Anterior interosseous nerve syndrome.

The anterior interosseous nerve syndrome is a partial motor paralysis of the median nerve. Despite being well protected in the deep forearm, the nerve may be subject to traction and pressure phenomena occurring spontaneously or related to trauma. Forearm pain, fracture of an upper extremity, or the characteristic disturbance of pinch should alert the examiner to the possibility of a lesion of the anterior interosseous nerve. Function may be regained in some instances with conservative management. It is probably unwise to delay surgical exploration beyond eight to 12 weeks if the patient has sh

Anterior interosseous nerve avulsion.

A 19-year-old man with a fracture of the distal humerus avulsed the anterior interosseous nerve. Avulsion of this nerve may be more common than previously believed. As shown by a cadaver study, the mechanism of injury is related to traction on the median nerve. Complete clinical assessment requires an accurate neurological examination; specifically, the integrity of the anterior interosseous nerve is tested, and a characteristic disturbance of pinch is identified. By treatment with a free nerve graft it is possible to achieve full functional recovery.

Local arterialized island flap coverage of difficult hand defects preserving donor digit sensibility.

Small to moderate-sized defects of the hand overlying joint surfaces, flexor tendons, Silastic implants, and tactile surfaces require full-thickness skin and subcutaneous tissue either for primary coverage or in anticipation of secondary reconstruction. In this report, six difficult hand wounds are resurfaced with an arterialized island pedicle from the lateral surface of the nearby digit in lieu of multistage distant pedicle flaps. The Littler neurovascular island flap has been modified to include only the digital artery and venae comitantes at its pedicle, preserving digital nerve intact in

Intraoperative brachial plexus injuries during reconstruction of the burned axilla.

Since opening of the Shriners Burns Institute, Cincinnati Unit, 501 operations for release of burn scar-related contractures of the axilla have been carried out with skin grafting. Six of these cases, 1.2%, resulted in intraoperative stretch injuries to the brachial plexus. In two cases, isolated axillary nerve involvement was encountered. In four cases, additional diffuse injury to the brachial plexus was present. Intraoperative motion and postoperative splinting in a position of abduction were believed to be possible etiological agents. Prompt removal of the splint and institution of physica

Sports and peripheral nerve injury.

Peripheral nerve injury is one of the serious complications of athletic injuries; however, they have rarely been reported. According to the report by Takazawa et al., there were only 28 cases of peripheral nerve injury among 9,550 cases of sports injuries which had been treated in the previous 5 years at the clinic of the Japanese Athletic Association. The authors have encountered 1,167 cases of peripheral nerve injury during the past 18 years. Sixty-six of these cases were related to sports (5.7%). The nerves most frequently involved were: brachial plexus, radial nerve, ulnar, peroneal, and a

The carpal tunnel syndrome in chronic dialysis patients, it is a late complication of the arterio-venous fistula?

Out of 100 patients undergoing chronic hemodialysis in Lausanne (Switzerland), 12 developed a carpal tunnel syndrome (i.e. 8 men and 4 women, from 34 to 76 years old). Out of 66 patients with an arteriovenous fistula for less than 4 years, it is interesting to note that only 3 carpal tunnel syndrome were observed; whereas, out of 34 hemodialysis patients being dialyzed more than 4 years, 9 of them showed that syndrome. The symptomatology of the carpal tunnel syndrome is described by the authors. It has always been confirmed by EMG. On 10 patients, the symptoms were so acute that they needed a

Late ulnar paralysis. Study of seventeen cases.

Seventeen cases of late ulnar paralysis treated by neurolysis-transposition are reported. The clinical characteristics of these paralysis are emphasized. A very prolonged symptom free interval, a rapid onset and a severe involvement. The ulnar transposition was most often done subcutaneously. Cubitus valgus and definite nerve compression proximal to the arcade of the flexor carpi ulnaris muscle are almost always present. The results as regards the neuropathy are notable: no patient is completely cured and only half are improved. An anatomical study of the nerve path shows the essential role, i

[Treatment of intra-articular metacarpal and phalangeal fractures with an external fixation minidevice].

Open reduction of intraarticular fractures involving the metacarpals and phalanges and fixation with the small external fixation apparatus is a promising new method which fulfills a need. Full return of function in seven patients and a small limitation of function in three patients can be regarded as a satisfactory result. All patients returned to work after a short time without any complications. The external mini-fixation device which has already been useful in the treatment of open and closed fractures, infected nonunions and in performing arthrodeses has now found another application in th

Severed body parts: radiologic evaluation following replantation.

Sixty-two replantations of totally severed body parts in 25 patients were followed via serial radiography. Most of these replantations were of digits. Uncomplicated digital replantation showed a definite temporal pattern consisting initially of decreasing mineral content followed by increasing mineral content, decreasing soft-tissue swelling, and increasing callus formation. Deviations from the expected patterns were helpful in predicting complications such as infection and/or osteonecrosis.

