Disease Burden and Long-term Psychosocial Outcomes in Children and Young Adults with Graves' Disease.

OBJECTIVE: We aimed to compare disease burden and psychosocial outcomes between patients with GD in childhood/young adulthood and an unexposed population cohort. METHODS: This cohort study used prospectively collected data from medical records and multiple Swedish registers. Patients diagnosed with GD aged <21 years (n=87, of whom 55 were <18 years) in 2003-2005 were included. Each patient was matched with 10 individuals without GD. RESULTS: Over the 17-year follow-up (median: 15.5 years), patients with GD had significantly higher recorded rates of 'endocrine, nutritional, and metabolic dise

Upper extremity deep vein thrombosis after arthroscopic rotator cuff repair.

Arthroscopic shoulder surgery is generally associated with a low complication rate. While complications such as neurological injury, stiffness and iatrogenic damage are relatively well recognised, the occurrence of deep vein thrombosis (DVT), particularly of the upper extremity deep vein thrombosis (UEDVT), is exceedingly rare. We report two unusual postoperative cases of UEDVT following arthroscopic rotator cuff repair in two elderly sisters. No additional predisposing factors were identified beyond the perioperative setting; thrombophilia screening was negative. These cases underscore the im

Distribution of Infralesional Lower Motor Neuron Injury in Traumatic Cervical Spinal Cord Injury Candidates for Upper Limb Reconstruction: A Single Center Experience.

INTRODUCTION/AIMS: Patients with cervical spinal cord injury (SCI) prioritize recovery of upper extremity function. Upper extremity reconstruction can restore function in carefully selected individuals. Infralesional cord damage, observed below the neurological level of injury (NLI), may affect postoperative outcomes. We characterized the extent of infralesional lower motor neuron (LMN) injury in patients with traumatic cervical SCI. METHODS: Each patient underwent electromyography (EMG) of potential donor and recipient muscles based on preoperative clinical evaluation and specific candidacy

Use of extracorporeal circulation in multidisciplinary approach and replantation strategies for traumatic limb amputations: A case report.

BackgroundTraumatic major limb amputations require urgent intervention to prevent irreversible tissue necrosis and life-threatening systemic complications due to warm ischemia. We aim to demonstrate the efficacy of extracorporeal circulation (ECC) in extending the ischemia tolerance window and stabilizing metabolic parameters during multidisciplinary replantation.Case PresentationA 23-year-old female presented with bilateral total upper extremity amputations following a high-energy motor vehicle accident. While damage control surgery addressed intra-abdominal injuries, the amputated limbs were

Surgical fixation versus non-surgical care for children with a displaced medial epicondyle fracture of the elbow (the SCIENCE study): a multicentre, randomised controlled, superiority trial and economic evaluation.

BACKGROUND: Displaced medial epicondyle fractures are among the most controversial injuries in children, with increasing trends towards surgical fixation despite little supporting evidence. Approximately half of affected children undergo surgical fixation, while others receive non-surgical care. The SCIENCE trial aimed to determine whether surgical fixation to restore the position of the bone provides superior functional outcomes and is cost-effective compared with non-surgical care. METHODS: We conducted a pragmatic multicentre, randomised, superiority trial across 59 hospitals in the UK, Au

Effectiveness of magnets in removing metallic foreign bodies.

BACKGROUND: Metallic foreign bodies (MFB) in the extremities are a common clinical problem that requires removal to prevent complications and to avoid contraindications to magnetic resonance imaging. While magnetic extraction represents an alternative to conventional surgical techniques, systematic data regarding its efficacy and clinical utility remain limited. This study aimed to evaluate the clinical efficacy of magnetic extraction for MFBs in extremities, its impact on radiation exposure, and success rates across different anatomical locations and foreign body types. METHODS: We retrospec

Scar Contracture Recurrence After Axillary Burn Reconstruction in Adults: A Single Institution's 14-Year Experience.

Axillary burn contractures impair upper limb function and can recur after initial reconstruction. The risk factors for recurrence remain unclear. This study aims to evaluate the recurrence rate of axillary burn contractures and identify associated risk factors. A retrospective chart review was conducted on patients who underwent reconstructive surgery for axillary burn contracture at a single institution between 2009 and 2022. Data collected included demographic information, injury details, reconstruction type, follow-up, reoperations, and complications. There were 30 axillary burn scar contra

Short-Term Results of an Algorithm for Brachial Artery Injury Repair Favoring a Reversed Ipsilateral Arm Vein Conduit.

BACKGROUND: The great saphenous vein (GSV) has been the traditional conduit for repair of brachial artery injury that is not amenable to primary anastomosis. The use of the ipsilateral upper limb cephalic or basilic vein can preserve the saphenous vein and avoid an extra incision. However, there is a lack of robust data on long-term outcomes. The aim of this study was to evaluate the feasibility, complications, patency rates, and limb salvage rates associated with this technique. METHODS: A prospective cohort study was conducted on 52 patients with brachial artery injury requiring interpositi

Early causes of failure in hand and macroreplantations: A multicenter retrospective analysis of 43 cases.

INTRODUCTION: Macroreplantation is a rare yet technically challenging procedure performed following traumatic amputations. Despite advances in surgery, early failure rates remain high. The aim of this study was to identify predictors of early failure after macroreplantation. METHODS: We conducted a multicenter retrospective analysis of 43 macroamputations (from metacarpal level to proximal humerus) that were treated with replantation at four level-1 trauma centers in France between 2008 and 2024. Patients were classified into two groups: a failure group (defined as secondary amputation within

Pigtail assisted tracking of guide catheter during transradial coronary angioplasty on forearm complications: prospective randomized trial.

