Lower Extremity Fillet Flap With Plantar Skin for Reconstruction of Pressure-Related Ulceration in a Patient With Transverse Myelitis.

Transverse myelitis is a rare, acquired inflammatory neurologic disorder that can result in chronic disability and secondary complications, including decubitus ulcers. These ulcers may serve as portals for infection and can progress to osteomyelitis, particularly when involving high-pressure areas. We present the case of a 45-year-old female with chronic transverse myelitis who developed a stage 4 perineal, ischial, and femoral decubitus ulcer complicated by acute-on-chronic osteomyelitis of the pelvis and femur. The patient underwent a left lower extremity amputation with preservation of the

A Comparative Study of the Radiological and Functional Outcomes Between the Titanium Elastic Nailing System and Long-Leg Cast Immobilization in Pediatric Femoral Shaft Fractures: A Retrospective Study.

BACKGROUND: Pediatric femoral shaft fractures are common injuries that can cause significant morbidity and functional impairment if not treated properly. Traditionally, long-leg cast immobilization has been employed; however, prolonged immobilization may lead to joint stiffness, muscle atrophy, limb-length discrepancy, and delayed functional recovery. The titanium elastic nailing system (TENS) provides elastic, stable intramedullary fixation, promotes biological healing, and enables early mobilization. This study compared radiological union, limb length discrepancy, callus formation, pain, ran

Locoregional Surgery for Infected Penile Squamous Cell Carcinoma With Bulky Nodal Disease: A Case Report.

For chemotherapy-fit patients with clinical N3 (cN3) penile squamous cell carcinoma (SCC), contemporary guidelines favor neoadjuvant cisplatin- and taxane-based chemotherapy followed by consolidative surgery when feasible. Severe tumor-associated infection and necrosis may, however, temporarily alter this sequence. A 53-year-old man with poorly differentiated penile SCC had previously undergone glansectomy and left inguinal lymphadenectomy, followed by one cycle of paclitaxel, ifosfamide, and cisplatin (TIP). After relocating to São Paulo, he was lost to oncologic follow-up and did not complet

Circumferential Negative Pressure Lymphedema Therapy: A Novel Rapid Perioperative Decongestion Strategy for Advanced Stage III-IV Lymphedema.

INTRODUCTION: Advanced Stage III-IV lymphedema, particularly in filariasis-endemic regions, presents with severe fibrosis, massive limb enlargement, and a poor response to conventional conservative therapy, complicating reductive surgical interventions in resource-limited settings. This study evaluated circumferential negative pressure lymphedema therapy (CNPLT), a novel rapid preoperative decongestion technique, for transforming inoperable limbs into suitable surgical candidates. METHODS: This prospective clinical study enrolled 14 patients (16 limbs) with Stage III-IV lower extremity lymphe

Giant Pseudoaneurysm as an Uncommon Late Complication Following a Fourteen-Year Femoropopliteal Bypass in a Visually Impaired Patient.

Background: Non-anastomotic pseudoaneurysm formation due to very late prosthetic graft failure after femoropopliteal bypass is exceptionally rare. Case Presentation: We describe a 73-year-old blind man who presented with rapid enlargement of a mid-thigh mass on the left side, associated with skin necrosis. His history included advanced atherosclerosis with bilateral superficial femoral artery occlusion and prior femoropopliteal bypasses: a right-sided great saphenous vein graft (2006) and a left-sided Dacron® graft (2008). Computed tomography angiography revealed a giant pseudoaneurysm origina

Introducing the Minimally Invasive Novel Open Revascularization - The "MINOR" Approach - To Improve Outcomes in Lower and Upper Limb Arterial Bypass Surgery.

BACKGROUND: Traditional open limb bypass surgery has risks of perioperative complications and prolonged recovery due to large wounds and extensive dissections. We describe a new concept in bypass revascularization, the minimally invasive novel open revascularization (MINOR) approach - which has 3 elements: shortest possible length of vein bypass, endoscopic vein harvesting (EVH), limited incisions to minimise tissue damage. METHODS: Analysis of prospectively collected data over 18 months on sequential, unselected patients treated by the MINOR approach. Primary outcomes were technical success,

Lumbar Spine Surgery in a Patient With Recent Monkey Bite Receiving Rabies Immunoglobulin: A Case Report on Perioperative Anesthetic Challenges.