Isolated compression of the motor branch of the ulnar nerve.

This paper reports four cases of isolated compression of the deep branch of the ulnar nerve, and reviews the various etiologies (tumor, malformation and microtraumatism). The authors stress the importance of the electric examination, particularly conduction velocity in the palm in localizing the nerve lesion. Surgical exploration in these cases revealed a Giant cell tumor, two synovial cysts, and an abnormal fibrous band stretching between the flexor digiti quinti brevis and the opponents digiti quinti. Postoperative courses were uncomplicated and every patient recuperated quickly and complete

Crush injury of the upper limb. Early and late management.

The care of the patient with a crushed upper limb requires accurate assessment, carefully planned and executed surgical procedures, diligence, and reassessment. Often, a long period of rehabilitation is also needed. The quality of care rendered by the surgeon and therapist often affects the patient's functional end result and his chance to regain his ability to provide for himself independently.

[Atloaxoid dislocation with basilar pressure and spinal cord symptoms in rheumatoid arthritis. A case of occipitocervical arthrodesis followed by resection of the odontoid].

The authors report a case of atlanto-axial dislocation in a patient with rheumatoid arthritis. There was some neurological impairment with upper limb pain and hyperreflexia. As a first stage, a cranio-cervical fusion was performed through a posterior approach. Three weeks later the axis was excised through an antero-lateral approach, together with its odontoid process. The patient's condition was much improved and the neurological lesions disappeared.

Delayed skin graft application following burn scar release of the face and hand.

Hypertrophic scars and contractures involving the face and hand secondary to burns were excised and the defects covered with saline compresses changed every 4 hours. Banked split-thickness skin grafts were applied 24 to 48 hours later. Upper and lower lip, cheek, forehead, scalp, neck, the first web space of the hand, and the flexor surfaces of the fingers were treated. Head and neck defects were grafted open. Hand defects created by release were maintained with dynamic splints and grafted open or covered with bolus dressings kept in place with sutures inserted at the time of creation of the d

Anterior interosseous nerve syndrome following supracondylar lesions of the median nerve: clinical findings and electrophysiological investigations.

Lesions at the volar aspect of the forearm may lead to isolated paralysis of the anterior interosseous nerve, which is a purely motor branch of the median nerve. The same syndrome may result from circumscribed damage at the level of the upper arm. Two patients are reported with the typical anterior interosseous nerve syndrome following supracondylar lesions of the median nerve and the literature is reviewed. In most cases this syndrome is a complication of supracondylar fractures of the humerus in children. A simple clinical test for the diagnosis, the pinch test, and the typical electromyogra

Late results in fractures of the radial head treated by excision.

Late results of radial head excision were studied in 60 patients who had isolated fractures of the radial head. Symptomatic elbows were found in 43% of the patients, osteoarthritis in 52%, and valgus deformity in 30%. Elbow motion was restricted in 63%, and new bone formation at the surgical site was noted in 57% of the patients. Forearm rotation was limited in 58%. A proximal radioulnar synostosis was found in 7%, and proximal migration of the radius was present in 47%. A definite and symptomatic subluxation of the distal radioulnar joint was found in 25%. Overall results were unsatisfactory

Rotatory subluxation of the scaphoid after excision of dorsal carpal ganglion and wrist manipulation--a case report.

Surgical excision of a ganglion on the dorsum of the wrist is usually a benign procedure. The most frequent complications are transient postoperative stiffness and recurrence of the ganglion. This paper reports the development of a rotatory subluxation of the scaphoid after the manipulation of the wrist of a patient who had developed postoperative stiffness after the surgical excision of a dorsal wrist ganglion. This unusual complication was successfully treated by closed pinning under radiographic control followed by immobilization in palmar flexion. Manipulation of the wrist for the manageme

Fixed post-traumatic flexion-contractures of digits. Review of thirty-three cases.

A fixed post-traumatic flexion contracture of a finger is usually secondary to multiple previous operations. We have observed that a former flexor tendon laceration is not constant and is missing in 18% of our cases. The flexor tendons are, nevertheless, always involved in the contracture. A volar skin contracture was present in all cases, but only in half of them was noted a retraction of the volar components of the PIP joint. This articular involvement has no statistical correlation with the time elapsed from the onset of the contracture. We have reviewed 33 cases of post-traumatic flexions

Trigger wrist phenomenon in rheumatoid arthritis.

Flexor tendon synovitis in patients with rheumatoid arthritis commonly presents with a carpal tunnel syndrome and a concomitant trigger finger. Triggering at the wrist joint is in this disease an uncommon additional feature. In the two cases reported, histological examination of the tissue has indicated that, as well as chronic synovitis, there was a true rheumatoid nodule present at the wrist joint, either pedunculated or fusiform. The passage of the nodule, with a 'Click' from the proximal side of the transverse carpal ligament to the distal aspect, when the fingers are flexed, creates the s