OBJECTIVES: During transradial percutaneous intervention (PCI), sharp edge of guide catheter tip may produce "razor blade effect" leading to radial vascular injury. Pigtail assisted tracking (PAT) technique has been described to overcome it, but has not been systematically evaluated. We prospectively evaluated the safety and efficacy of PAT technique in transradial PCI. METHODS: Consecutive patients undergoing transradial-PCI were randomized into two groups (Group 1: transradial navigation using PAT technique, Group 2: transradial navigation without PAT technique). Patients were observed for

Non-surgical casting versus surgical reduction for children with severely displaced distal radial fractures (the CRAFFT Study): a multicentre, randomised, controlled non-inferiority trial and economic evaluation.

BACKGROUND: Severely displaced distal radial fractures are among the most common and controversial injuries in children. Despite observational evidence of reliable remodelling with growth in younger children, their alarming radiographic appearance-particularly when completely displaced (off-ended)-has driven routine surgical reduction and fixation. The Children's Radius Acute Fracture Fixation Trial (CRAFFT) aimed to evaluate the clinical and cost-effectiveness of surgical reduction compared with non-surgical casting. METHODS: This pragmatic, multicentre, randomised, non-inferiority trial inc

Masquelet technique in adolescent distal humerus fractures with bone loss.

Adolescent distal humerus fractures usually result from high-energy trauma; however, open fractures with associated bone loss are quite rare. Although the Masquelet technique has demonstrated proven success in the reconstruction of lower extremity bone defects, its use in upper extremity periarticular defects, especially in adolescents, has been very rarely reported. This study aims to present the clinical and radiological outcomes of the Masquelet technique in adolescent distal humerus fractures with bone loss. A retrospective review was conducted of patients aged 12-16 years who underwent th

[Comparative efficacy of island latissimus dorsi myocutaneous flap versus lower trapezius myocutaneous flap for repair of posterior neck skin and soft tissue defects].

OBJECTIVES: Posterior neck skin and soft tissue defects often result from electrical burns, post-tumor resection, infections, or trauma. The region has complex anatomy, high mobility, and proximity to the cervical spine, major vessels, and critical nerves; thus, reconstruction must balance functional restoration and aesthetic outcomes. The island latissimus dorsi myocutaneous flap, with its reliable blood supply, large harvestable area, and long rotation arc, is widely used for trunk and limb reconstruction and shows particular advantage for large defects in the thoracic, dorsal, and posterior

Scapulothoracic Dissociation in Contemporary Practice: Revisiting Clinical Reality Through the Trauma Quality Improvement Program.

BackgroundScapulothoracic dissociation is a rare, limb-, and potentially life-threatening injury in which the scapula and shoulder girdle are violently detached from the thoracic cage. However, the published data on the condition is mainly composed of case reports and single institution samples, which limits the overall generalizability. The aim of the current investigation was consequently to use a multi-institutional data set to characterize the injuries observed in patients with scapulothoracic dissociation along with the treatment strategies selected.MethodsThe 2016-2021 Trauma Quality Imp

Concomitant upper extremity fractures do not worsen outcomes in conservatively managed pelvic fractures: a cohort study of 1840 geriatric patients.

INTRODUCTION: Pelvic fractures in elderly patients are associated with significant pain, functional decline, and prolonged rehabilitation, with the majority managed non-operatively. In the conservative management paradigm, intact upper extremity function is critical for mobilization with assistive devices; concurrent upper extremity (UE) fractures may therefore complicate recovery. The aim of this study was to evaluate the clinical outcomes associated with this injury combination in patients with conservatively managed pelvic fractures. METHODS: A retrospective cohort study was conducted at a

Evaluating the Efficacy and Safety of Venous Flow-Through Flaps in Hand Reconstructive Surgery: A Systematic Review.

BACKGROUND: Venous flow-through (VFT) flaps offer a valuable reconstructive option for the upper extremity due to ease of harvest and low donor-site morbidity. Despite their potential, survival rates are perceived as unpredictable, and their complication profile remains poorly defined. This study reviews surgical outcomes, applications, and challenges of VFT flaps in upper extremity reconstruction. METHODS: Following PRISMA guidelines, a computerized search of 5 databases was performed on April 7, 2024, using terms including "forearm," "hands," "digits," "upper extremity," "flaps," "reconstru

[Immobilization recommendations following upper limb trauma - literature review].

Upper limb injuries are common and account for a significant number of emergency department and primary care consultations. While surgery for these injuries is well-documented, nonoperative treatments, particularly the optimal duration of immobilization, remain understudied. These durations are often based on institutional dogma rather than scientific evidence, which have gone unchallenged for years. However, prolonged immobilization can lead to increased morbidity or even complications for patients. The aim of this systematic literature review is to update the recommendations regarding optima

Sensory Recovery Following Digital Nerve Repair Using Conduits Versus Autologous Nerve Grafts: A Systematic Review and Meta-Analysis.

BACKGROUND: Peripheral nerve injuries (PNI), particularly digital nerve injuries, can lead to significant loss of function, pain, and sensory deficits. Autologous nerve grafting (ANG) has been the gold standard for nerve gap reconstruction, but it is associated with donor site morbidity and other complications. Nerve conduits, including collagen-based, chitosan-based, and muscle-in-vein conduits, have emerged as potential alternatives with the advantage of avoiding donor site morbidity. The objective of this systematic review and meta-analysis was to compare the efficacy and safety of nerve co
