Other influencing factors were subluxation reflex sympathetic dystrophy syndrome (RSD), isolated tendon lesion cuff rotator tear or association of some of these. Roentgen examinations were done for each pat

Gouty tenosynovitis can present as an infection, tendon rupture, nerve compression, or digital stiffness. In ten patients, extensive urate deposition was encountered in the extensor tendons at both the wrist and digital levels in addition to involvement of the flexor tendons in the carpal canal and digital theca. Direct nerve or muscle involvement was not observed in the hand. Medical therapy, which is now the cornerstone of treatment for most aspects of gout, may not be the best treatment for tophaceous deposits in the hand. Operative treatment may be required to debulk tophaceous deposits, i

The occurrence of the anomalous Palmaris Profundus muscle in association with median nerve compression in the wrist in two patients is described. In one of the patients this muscle was a substitute for an absent Palmaris Longus in the left hand. In the right hand, and in the left hand in the second patient, this anomalous muscle was present in addition to the normal Palmaris Longus muscle.

Within a period of twelve years, the authors have treated twelve patients with metacarpophalangeal joint luxations of thumb and long fingers which could not be reposed by conservative treatment. The case reports are presented in order to describe the problems posed by this relatively rare injury, the reasons hindering a reposition, and the treatment methods. This study is intended to underline the importance of early diagnosis and immediate surgical treatment in order to maintain the function of the joint.

A case of isolated traumatic avulsion of the tendon of extensor carpi radialis longus (ECRL) is described. Surgical repair was attempted 12 days later, but an early contracture of the muscle was encountered. The tendon could not be pulled down to its insertion and was sutured side to side to the tendon of extensor carpi radialis brevis (ECRB). A year later the patient had residual weakness of wrist extension and hand grip. The likely cause of the early contracture of the muscle and the need for early and accurate reposition of extensor carpi radialis longus tendon are described.

A case of posttraumatic syringomyelia which appeared 26 years after the injury was presented. A patient was 61 year old female, who sustained thoraco-lumbar spine injuries rendering her to paraplegic in 1954. Eleven months later she had an operation of T6-T9 and L1-L2 laminectomies and regained motor and sensory functions of the both lower extremities. She was ambulatory with crutches till 1979. In 1980, burning pain was noticed in the left scapular region, and thereafter, extended to the ulnar side of the left forearm. The pain became progressively worse and intractable. Analgesics were ineff

Wrist pain may originate from problems of the arteries, nerves, muscles, tendons, or ligaments and from injuries to the bones or joints that compose the wrist. Diagnosis of the exact cause of pain is made by carefully assessing the function of each structure of the wrist. Treatment for wrist pain is primarily surgical but may also include the use of drugs, application of splints, and other therapies.

A 26-year-old male patient developed a writer's cramp syndrome, the underlying cause of which was shown to be a congenital subluxation of the first metacarpophalangeal joint. The clinical symptoms are presented, the indications for surgery reviewed and the operative technique in accordance with A.N. Witt described. Post-operatively, the pathological hypermobility of the first metacarpophalangeal joint has been eliminated, and global function of the hand has normalized.

This paper reports four cases of isolated compression of the deep branch of the ulnar nerve, and reviews the various etiologies (tumor, malformation and microtraumatism). The authors stress the importance of the electric examination, particularly conduction velocity in the palm in localizing the nerve lesion. Surgical exploration in these cases revealed a Giant cell tumor, two synovial cysts, and an abnormal fibrous band stretching between the flexor digiti quinti brevis and the opponents digiti quinti. Postoperative courses were uncomplicated and every patient recuperated quickly and complete

The care of the patient with a crushed upper limb requires accurate assessment, carefully planned and executed surgical procedures, diligence, and reassessment. Often, a long period of rehabilitation is also needed. The quality of care rendered by the surgeon and therapist often affects the patient's functional end result and his chance to regain his ability to provide for himself independently.

The ischemic compartment syndrome is thought to develop following an unphysiological pressure increase in one of the numerous muscular compartments of the extremities, confined by bone surface and strong fascia. Pressure-induced ischemia may cause partial or total necrosis of the vulnerable structures in the compartment. This ischemic compression syndrome was found in the deep extensor compartment of the distal forearm in a young female patient following osteotomy of the ulna. The syndrome at this rather unusual site was not diagnosed and treated in the acute phase. Resection of fibrotic, cont
