Necrotising fasciitis of the hand: emergency management and early functional reconstruction with scapular and parascapular free flap.

Necrotising fasciitis is a rare and rapidly progressive infection that demands early diagnosis and aggressive management to prevent fatal outcomes. We present a case of a woman in her 70s who developed a necrotising fasciitis of the hand and forearm after a minor thumb injury while cleaning artichokes. The initially underestimated wound progressed rapidly, requiring multiple surgical debridement. After clinical stabilisation, a complex full-thickness defect remained, exposing critical structures. Reconstruction was performed using scapular and parascapular free flap, ensuring excellent function

The DRoPH score: development of a tool for predicting reduction difficulty in proximal humerus fracture osteosynthesis.

BACKGROUND: Proximal humerus fractures (PHFs) are among the most common and challenging fractures due to complex anatomy and variable fracture patterns. While anatomical reduction is critical for successful outcomes, no tool currently predicts the intraoperative difficulty of achieving it. Despite advances in surgical techniques, complications persist. Existing classification systems, such as Codman's, Neer's, AO/OTA, and Mayo-FJD, focus on fracture morphology and displacement but do not address intraoperative complexity. This study aims to develop a new score for the early prediction of intra

Anterior Interosseous Artery as a Recipient Vessel in Superficial Circumflex Iliac Artery Perforator Flap Transplantation to the Forearm.

BACKGROUND: When transplanting a superficial circumflex iliac artery perforator (SCIP) flap to the forearm, the small diameter of the superficial circumflex iliac artery (SCIA) complicates end-to-end anastomosis with the radial or ulnar artery. The small diameter of the anterior interosseous artery (AIA) may make it suitable for end-to-end anastomosis with the SCIA; however, no such cases have been reported to date. We assessed AIA diameter using computed tomography angiography (CTA) and analyzed six cases of SCIP flap transfer to the forearm using the AIA as the recipient vessel. METHODS: In

Cerebral arterial oxygen embolism as a complication of hyperbaric oxygen treatment: a case report.

Cerebral arterial gas embolism (CAGE) is a recognised complication of diving-related barotrauma and of medical procedures whereby gas enters the vascular system. CAGE is a principal indication for hyperbaric oxygen treatment (HBOT). In contrast, CAGE resulting from HBOT itself is exceedingly rare. We describe the case of a 73-year-old man undergoing his first session of HBOT for a chronic lower limb wound, who developed acute focal neurological deficits during decompression. He had no known respiratory disease or smoking history. Immediate assessment demonstrated stable physiology. Initial com

Paediatric Hand Compartment Syndrome Secondary to Intravenous Contrast Extravasation: A Case Report.

Iodinated contrast extravasation is usually self-limited; however, in confined regions such as the dorsum of the hand - especially in paediatric patients - it can progress to acute compartment syndrome. We report a 9-year-old girl who developed hand compartment syndrome following extravasation of 50 mL iohexol (350 mgI/mL) via a 22G catheter during contrast-enhanced abdominal computed tomography. Despite standard extravasation management, symptoms progressed rapidly. Compartment syndrome was diagnosed clinically based on pain disproportionate to injury, refractory to analgesics, exacerbated by

Pediatric digit replantation: A mixed methods outcomes study.

BACKGROUND: Indications for pediatric digital replantation/revascularization (replant/revasc) are broader than those for adults. Pediatric patients suffer many of the same challenges seen in adult replant/revasc patients with similar rates of digit salvage. The purpose of this study was to review the incidence of and long-term outcomes after pediatric replant/revasc. METHODS: Data was collected from medical records of patients presenting to our Level 1 Trauma Pediatric Hospital Emergency Room between January 2011 and March 2017. All charts with ICD-9 codes for upper extremity trauma were quer

Arthroscopic debridement with microfragmented adipose tissue for wrist and hand chondral lesions in professional athletes.

BACKGROUND: Focal chondral lesions of the wrist and hand are an underrecognized source of pain and functional limitation in athletes. Their management remains challenging due to the limited healing capacity of hyaline cartilage, the biomechanical demands of small joints, and the lack of standardized treatment algorithms. Arthroscopy allows accurate diagnosis and targeted treatment with minimal morbidity, while biologic augmentation using autologous adipose-derived products has emerged as a potential adjunct to improve clinical outcomes. OBJECTIVE: To evaluate the clinical outcomes of arthrosc

Key Outcomes for Evaluating Hand and Wrist Scars: A Nationwide Survey of Clinicians in Saudi Arabia.

