Efficacy of topical vancomycin in preventing surgical site infection following fracture surgery : a systematic review and meta-analysis.

AIMS: Fracture-related infection (FRI) is a major cause of morbidity following limb fracture surgery. Topical vancomycin has gained popularity as an adjunct to systemic prophylaxis; however, concerns regarding nephrotoxicity, impaired bone healing, and wound complications remain. This review aims to evaluate the efficacy and safety of intraoperative topical vancomycin powder in preventing FRI in patients undergoing surgical management of upper or lower limb fractures. METHODS: A systematic review of randomized controlled trials (RCTs) was conducted in February 2026. Comprehensive searches of

Open Fractures in Polytrauma and Non-Polytrauma Patients: Outcomes and Predictive Factors in a Level III Trauma Center.

PURPOSE OF THE STUDY: Open fractures represent complex injuries characterized by bone exposure, soft-tissue damage, and a high risk of infection, often requiring multidisciplinary management. In polytraumatized patients, these injuries further increase clinical complexity and hospital resource utilization. The purpose of this study was to evaluate adherence to international guidelines in the management of open fractures at a third-level trauma center and to identify clinical and organizational predictors of outcomes, complications, and hospitalization burden in polytraumatized and non-polytrau

Paediatric fracture complications following traditional bone setting in low- and middle-income countries: a systematic review and meta-analysis protocol.

INTRODUCTION AND AIMS: Traditional bone setters (TBS)-lay practitioners who manage fractures without formal medical qualification-treat an estimated 10%-85% of fracture patients in low- and middle-income countries (LMICs). Children are disproportionately vulnerable due to physeal immaturity and lower diastolic blood pressure thresholds that increase susceptibility to compartment syndrome at lower absolute intracompartmental pressures than in adults. No prior systematic review has restricted analysis exclusively to the paediatric population or generated pooled meta-analytic estimates of complic

Fatal arrhythmia associated with myocardial contusion caused by a horse kick: A forensic autopsy case.

BACKGROUND: Horse-related injuries often involve high-energy blunt trauma that can be fatal. In cases of chest trauma associated with horse kicks, death is typically attributed to structural cardiac injuries such as cardiac rupture or tamponade. Fatal arrhythmia associated with myocardial contusion following a horse kick has rarely been reported in the forensic literature. CASE PRESENTATION: A 48-year-old man was kicked in the left lower chest by the right hind limb of a Thoroughbred horse while handling the animal. The patient collapsed immediately after the impact. Documented Penicillin Allergy Is Associated with Increased Post-Operative Complications Following Lower Extremity Fracture Fixation.

INTRODUCTION: Penicillin allergy is a risk factor for infection in an elective orthopedic surgical procedure, but this has not been studied in orthopedic trauma. We hypothesize there will be an increase in deep surgical site infections in patients sustaining lower extremity fractures with documented penicillin allergy. METHODS: A retrospective cohort study was performed at an academic, level-one trauma center of patients who underwent surgical fixation of lower extremity fractures. Cohorts were divided based on the documented presence of a penicillin allergy in the medical record. The primary

Management, reconstructive strategies and outcomes following open tibia/fibula and ankle fractures: An age-based comparison.

INTRODUCTION: Open lower limb fractures require multidisciplinary orthoplastic management. With an ageing population, older patients present with distinct injury patterns, greater comorbidity and reduced physiological reserve. In this study, we compared presentation, management and outcomes between younger and older patients. METHODS: A 4-year retrospective complete case cohort study (2018-2022) was conducted at a tertiary major trauma centre. Adult patients with open tibia, fibula or ankle fractures were included. Patients were stratified into younger adult (<65 years) and older adult (≥65 y

Mini-Open-Assisted Screw Fixation for Displaced Intra-Articular Calcaneus Fractures: Three Case Reports of Patients With Good Postoperative Outcomes.

Displaced intra-articular calcaneus fractures (DIACFs) are debilitating injuries often resulting from a fall from height or motor vehicle accidents. Traditional fixation methods involve extended incisions that can lead to soft tissue breakdown or necrosis. Novel techniques such as percutaneous fixation augmented by mini-open sinus tarsi approaches are theoretically associated with fewer soft tissue complications while achieving noninferior reduction and fixation. We introduced this technique to our peripheral hospital and completed this study as part of a departmental audit and self-critique.

