Uvular injury: A narrative review.

Uvular injury, although rare, is a clinically significant complication of airway and pharyngeal manipulation and procedural intervention across multiple specialties including anesthesiology, otolaryngology, gastroenterology, obstetrics and gynecology, pediatrics, emergency medicine, and infectious disease. This narrative review comprises case reports and series from MEDLINE and Embase databases and Google Scholar from 1978 to 2025 outlining the embryological origin and anatomy of the uvula, the pathophysiology and management of uvular injury, and preventative measures. Common etiologies can be

Ground-truth encoding of self-motion in the primate cerebellar nodulus and uvula.

Accurate internal estimates of self-motion and orientation relative to gravity are fundamental for stabilizing gaze, controlling posture, and navigating through dynamic environments. Prevailing theories propose that the cerebellar nodulus and uvula (NU) employ internal models to suppress sensory input arising from predictable, self-generated motion. However, this assumption has never been directly tested. Here, we recorded NU Purkinje cell activity in rhesus monkeys during active and passive head movements. We found neurons responsive to passive translations remained equally sensitive to self-

Feasibility of Imaging the Uvula at the Midtrimester Anomaly Ultrasound: A Retrospective Study.

OBJECTIVES: The fetal palate is not routinely imaged as part of the midtrimester fetal anomaly ultrasound, despite being associated with many syndromes. The "equal sign" depicts the lateral borders of the uvula on 2-dimensional fetal ultrasound. We assessed the feasibility of adding the equal sign to the midtrimester fetal anomaly ultrasound. METHODS: A retrospective audit of an ultrasound practice in Cape Town, South Africa was performed. Women with singleton pregnancies presenting for a routine anomaly ultrasound between October 2013 and September 2019 were included. RESULTS: A total of 53

Case Report: Positioning head tilt observed in six dogs with meningoencephalitis of unknown origin.

Positioning head tilt (PHT) is a dynamic clinical neurological sign that is characterized by a head tilt to the opposite side of a voluntary lateral turn of the head. Based on recent publications, various etiologies are proposed for the occurrence of PHT in dogs and cats. One suggested cause is a lack of inhibitory input to the vestibular nuclei due to dysfunction of the cerebellar nodulus and uvula (NU). In that category, it has been reported in dogs with hypoplasia of the NU, dogs with lysosomal storage diseases, and in a dog with a cerebellar tumor. Other proposed causes of PHT include redu

Multimodal MRI of the reorganization of multisensory and sensorimotor networks in chronic bilateral vestibulopathy.

Chronic bilateral vestibulopathy (BVP) occurs preferably in elderly patients presenting with postural imbalance and head movement induced oscillopsia. The condition is often incomplete with residual functions in both ears. Beyond the vestibular reflexive deficits, an impairment of spatial orientation and navigation has been described associated with an atrophy of the hippocampal formation. However, this finding was inconsistent in various studies on rodents and humans. In the current MRI study on 15 BVP patients and 15 healthy controls (HC) we combined analyses of whole brain voxel-based morph

Risk Analysis Index Versus mFI-5 for Predicting Outcomes After Uvulopalatopharyngoplasty.

OBJECTIVE(S): Current preoperative risk stratification for uvulopalatopharyngoplasty (UPPP) primarily relies on anatomic and disease-severity measures, which do not capture physiologic reserve or vulnerability to postoperative complications. This study aimed to compare the predictive performance of the risk analysis index (RAI) and the modified frailty index-5 (mFI-5) in stratifying postoperative risk among patients undergoing UPPP. METHODS: Adult patients who underwent UPPP between 2005 and 2020 were identified. Frailty was assessed, and associations with postoperative complications, length

Predictive factors for surgical outcomes and symptom persistence after uvulopalatopharyngoplasty in patients with obstructive sleep apnea.

OBJECTIVE: For individuals diagnosed with obstructive sleep apnea (OSA), uvulopalatopharyngoplasty (UPPP) represents a frequently utilized surgical intervention, yet its postoperative efficacy varies significantly among individuals. Identifying key preoperative predictors of surgical success is crucial for optimizing patient selection and enabling personalized treatment strategies. METHODS: This retrospective study included 191 OSA patients who underwent UPPP. Postoperative outcomes were assessed at a three-month follow-up, and a multivariate logistic regression model combining body mass index

Velocity Storage Integrator (VSI): Mechanisms, circuits, and clinical implications A narrative review.

The velocity storage integrator (VSI) is a central, gravity-referenced neural network that prolongs and spatially orients vestibular/optokinetic signals beyond semicircular-canal dynamics. Classical systems analyses framed it as a leaky integrator in an indirect pathway, complementing a high-frequency direct pathway that rapidly stabilizes gaze. Converging unit recordings, lesions, and modeling now localize the spatiotemporal core of VSI to commissural vestibular-nuclei networks-particularly vestibular-only (VO) and vestibular-pause-saccade (VPS) populations-under cerebellar nodulus-uvula (NU)

Multidimensional factors in submucous cleft palate velopharyngeal function: a large cohort study.