Background and Objectives: Hand and wrist scars alter physical appearance and can result in functional impairments and psychosocial difficulties. Although these effects are clinically important, rehabilitation services in Saudi Arabia lack consistent and standardised scar assessment protocols. The limited use of validated outcome measures hinders both clinical practice and research. Standardised scar assessment is essential for evidence synthesis, developing new scar care interventions and promoting best outcomes. We aim to investigate healthcare professionals' perspectives on key scar outcome

Gunshot Wounds and Orthopaedic Injury at a Pediatric Level I Trauma Center.

BACKGROUND: Gunshot wounds (GSWs) and firearm-related injuries are a leading cause of pediatric unintentional injury and death. Orthopaedic firearm injuries are also associated with higher than anticipated mortality, disproportionately affecting socioeconomically and racially disadvantaged patients. The purpose of this study was to characterize GSW injuries at our institution and to identify associated risk factors for these injuries in affected populations. METHODS: A retrospective review was performed of patients aged 0-17 years who were treated for a GSW from April 2012 to October 2023 at

Determining the reliability and validity of a new method for measuring upper extremity joint range of motion in patients with burn injury using a tracking system.

DesignA cross-sectional study.SettingPatients with burn injuries often present with joint contracture caused by hypertrophic scars. To assess the patient's degree of disability or outcome to rehabilitation treatment, various objective assessment tools are used, such as a standard goniometer, which is often used for measuring joint range of motion; however, measurements using a handheld goniometer may have a large margin of error depending on the therapist's experience. Herein, the reliability and validity of a novel marker-based system were investigated for evaluating joint range of motion in

Diagnostic performance of a gravity varus stress computed tomography protocol in detecting instability in isolated coronoid fractures.

BACKGROUND: Instability associated with isolated coronoid fractures can be subtle and frequently missed on routine imaging. A stress-based computed tomography (CT) protocol may improve detection of occult instability in isolated coronoid fractures. The purpose of this study was to evaluate the diagnostic performance of a gravity varus stress CT protocol in detecting instability in isolated coronoid fractures and compare it with routine static CT. METHODS: A retrospective cohort study of patients with isolated coronoid fractures who underwent either gravity varus stress CT or routine static CT

Upper extremity fractures and subsequent total knee arthroplasty incidence in knee osteoarthritis: a cross-continental propensity score-matched cohort study.

BACKGROUND: Knee osteoarthritis (KOA) represents a major clinical and economic burden globally, and total knee arthroplasty (TKA) remains the definitive treatment for end-stage disease. Upper extremity fractures in older adults often indicate skeletal fragility and reduced physiological reserve, but their influence on subsequent TKA incidence remains unclear. We investigated whether upper extremity fractures influence TKA incidence among patients with KOA. METHODS: We conducted a retrospective propensity score-matched cohort study using the TriNetX US Collaborative Network (2005–2024) and perf

Functional results after extensor indicis proprius to extensor pollicis longus tendon transfer for ruptures associated with distal radius fractures.

Rupture of the extensor pollicis longus (EPL) tendon is a recognised complication after distal radius fracture (DRF) that leads to loss of active thumb extension. Transfer of the extensor indicis proprius (EIP) tendon to the EPL is one of the most common reconstructive procedures, but prospective outcome data are limited. This study evaluated functional and patient-reported outcomes after EIP-to-EPL tendon transfer exclusively in patients with EPL rupture secondary to DRF. Thirty consecutive patients (26 women, 4 men; median age 65 years, range 17-80) underwent EIP-to-EPL tendon transfer betwe

Cannabis and nicotine use are independently associated with adverse surgical, medical, and psychosocial outcomes following upper extremity fracture fixation.

BACKGROUND: Marijuana use is rising in the United States, yet its impact on perioperative outcomes remains poorly understood, particularly in orthopaedic trauma where cessation is often not feasible. This study evaluates the risks associated with cannabis and nicotine use in patients undergoing fixation of upper extremity fractures. METHODS: We performed a retrospective analysis of adult trauma patients with upper extremity fractures (2015-2023) identified using CPT codes for surgical fixation in the TriNetX database. Four cohorts were defined: cannabis-only users (n = 801), nicotine-only use