Rabies is a universally fatal acute zoonotic disease caused by rhabdovirus, and postexposure prophylaxis (PEP) remains the mainstay of prevention. Patients who sustain animal bites may require urgent or elective surgical interventions for unrelated conditions while simultaneously undergoing rabies prophylaxis. The perioperative team often faces uncertainty regarding the safety of anesthesia and surgery in patients receiving rabies vaccines or rabies immunoglobulin (RIG). This report discusses such a scenario in a patient posted for elective lumbar spine surgery. We report the case of a male in

Timing of Flap Coverage in Open Fractures: A Systematic Review and Meta-Analysis.

Open tibial fractures remain the most prevalent open fracture, necessitating orthoplastic management with flap reconstruction. Early soft tissue coverage, ideally within 72 hours, is widely regarded as the gold standard for reducing infection, promoting bone healing, and improving limb salvage. However, injury severity, patient comorbidities, and pre-theater coordination pose challenges to the recommended management window, thereby increasing the number of patient-reported complications. A systematic review and meta-analysis were conducted in accordance with the Preferred Reporting Items for S

Drug-eluting balloon angioplasty versus uncoated balloon angioplasty for peripheral arterial disease of the lower limbs.

BACKGROUND: Atherosclerotic peripheral arterial disease (PAD) can lead to chronic limb-threatening ischemia (CLTI) and limb loss. Treatments include lifestyle modifications, medications, and both open and minimally invasive operative approaches, including balloon angioplasty. Drug-eluting balloon (DEB) angioplasty is a promising alternative to uncoated balloon angioplasty for treating PAD. Ballooning and coating the inside of atherosclerotic vessels with cytotoxic agents inhibits cellular mechanisms responsible for atherosclerosis and neointimal hyperplasia, thereby preventing or postponing it

Topical antibiotic powder reduces fracture-related infections in pelvic and lower limb fractures: a systematic review and meta-analysis.

AIMS: Fracture-related infection (FRI) remains a significant public health issue. Intrawound vancomycin and tobramycin powder achieve high local antibiotic concentrations, yet their efficacy and safety remain uncertain. We aimed to evaluate the effect of prophylactic topical antibiotic powder in patients undergoing operative treatment for pelvic and lower-limb fractures. METHODS: PubMed, Embase, and the Cochrane Library were systematically searched for randomized controlled trials (RCTs) and observational studies comparing intrawound antibiotic powder (vancomycin and/or tobramycin) with stand

Residential radon in Peru: challenges for health surveillance and lung cancer prevention.

The Journal of Genetics, started by William Bateson in 1910, played a distinguished role in the early years of genetics. However, it stopped publishing in 1978. The Indian Academy of Sciences revived it in 1985, and has published it regularly since then. To commemorate this landmark, I highlight one of the 17 articles published that year. 'The isolation and genetic analysis of a Caenorhabditis elegans… X-chromosome balancer' by András Fodor and Péter Deak, of the Institute of Genetics, Biological Research Centre, Hungarian Academy of Sciences, Szeged, Hungary. Severe Hyperkeratosis Associated With Chronic Lower-Extremity Lymphedema Complicated by Cellulitis: A Case Report.

Elephantiasis nostras verrucosa (ENV) is a rare, end-stage complication of chronic nonfilarial lymphedema, marked by progressive dermal fibrosis, hyperkeratosis, and verrucous skin changes, which increase susceptibility to recurrent infections and chronic wounds. We present a 77-year-old man with longstanding bilateral lower-extremity lymphedema, severe hyperkeratosis, malodorous drainage, and polymicrobial wound infection. Examination revealed extensive verrucous lesions with worsening edema and drainage from chronic venous stasis wounds, along with hyperkeratotic changes consistent with ENV.

Pragmatic randomised controlled trial to evaluate the clinical and cost-effectiveness of compression therapy for surgical wounds healing by secondary intention following excision of lower limb keratinocyte cancers in UK skin cancer surgery centres (HEALS2 trial): study protocol.

INTRODUCTION: Keratinocyte cancers (KCs) are increasing in incidence in the UK, with over 234 000 cases annually, largely due to demographic changes and sun exposure. Skin cancers, including malignant melanoma and KCs, are projected to cost the NHS over £180 million annually and impose healthcare demands. Although KCs have high cure rates following surgical excision and primary closure, wounds healing by secondary intention (HBSI) particularly on the lower leg present clinical challenges. Compression therapy (CT) known to improve venous insufficiency and venous ulcer healing may also benefit H

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l