[Injuries of the toes].

Toe injuries are common in clinical practice and cover a wide spectrum, from simple fractures and dislocations to complex tendon and soft tissue injuries. While most injuries can be conservatively treated, displaced fractures, open injuries or relevant loss of function require a differentiated surgical approach. Rare but prognostically important entities, such as décollement injuries or proximal tendon ruptures pose a particular challenge. A structured clinical examination supported, if necessary, by imaging is essential for making the correct diagnosis and deciding on the appropriate treatmen

The masquelet technique for severe open fractures: current insights.

Open fractures of the lower extremity remain challenging to manage even in the most experienced surgical teams. These injuries when they present with bone loss pose even greater challenges with often unpredictable outcomes. Whilst traditionally, bone transport has been used as the surgical technique to treat open long bone injuries with bone loss, recently the Masquelet technique has gained great popularity amongst surgeons to manage these injuries. Understanding better the role of the induced membrane, its molecular signature (possessing angiogenic, osteogenic and inductive properties) and th

Effects of virtual walking on neuropathic pain following spinal cord injury: a systematic review.

BACKGROUND: Neuropathic pain following spinal cord injury (SCI) is associated with significant functional and psychological impairments. Virtual walking is a form of virtual reality–based intervention that simulates lower-limb movement through visual feedback, using either immersive or non-immersive environments. It has been proposed as a potential strategy to reduce neuropathic pain. This review aims to evaluate and synthesize the existing literature on virtual walking interventions in individuals with SCI, focusing on their effects on pain intensity, pain quality, pain interference, and pain

A Novel Minimally Invasive Technique for Hallux Fracture-Dislocation Fixation Using Hypodermic Needles: An Effective Solution for Disaster and Catastrophic Settings.

Hallux fractures, particularly those involving the proximal phalanx, can lead to significant functional impairments if not promptly managed. This report introduces a novel, minimally invasive technique for hallux fracture fixation using hypodermic needles. This technique is specifically designed for resource-limited settings such as disasters or emergencies. A 30-year-old female sustained an open proximal phalanx fracture of the left hallux with interphalangeal joint dislocation following a motor vehicle accident. Under local anesthesia in the emergency service settings, fracture reduction and

The role of plastic reconstructive surgery in surgical reconstruction of soft tissue defects after resection of musculoskeletal tumors.

BACKGROUND: Musculoskeletal tumor resections often result in large soft tissue defects that require complex reconstructive surgery to restore function and appearance. An interdisciplinary "orthoplastic" approach-coordinating orthopedic and plastic surgical expertise-aims to ensure durable wound coverage, minimize complications, and facilitate timely adjuvant therapies, ultimately improving functional recovery and oncologic outcomes. METHODS: We retrospectively analyzed patients (2013-2020) at the First Department of Orthopedic Surgery, Athens University Medical School, "Attikon" University Ge

Treatment of popliteal artery injuries: a comparison of application patterns and short-term outcomes between open surgical and endovascular repair.

BACKGROUND: After femoral artery injury, popliteal artery injury (PAI) is the second most frequent form of arterial injury in the lower extremities. Its exceptionally high risk of limb loss has attracted a lot of attention in the field of acute lower extremity vascular damage. Nowadays, open surgical repair or endovascular surgical repair are the main methods used to treat popliteal artery damage due to developments in medical technology. The effectiveness and complication rates of these two surgical techniques are still hotly debated, though, and there is a lot of room for advancement in this

Effectiveness of indocyanine green in assessing tissue perfusion in lower limb open fractures: A prospective pilot study.

BACKGROUND: Lower limb open fractures (LLOFs) represent complex injuries in which accurate intraoperative assessment of tissue viability is crucial to prevent infection, necrosis, and delayed healing. Clinical evaluation of perfusion is subjective and may underestimate ischemic areas, especially in high-energy trauma. Indocyanine green (ICG) fluorescence angiography provides a real-time, objective visualization of tissue perfusion and may optimize surgical decision-making during debridement. MATERIALS AND METHODS: This prospective, single-center case series included patients with Gustilo-Ande