This retrospective study analyzed clinical characteristics and surgical outcome predictors in 676 patients with submucous cleft palate (SMCP) treated between 2008 and 2025. Patients were stratified by palatal morphology to evaluate velopharyngeal function and articulation. Among 183 patients surgically treated for velopharyngeal insufficiency (VPI), postoperative cure rates were assessed using regression analysis. Preoperatively, 52.96% of the cohort presented with VPI. Palatal morphology significantly correlated with function; the bifid uvula subtype exhibited the highest rate of velopharynge

Elongated inflamed uvula mimicking early recurrence on planning CT in post-laryngectomy laryngeal squamous cell carcinoma: a diagnostic pitfall in adjuvant radiotherapy.

BACKGROUND: Laryngeal squamous cell carcinoma (SCC) requires rigorous post-operative imaging surveillance to detect early recurrence, yet benign post-surgical changes may mimic malignancy and create diagnostic uncertainty. CASE PRESENTATION: We report a case of a 65-year-old male with well-differentiated laryngeal SCC who developed a suspicious lesion in the oropharynx on planning CT for adjuvant radiotherapy, 2 months after total laryngectomy, raising concern for early local recurrence. Clinical Discussion: Further assessment using multiplanar MRI demonstrated a well-defined elongated uvula

Rare Causes of Unexplained Chronic Cough in Adults.

Cough can result from a variety of etiologies, and rare pulmonary, otolaryngologic, cardiac, neurologic, medication-related, and functional conditions are often overlooked. Pulmonary etiologies, including structural airway abnormalities, endobronchial obstruction, expiratory central airway collapse, and occupational eosinophilic bronchitis, frequently require advanced imaging and bronchoscopy for diagnosis. Otolaryngologic conditions include elongated uvula, tonsillar hypertrophy, and external auditory canal pathology, which can trigger vagally mediated cough reflexes. Obstructive sleep apnea

Which surgical procedure is more effective for obstructive sleep apnea-CWICKs or barbed reposition pharyngoplasty (BRP)?

OBJECTIVE: Uvulopalatopharyngoplasty has long been a standard surgical treatment for obstructive sleep apnea (OSA). Recently, novel suture techniques such as CWICKs and barbed reposition pharyngoplasty (BRP) have been developed. This study aimed to compare the surgical efficacy of CWICKs and BRP. METHODS: A retrospective case-control study was conducted involving 21 patients (18 males and 3 females) with OSA who underwent either CWICKs (n = 9) or BRP (n = 12) between April 2020 and January 2025. Outcomes were evaluated using home sleep apnea testing before surgery and two months postoperative

Buccal Sulcus Myomucosal Flap-an Approach for the Closure of Anterior Palatal Fistulae: A Retrospective Cohort Analysis of 132 Patients.

Anterior palatal fistulae (APFs) present a considerable challenge following cleft palate repair, particularly when involving the alveolus and the anterior half of the hard palate. These defects are notoriously difficult to close due to limited local tissue availability, scarring, restricted flap mobility, and altered vascularity, all contributing to high recurrence rates. This study evaluates the effectiveness of the buccal sulcus myomucosal flap (BSMF) as a practical and reliable option for APF closure. A retrospective review of 132 patients treated between January 2021 and December 2023 was

Nasopharyngeal hirudiniasis, a rare cause of masked hemoptysis in an older patient: A case report.

Leeches commonly inhabit freshwater environments and feed on blood; however, pharyngeal leech infestation in humans remains an unusual cause of hemoptysis that poses significant diagnostic challenges. We report the case of a male patient in his early 60s presenting with a 2-week history of persistent hemoptysis and pharyngeal discomfort. Physical examination revealed a transient, mobile structure behind the uvula that initially mimicked a blood clot. Flexible nasopharyngoscopy subsequently demonstrated a moving foreign body attached to the nasopharyngeal wall. The organism, identified as a 3-c

Foreign Body Impaction in the Pediatric Palate: A Case Report of a Frequently Misdiagnosed Condition.

This case report aims to increase awareness about foreign body (FB) impaction in the pediatric palate. The significance of this clinical entity lies in the fact that it is frequently misdiagnosed as a palatal fistula or mass. Patients are therefore subjected to a variety of unnecessary investigations, for example, computed tomography scans and x-rays. In this case report, a 13-month-old boy was referred for a newly developed oronasal fistula. Clinical evaluation, however, correctly identified a FB impacted in the palatal vault. The FB (button cap) was successfully removed under general anesthe
