Pedicled extensor digitorum brevis flap for the distal third lower limb reconstruction: A review of the literature.

INTRODUCTION: Reconstruction of the distal lower limb is often complex due to limited local tissues. The pedicled extensor digitorum brevis (EDB) flap is a reliable, loco-regional option for small- to medium-sized ankle and foot defects. This review summarizes literature on pedicled EDB flaps, focusing on indications, outcomes, and donor-site morbidity. MATERIALS AND METHODS: A search of PubMed, Embase, and Scopus (1987-2024) using predefined terms identified studies on pedicled EDB flaps for lower limb reconstruction. Exclusions included anatomical studies, case reports, and free or multiple

Masquelet technique in adolescent distal humerus fractures with bone loss.

Adolescent distal humerus fractures usually result from high-energy trauma; however, open fractures with associated bone loss are quite rare. Although the Masquelet technique has demonstrated proven success in the reconstruction of lower extremity bone defects, its use in upper extremity periarticular defects, especially in adolescents, has been very rarely reported. This study aims to present the clinical and radiological outcomes of the Masquelet technique in adolescent distal humerus fractures with bone loss. A retrospective review was conducted of patients aged 12-16 years who underwent th

Penetrating Injury to the Lower Limb by a Needlefish: A Case Report.

Needlefish are elongated marine fish known for their high-speed leaps toward light sources, occasionally resulting in penetrating injuries in humans. We report a case of a 42-year-old fisherman who presented with a penetrating injury to the left leg caused by a needlefish beak. Clinical examination revealed a small entry wound with a protruding foreign body, and imaging confirmed multiple retained fragments within the soft tissue. The patient underwent surgical exploration and removal of the foreign bodies under spinal anesthesia, followed by wound debridement and irrigation. Due to the risk o

Novel Porcine Xenograft as a Salvage Strategy for Lower Extremity Wound Reconstruction Following Free Flap Failure.

BACKGROUND: Lower extremity wounds involving the proximal and middle tibia remain a reconstructive challenge, especially following free flap failure. Traditional salvage strategies are limited in patients with comorbidities or wound contamination. Porcine-derived xenografts, such as XCellistem, offer a biologically active scaffold to promote granulation tissue and enable secondary grafting. METHODS: This case series included 2 male patients with complex tibial wounds and failed anterolateral thigh free flaps. Both underwent surgical debridement followed by multilayered application of the porc

Open 4-Compartment Fasciotomy for Chronic Exertional Compartment Syndrome of the Lower Extremity.

BACKGROUND: Chronic exertional compartment syndrome (CECS) is an underrecognized cause of lower extremity dysfunction in athletes. Open 4-compartment fasciotomy remains the gold standard for surgical management. INDICATIONS: Operative intervention is indicated for patients with clinically and objectively confirmed CECS who fail to respond to conservative management. Contraindications include inadequate nonoperative treatment or unaddressed medical comorbidities. Proper diagnosis requires exclusion of vascular or neurologic causes that can mimic CECS symptoms. TECHNIQUE DESCRIPTION: A 2-incis

Major limb replantation: Current status and perspectives.

BACKGROUND: Major limb replantation is a demanding microsurgical procedure aiming to restore form and function after traumatic amputation. Although survival has improved since the 1960s, ischemia, infection, and complex reconstruction continue to limit outcomes. Emerging preservation and reconstructive strategies may help expand indications and improve results. METHODS: For this scoping review, MEDLINE, Embase, and the Cochrane Library were searched from inception through December 2024 using predefined MeSH terms related to major limb replantation. Adult case series and reports were included;

The DRoPH score: development of a tool for predicting reduction difficulty in proximal humerus fracture osteosynthesis.

BACKGROUND: Proximal humerus fractures (PHFs) are among the most common and challenging fractures due to complex anatomy and variable fracture patterns. While anatomical reduction is critical for successful outcomes, no tool currently predicts the intraoperative difficulty of achieving it. Despite advances in surgical techniques, complications persist. Existing classification systems, such as Codman's, Neer's, AO/OTA, and Mayo-FJD, focus on fracture morphology and displacement but do not address intraoperative complexity. This study aims to develop a new score for the early prediction of intra

Association of Cigarette Smoking With Pulmonary Embolism and Healthcare Utilization Following Hip and Knee Versus Shoulder Arthroscopy.

BACKGROUND: Cigarette smoking is a well-established risk factor for adverse surgical outcomes due to its prothrombotic, inflammatory, and cardiopulmonary effects. However, its impact on short-term complications following arthroscopic surgery remains poorly defined. The purpose of the study is to evaluate the association between cigarette smoking and 90-day postoperative complications after shoulder, hip, and knee arthroscopic procedures. METHODS: Using a multicenter database, TriNetX, patients undergoing arthroscopic shoulder, hip, or knee procedures were retrospectively identified. Smokers w

Ergonomic evaluation of a novel suturing device (SutureTOOL) versus traditional needle-driver for aponeurotic closure: a randomized experimental study.

INTRODUCTION: Adequate aponeurotic closure is essential to reduce postoperative complications, particularly incisional hernia. However, maintaining guideline-recommended closure principles such as a suture length-to-wound length (SL/WL) ratio ≥ 4 can be technically demanding and is often performed at the end of long procedures. The SutureTOOL is a handheld device designed to standardize stitch placement, facilitate SL/WL ≥ 4, and potentially reduce operator workload. METHODS: Sixteen physicians (8 residents, 8 surgeons) performed eight 12-cm aponeurosis closures on silicone models using either

The Importance of Plastic Surgery in Multidisciplinary Teams: A Systematic Review and Meta-analysis of Comparative Studies.

BACKGROUND: Multidisciplinary team (MDT) care facilitates coordinated expertise that leads to improved patient safety and outcomes. Yet, no prior study has quantified reported benefits from plastic surgery-inclusive MDTs across several procedures. We thereby hypothesized that plastic surgery-inclusive MDTs would lead to better surgical outcomes following cranioplasty, spine surgery, abdominoperineal resection, abdominal wall reconstruction, and extremity reconstruction, compared with the primary specialties acting alone. METHODS: Following Preferred Reporting Items for Systematic Reviews and

Conduit choice in open repair of iliac artery injuries: A comparative analysis of in-hospital outcomes from the National Trauma Data Bank.

OBJECTIVE: Iliac artery trauma is associated with substantial morbidity and mortality. Although open bypass or patch arterioplasty remain common even in the endovascular era, associations between conduit choice and outcomes remain unclear. This study compares short-term outcomes among conduit types for iliac artery injuries in a national cohort. METHODS: Patients undergoing open patch or bypass repair of an iliac artery injury were identified from the National Trauma Data Bank (2017-2023) via deterministic, fully supervised natural language processing techniques and stratified by repair mater

Impact of altered consciousness on the risk of fasciotomy for acute compartment syndrome following traumatic tibial fracture.

BACKGROUND: Acute compartment syndrome (ACS) following tibial fracture is a limb-threatening emergency that relies heavily on clinical assessment for diagnosis. In patients with altered consciousness (hereafter referred to as obtunded status), key symptoms such as pain out of proportion cannot be reliably assessed, potentially influencing surgical decision-making. Whether obtunded status independently affects fasciotomy rates remains poorly understood. OBJECTIVE: To determine whether obtunded status is independently associated with increased likelihood of fasciotomy for suspected ACS in patie
