BACKGROUND: High-voltage electrical burns can significantly damage the tendons and nerves, causing severe upper-limb dysfunction. Although early debridement and free flap coverage are proven techniques for limb salvage, the functional benefit of subsequent secondary tendon and nerve reconstruction has not been adequately investigated. Therefore, in this study, we assessed whether a systematic, staged reconstruction strategy improves long-term functional outcomes beyond those achieved with successful wound closure alone.
METHODS: A single-center retrospective cohort study was conducted on 42 p

Distal radius fractures are common upper extremity injuries. Some intra-articular fracture patterns may benefit from a dorsal approach to assist in optimizing joint congruency by direct visualization and reduction of the articular surface. Dorsal plating can be associated with extensor tendon irritation and possible risk of attritional rupture. However, in combination with newer-generation thinner dorsal plates or fragment-specific dorsal plating, the risk of extensor tendon complications can be minimized with the use of extensor retinacular flaps for plate coverage. This paper summarizes a te

Distal radius fractures in adults primarily occur in the elderly, in whom comorbidity, polypharmacy, dependency, and limited functional demands often coexist. Most patients have been managed non-surgically with casting. However, only a few trials with high evidence exist on comparing non-surgical and surgical treatments in the elderly. Given the heterogeneity of the aging population, a universal approach to treatment selection may not be feasible. This educational article aims to discuss the advantages and disadvantages of non-surgical vs surgical management, radiological assessment, complicat

BACKGROUND: It is undetermined whether the quality of surgical technique in a humeral shaft fracture (reduction and fixation) affects outcomes. The aim of this study was to score the quality of surgical technique performed in patients with humeral shaft fractures and to assess whether the quality of surgical technique was associated with complications and (functional) outcomes.
METHOD: A panel of expert orthopedic trauma surgeons, experienced in upper extremity surgery, assessed the quality of surgical technique (good, sufficient, or poor) in patients who participated in the HUMMER study on p

INTRODUCTION: Obese, osteoporotic patients with intertrochanteric fractures face higher surgical risks and delayed recovery. The "Gold Wrist," an auxiliary curved guiding instrument for proximal femoral nail anti-rotation (PFNA), was developed to assist guidewire placement and improve procedural efficiency.
METHODS: In this retrospective cohort study, 500 patients (almost all with BMI > 28 and varying degrees of osteoporosis) underwent PFNA using either the Gold Wrist (n = 250) or conventional instruments (n = 250). Outcomes included operative time, fluoroscopy exposure, incision length, infl

OBJECTIVE: Distal radius fracture (DRF) is the most frequent injury to the upper limb. Complications include nonunion, wrist stiffness and instability, local discomfort, and tendon irritation. The purpose of the current open-label clinical study was to evaluate the safety, efficacy, and tolerability of CL16019, an herbal formulation of Cissus quadrangularis and Boswellia serrata extract, in accelerating fracture healing in volunteers with DRFs.
METHODS: Fifty participants (both male and female; age: 30-50 years) with type I, IIA, or IIB DRFs (as per Lidstrom classification) were equally rando

PURPOSE: Because of the biological, anatomical, and biomechanical characteristics of the scaphoid, treating some proximal pole nonunions remains challenging. The location of the nonunion (proximal or distal to the dorsal apex) may influence its natural history. Based on this concept, patients with a proximal pole nonunion located proximal to the dorsal apex and maintaining an intact insertion of the dorsal scapholunate ligament in the distal fragment of the scaphoid may benefit from simple excision of the proximal pole. This study evaluated the clinical and functional outcomes of this treatmen

BACKGROUND: Distal radius fractures (DRFs) are among the most prevalent in upper-extremity injuries, particularly in older adults and individuals with osteoporosis. The common treatment options include surgical interventions such as volar locking plate (VLP) fixation and percutaneous pinning, as well as conservative strategies like splinting and bracing. However, the comparative effectiveness of these modalities remains unclear. This study aims to compare the functional outcomes and complication rates associated with VLP, percutaneous pinning, bracing, and splinting in the treatment of DRFs.

BACKGROUND: The optimal management of acute Rockwood type III acromioclavicular joint (ACJ) disruptions remains controversial. Although conservative treatment is recommended for many patients, surgical stabilization may benefit selected high-demand individuals. This study evaluated the radiographic and functional outcomes of percutaneous double-endobutton fixation in selected patients with acute Rockwood type III ACJ disruptions.
METHODS: A prospective observational case series was conducted involving 16 consecutive patients with acute Rockwood type III ACJ disruptions treated using percutane

OBJECTIVE: In the academic training environment, hand surgeons are tasked with the challenge of teaching operative skills to trainees and balancing efficiency with more complex surgical procedures. In light of a lack of evaluation regarding resident involvement during common hand surgical procedures, we sought to evaluate whether resident involvement in hand surgery cases affects postoperative complication rates and efficiency.
DESIGN: Retrospective, matched case-control study evaluating patient demographics, comorbidities, operative times, lengths of stay (LOS), and short term (30-day) posto

BACKGROUND: Monteggia lesion (ML) is a complex and rare injury in children that requires expert diagnosis and treatment. Failure to recognize or insufficiently diagnose this condition can be catastrophic for the child in terms of upper-limb motor function. The aim of this study is to analyze retrospective data of patients with ML treated at the Department of Pediatric Surgery, Comenius University and National Institute of Children's Diseases in Bratislava between 2010 and 2020, and to evaluate treatment outcomes.
MATERIAL AND METHODS: A retrospective analysis of 131 patients with ML was perfo

Intraoperative nerve injuries during orthopaedic shoulder procedures can cause pain, paresthesia, and muscle weakness. Iatrogenic nerve injuries are often transient or subclinical, and it is believed that many neurologic injuries go undiagnosed. Several risk factors predispose patients to intraoperative nerve injury, including local tissue stiffness, revision surgery, low body mass index, medications, and decreased range of motion. Apart from patient-specific risk factors, there are direct and indirect mechanisms of injury related to surgical technique, including device implantation, nerve tra

It is inevitable that every orthopaedic surgeon will need to treat patients with pathologic or impending pathologic fractures secondary to metastatic disease. Although the input of an orthopaedic oncologist is usually helpful and prudent, transferring care to an orthopaedic oncologist may not be feasible at all institutions in a prompt time frame. To avoid potentially detrimental delays to the patient, many cases can be appropriately managed or resolved by a nononcologic orthopaedic surgeon. It is therefore imperative for all orthopaedic surgeons to be familiar with the appropriate clinical an

BackgroundThe modified anterior neurovascular interval approach provides direct access to the anterior coronoid while preserving the flexor-pronator origin. However, the narrow working corridor makes conventional mini-plate fixation technically challenging, often requiring repeated contouring and supplementary lag screws. We developed a specialized anatomical hook-plate (SAHP) and compared it with conventional mini-plate (CMP) fixation.MethodsWe retrospectively reviewed patients with unstable ulnar coronoid fractures (Regan-Morrey type II/III) treated via the anterior approach between January

BACKGROUND AND PURPOSE: Osteoporosis increases the risk of fragility fractures, contributes to significant morbidity, and is a psychological challenge-fear of falling (FoF)-which can limit activity and engagement in rehabilitation. These challenges are often overlooked after a fracture, despite their relevance to rehabilitation planning. Understanding these challenges is crucial for health care practitioners, particularly physical and occupational therapists, who are central to fall prevention and post-fracture care. This study explored the perspectives of older adults with osteoporosis who ha
