When Coverage Isn't Enough: The Sociodemographic Impact on Weight-Bearing Progression After Flap Reconstruction for Lower Extremity Open Fractures.

BACKGROUND: Lower extremity (LE) open fractures are difficult wounds to manage, oftentimes requiring flap reconstruction. These procedures involve complex recoveries that impact daily function and quality of life, with early weight-bearing progression as a key milestone in recovery. Few studies have examined how factors influence weight-bearing progression in patients undergoing flap reconstruction after traumatic LE open fractures. METHODS: All patients > 18 years old between 2012 and 2024 who presented with a Gustilo III or higher LE open fracture and underwent flap reconstruction at our le

Immediate Versus Delayed Repair for Lower Extremity Arterial Injury: A Propensity Score-Matched Analysis.

INTRODUCTION: Delayed repair is often used for lower extremity arterial injuries, typically following temporary intravascular shunting. However, supporting evidence remains limited. We hypothesized that delayed repair is not associated with inferior outcomes compared to immediate repair. METHODS: Adult patients (aged ≥16 y) undergoing open lower extremity vascular repair between 2017 and 2021 were identified using the Trauma Quality Improvement Project database. Repairs performed within 4 h of arrival were classified as immediate, whereas those performed thereafter were considered delayed. Co

Exploring perfusion limits in pedicled perforator propeller flaps via intraoperative indocyanine green angiography.

BACKGROUND: Pedicled perforator propeller flaps are widely used for soft-tissue reconstruction because they preserve donor-site function while providing reliable defect coverage. However, intraoperative assessment of flap perfusion remains challenging, particularly in longer flaps or after axial rotation. Indocyanine green (ICG) fluorescence angiography allows real-time visualization of tissue perfusion and may improve intraoperative decision-making. METHODS: A retrospective single-center study was performed including 30 patients who underwent reconstruction with pedicled perforator propeller

Development and Evaluation of a Smartphone Application for Patients After Vascular Surgery.

BACKGROUND: Unplanned hospital readmission following vascular surgery remains common and is often related to wound complications, poor communication, and inadequate post-discharge support. We developed a patient-centered smartphone application, the Vascular Surgery Discharge Application (VASDA), and conducted a qualitative and quantitative feasibility study to evaluate patient experiences and perceptions after a variety open lower extremity bypass procedures (LEB). METHODS: A single-center, mixed-methods feasibility study enrolled patients undergoing LEB at discharge and patients were instruc

Open Fractures Beyond Long Bones: Rethinking Age-Dependent Injury Mechanisms and Anatomical Patterns in Pediatric Trauma.

Background: Open fractures are traditionally associated with high-energy trauma, such as falls and traffic accidents, in healthy adults. However, the epidemiological characteristics of such injuries in children and adolescents remain incompletely defined. Most studies focus on long-bone injuries, while comprehensive epidemiological data across all anatomical regions are limited. This study aimed to analyze and provide an overview of age-dependent patterns of injury mechanisms and anatomical distribution in pediatric open fractures. Methods: This retrospective cohort study included all open fra

Bilateral Knee Dislocation in an Elderly Patient: From Vascular Emergency to Functional Recovery.

Traumatic knee dislocations are rare but serious injuries associated with limb-threatening vascular compromise and a frequently poor functional prognosis. We report the case of a 70-year-old man with known dementia who sustained a high-energy road traffic accident resulting in bilateral knee dislocation. On admission, he was hemodynamically stable with preserved and symmetrical distal peripheral pulses. Initial radiographs confirmed bilateral dislocation, and computed tomography angiography of the lower limbs showed no arterial injury, particularly of the popliteal arteries. After pre-anesthet

Optimal Timing in Reconstruction for Traumatic Lower Extremity Injuries.

Open lower extremity musculoskeletal injuries that require soft tissue reconstruction carry significant risks of complications, including flap failure, infection, and nonunion. These complications predispose patients to worse physical, psychological, and economic outcomes and negatively impact quality of life. Teaching based on the work of Marko Godina suggests that flap reconstruction performed within 72 hours of injury reduces major complications. However, contemporary studies incorporating advances in negative pressure wound therapy suggest that flap reconstruction within 7 days may provide

Lower Extremity Reconstruction for Open Fractures with Associated Vascular Injuries: Updates in the Management of Gustilo IIIC Injuries.

Gustilo IIIC fractures represent the most severe spectrum of lower extremity trauma, characterized by high-energy skeletal injury, vascular disruption, and soft-tissue loss. Contemporary management demands early orthoplastic collaboration, rapid revascularization, and thoughtful soft-tissue reconstruction to optimize limb salvage. This review synthesizes current evidence and evolving strategies, including timing of coverage, flap selection, and adjunctive microsurgical techniques including flow through flaps, to guide decision-making in these complex injuries. Emphasis is placed on functional

Open Book on the Water Slide: A Case Series of APC2 Pelvic Ring Injuries from High-Energy Aquatic Accidents.

Background and Objectives: Pelvic ring injuries with symphyseal disruption are classically associated with high-energy mechanisms such as motor vehicle collisions. Recently, waterslides have emerged as an underrecognized but distinct source of severe pelvic trauma. Waterslide-related pelvic trauma represents a distinct biomechanical entity characterized by a supine or semi-supine body position at splashdown, extreme forced hip abduction, asymmetric lower-limb positioning, and abrupt hydrodynamic deceleration. The high descent velocity, abrupt hydrodynamic deceleration, and forced hip abduction

Anterior auxiliary limited open incision for Schatzker IV total medial plateau fractures with posterolateral collapse: a retrospective study.

BACKGROUND: Schatzker type IV total medial plateau fractures with posterolateral articular surface collapse, abbreviated as Schatzker IV-TMP/PLC, present a complex surgical problem. The intact anterolateral rim obstructs conventional access to the posterolateral depression, while the frequent entrapment of the lateral meniscus adds a critical soft-tissue dimension to the challenge. This study describes and evaluates a anterior limited open incision that is designed to utilize the main fracture line as a direct corridor for addressing both the osseous and soft-tissue components of this injury.
