Overall survival rate for 143 digits with complete amputation of the distal phalanx was 78%. Replanted digits that underwent venous anastomosis showed a very high survival rate of 93%. Loss of the distal interphalangeal joint function in subzone IV was significantly inferior to that in subzones II and III. Protective sensation was achieved in 96% of replanted digits. Sensory recovery in the absence of nerve repair was significantly worse for avulsion injury than for crush injury. Nail deformity tended to be increased for replanted digits in subzone III or with crush-type injury. Successful ven

PURPOSE: When the index finger is injured or severed in conjunction with a traumatic amputation of the thumb, transfer of the injured index finger can restore the important function of the thumb. The purpose of this study is to evaluate the results of the transfer of an injured index finger for traumatic loss of the thumb.
METHODS: Seven patients treated by pedicled transfer of a traumatized index finger after amputation to the ipsilateral thumb were reviewed retrospectively. Postoperative evaluations included thumb range of motion, opposition and pinch function, grasp and pinch strength, sen

Death of tissue and/or deep infection leading to amputation is not an uncommon course of events after massive crush injuries of the central part of the hand. Management of this injury faces the dual problem of having to carry out debridement in the central part of the hand which is radical enough to remove all dead tissue but which, in itself, creates a huge dead space in the depths of the wound. Inadequate debridement and/or leaving a dead space which fills with fluid and detritus behind the flexor tendons leads on to infection, devascularisation of the fingers and amputation. BACKGROUND: Thumb defects distal to the interphalangeal joint do not cause any disability; therefore, any consideration to reconstruct the thumb is governed by the lifestyle and cultural background of the patient. This study presents the excellent results achieved by immediate partial great toe-to-hand transfer to reconstruct acute composite defects of the distal thumb.
METHODS: Fifty-three patients with amputation or crush injury of the distal thumb who underwent partial great toe-to-hand transfer at the authors' institute over an 11-year period were reviewed. Based on the amputation level o

Beau's lines are transverse depressions or ridgings of the nail surface that are the result of the temporary arrest of nail matrix formation. They are caused by systemic diseases, local trauma, drugs, and infection. Generally, Beau's lines after a fingertip injury occur on only the affected finger, not on the unilateral extremity. In this report, we present the case of a 36-year-old man who developed transverse ridgings in the nail plates of all five fingers of the affected extremity after a fingertip (thumb) crushing injury. During the follow up, the growth rate of affected fingernails was no

The medical records of 186 patients seeking treatment for landmine injuries in the authors' region between 1993 and 2001 were evaluated. Of these patients 13 died of accompanying complications. Ten (10) patients with general body trauma and upper limb trauma were excluded from the study. Of 163 patients with lower-limb injuries included in the study, 21 with traumatic amputation underwent surgical amputation at different levels. Patients without traumatic amputation were divided into 2 groups. There were 41 patients (29%) in Group I who were treated by limb salvage procedures. Treatments used

PURPOSE/BACKGROUND: What are the options for early diagnosis, treatment and prevention of the occlusion of microvascular anastomosis after replantations? METHOD AND (CLINICAL) MATERIAL: This is a retrospective analysis of 336 cases involving primary microvascular repair after an injury between 1984 and 2000. In 272 cases this was a revascularisation and in 187 a replantation. In 23 cases the vascular anastomosis was located proximal to the wrist (macroreplantations) and 313 times distal to it (microreplantations/-revascularisations).
RESULTS: Vascular complications developed in seven and 54 c

Complex injuries of the distal forearm and the hand by industrial machines often require extensive reconstructive procedures. Crush injuries with soft tissue damage extending over the limits of visible injury require a special approach. Large soft tissue defects often have to be covered by extensive flap procedures. The optimum point of time for plastic reconstruction is an essential question and represents a controversial issue in the literature. To be able to compare different patient cohorts in the future, we classify crush injuries into five clinically relevant categories. In the course of

