CASE: A 9-year-old boy presented with a severe median nerve palsy 6 months after a simple elbow dislocation. Neurotmesis was confirmed on surgical exploration, and it was repaired primarily in elbow flexion with the arm casted postoperatively. At 1-year follow-up from surgery, the patient reported near normal sensory improvement with motor deficits that did not affect his full return to activities.
CONCLUSION: The median nerve neurotmesis after elbow dislocation without an associated fracture is rare. This case highlights the importance of careful neurologic examination in children sustaining

Background: Brachial artery injuries are common in upper limb trauma and require timely repair to prevent ischemia and limb loss. In developing countries like India, delayed presentation is frequent due to limited access to specialised care, inadequate infrastructure and delays in multidisciplinary coordination, complicating timely revascularisation. This study aimed to evaluate limb salvage outcomes and factors influencing prognosis in patients undergoing delayed repair of traumatic brachial artery injuries. Methods: This prospective study included 30 male patients (ages 18-58 years) with tra

Carpal tunnel syndrome (CTS) is the most common compressive mononeuropathy. In patients with type 2 diabetes mellitus (T2DM), chronic hyperglycemia, microangiopathy, and systemic inflammation increase the vulnerability of peripheral nerves to compression. This study aimed to assess the relationship between CTS severity and clinical, metabolic, inflammatory, and electrophysiological parameters in patients with T2DM. A cross-sectional study was conducted from June 2023 to June 2024, involving patients diagnosed with T2DM. Electrophysiological assessment of the upper and lower limbs was performed

BACKGROUND: Carpal tunnel syndrome (CTS) is the most common peripheral neuropathy in the general population. Recurrent bilateral CTS could be the first manifestation of a more serious systemic disease, such as amyloidosis, which would help with early diagnosis and prevent mortality and morbidity associated with this illness.
CASE PRESENTATION: We present a rare case of a 75-year-old Middle Eastern man suffering from bilateral wrist edema with weakness and numbness of both hands and a history of recurrent bilateral CTS. The bilateral wrist masses were revealed at the MRI investigation. He was

INTRODUCTION: Upper extremity conditions such as carpal tunnel syndrome (CTS) are more prevalent in patients with diabetes mellitus (DM) than in the general population. This prospective study aimed to determine the viability of DM screening for patients presenting with upper extremity musculoskeletal disorders in an orthopaedic hand clinic and to inform future testing criteria for orthopaedic surgeons.
METHODS: Patients with a diagnosis of CTS, trigger finger, Dupuytren's contracture, and/or De Quervain's disease were prospectively enroled. Eighty-four patients (60.7% female, 60.0% White) und

Holstein-Lewis fractures are spiral fractures in the distal third of the humeral shaft, which carry increased risk of radial nerve palsy compared with other humeral shaft fractures; these fractures are usually managed conservatively. However, there is no case in the existing literature that describes the management of Holstein-Lewis fractures with radial nerve entrapment in a patient with a history of total elbow arthroplasty (TEA). Here, we present the first documented case of a complex revision elbow arthroplasty for Holstein-Lewis fracture with delayed onset radial nerve entrapment after a

BACKGROUND: Chronic pain in patients with multiple overlapping pain syndromes is often refractory to conventional treatments. Peripheral nerve stimulation (PNS) has emerged as a promising neuromodulation therapy, especially when spinal cord stimulation (SCS) is contraindicated or ineffective.
CASE REPORT: We present the case of a 68-year-old man with severe chronic pain in the left hand and wrist following carpal tunnel release surgery, superimposed on a background of cervical and lumbar radiculopathies, degenerative cervical spondylosis, sacroiliitis, and failed back surgery syndrome. Conser

Upper-extremity conditions, including carpal tunnel syndrome, de Quarvians disease, trigger finger, and joint laxity, are common during pregnancy and are related to hormonal fluctuations and fluid shifts. These conditions generally respond well to conservative management with medications, bracing, and corticosteroid injection. However, surgery should be offered during pregnancy if nonoperative treatment fails. Regional and local anesthesia decreases fetal anesthetic exposure, minimizes airway issues, and assists postoperative pain control. Pregnant patients with trauma should be evaluated for

Background. Cubital tunnel syndrome is the second most common compression neuropathy in the upper limb. The outcomes of operative treatment of cubital tunnel syndrome are predictable to some extent. Approximately 70-80% of patients are satisfied with the good long-term outcomes, but the remaining 20-30% benefits less or even are dissatisfied with the treatment. Moreover, in 7-24% recurrence occurs, or necessity of revision surgery due to lack of improvement after primary operation. The objective of this study was review of the literature identifying factors that affect the risk of recurrence

Trapeziometacarpal (TMC) joint arthroplasty has become an established and effective treatment option for advanced trapeziometacarpal osteoarthritis. Prosthetic replacement can also be employed in revision procedures following failed surgeries involving the trapezium or the base of the first metacarpal. Revision arthroplasty is technically demanding, as altered anatomical conditions, scar tissue, and compromised bone quality may complicate implantation. Patients and Surgical Techniques Based on specific cases, we present indications and techniques for revision arthroplasty of the TMC joint, inc

BACKGROUND: Congenital anomalies of kidney and urinary tract (CAKUT) are the leading causes of chronic kidney disease (CKD) in childhood. Determining the clinical course, outcome, and prognostic factors of this heterogeneous disease group is important to provide appropriate management and follow-up. Therefore, we aimed to identify the risk factors of CKD in CAKUT and the differences in clinical courses between disease subgroups.
METHODS: Three hundred patients (M/F: 203/97) divided into 16 CAKUT categories were enrolled in the study. Logistic regression and survival analyses were performed to

PURPOSE: To assess methicillin-resistant Staphylococcus aureus (MRSA) bloodstream dissemination risk from moxibustion burns in type 2 diabetes and raise awareness of iatrogenic skin injuries in high-risk populations.
BACKGROUND: Staphylococcus aureus (S. aureus) causes bloodstream and metastatic infections, with a mortality rate up to 30%. In diabetic patients, minor skin injuries can lead to fatal systemic infections due to immune dysfunction and neuropathy. Moxibustion is widely used for chronic pain, but its thermal safety in high-risk groups is under-evaluated. Multiorgan MRSA disseminati

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm

Microbiological culture and histology of deep tissue specimens are independent diagnostic criteria in fracture-related infection (FRI). However, the association between these tests has rarely been investigated, particularly in relation to clinical outcome after treatment. Patients undergoing surgery for International Consensus-confirmed FRI were included. All had ≥5 tissue specimens taken for microbiological culture and 2-3 for histology. The correlation between cultured pathogen, histological positivity (defined as ≥5 polymorphonuclear neutrophils/high power field), and outcome at one year af

RATIONALE: Fractures caused by low-voltage electric shocks are extremely rare, especially those involving the distal appendicular skeleton. Most reported cases involve the proximal skeleton, such as the shoulder or vertebrae, and phalangeal fractures following low-voltage shocks without direct mechanical trauma have not been reported previously.
PATIENT CONCERNS: A 60-year-old man presented with persistent pain, swelling, limited thumb motion, and a non-healing ulcer after sustaining a 220V household electric shock to his left thumb. The patient reported falling onto his buttocks immediately