We report our results using University of Wisconsin (UW) cold storage solution in two cases of major replantation of the upper extremity. We employed a technique of a complete vascular washout with 1000 ml UW solution at 4 degrees Celsius at 120 cm hydrostatic pressure into the artery via a size 18 cannula. The ischaemic time of the amputated limbs were 7 hours and 11 1/4 hours respectively. No fasciotomy was performed upon revascularisation and minimal swelling was observed postoperatively. No post-ischaemic syndrome was encountered following the replantation surgery. With a follow-up of 10 a

The main problem in avulsion or degloving injury is the extensive damage to long segments of vessels which makes direct suture of the structures difficult. Various techniques have been proposed over the years to bypass the segment of an injured vessel. Many of these are difficult and often require the execution of several vascular anastomoses. The transfer of vessels from adjacent fingers, as proposed by Doi, is a valid alternative. During the 5-year period from 1988 to 1993, vessel transfer from adjacent fingers was carried out on 15 patients. Three patients suffered incomplete amputation and

From May 1986 to December 1992, we performed 36 major limb replantations in 35 patients; 30 were complete and 6 were incomplete amputations. Twenty-eight of these patients were male. Their ages ranged from 5 to 81 years, the average being 22 years. The levels of injury were 13 hemihand, 8 wrist, 5 lower forearm, 5 upper forearm, and 5 above the elbow amputation. The degrees of injury were 8 guillotine, 14 local crush, 9 diffuse crush, and 5 avulsion amputation. There were a partial loss of digits in 4 hemihand amputations and one failure in an above-elbow amputation--a limb survival rate of 95

From 1975 to 1988, replantation operations of fingers or hands were performed in 2,040 patients at the Division of Plastic and Reconstructive Surgery, Technical University Munich, Germany. The meticulous repair of blood vessels is still the most important step for a successful replantation. The liberal use of vein grafts very often makes replantations possible even in avulsion and crush injuries. The best functional results are achieved in replantations of distal finger parts, because an intact proximal interphalangeal joint and tendon apparatus guarantees almost a full range of motion. Replantation of severed fingers or hand segments has become a routine procedure. Up to now the following clinical conditions are accepted as clear indications: 1. Amputation at the wrist or metacarpal level. 2. Amputation of several fingers. 3. Thumb amputation. 4. Amputation in childhood. 5. Special hand functions. Our clinical results in 1123 patients suffering from 973 total and 899 subtotal amputations are presented. Due to frequent employment of micro-venous interposition grafts success rates of about 80% could be obtained even in cases of crush or avulsion injuries. Recommendations conce

The results of treatment of 36 patients injured during the earthquake in Armenia are analyzed. The patients had severe traumas: open injuries to the limbs, complicated by nephro- and hepatopathies. The periods of the limb crushing with fragments of ruined buildings varied from 3 hours to 3 days. The treatment modalities included conservative and operative detoxication (hemoperfusion, hemodiafiltration, hemofiltration, prolonged endolymphatic infusions of drugs), active surgical strategy, rehabilitation of patients. Seven amputations, 7 reamputations, and 21 necrectomies were performed in 36 pa

In the 10 months since the establishment of a 24-hour replantation service, the centre at the Klinikum rechts der Isar of the Technical University of Munich has treated 162 amputation injuries; 132 underwent replantation surgery and 117 of these healed successfully. Surgical technique, medication and postoperative treatment with physiotherapy are described. Special emphasis is placed on the difficulties associated with crush and avulsion injuries; in most of them the only solution is an intermediate vein graft to bridge the defect in the vessels.

Experimental studies on rabbit ears amputated by either a clean sharp division or severed by a crushing blow showed that twice as much blood remained in the crush group. Microthrombi and tissue thrombi were seen in this group only. A review of 142 digital replantations performed over a 10 year period showed 126 survivals. Ninety-three were complete amputations and 80 of these survived; 49 were incomplete amputations and 46 survived after revascularization attemps. Of 74 clear amputations, 68 survived; of 68 crush-type amputations, 58 survived. Circulatory disturbances occurred in 36 replanted
