Injuries to the tarsometatarsal (Lisfranc) joint are not common, and the results of treatment are often unsatisfactory. Since no individual is likely to see many such injuries, we decided to make a retrospective study of patients from five different centres. In this way 119 patients with injuries of the Lisfranc joint have been collected. This paper classifies these injuries and describes their incidence, mechanism of production, methods of treatment, results and complications. Six children suffered Salter I fractures of the distal phalanx of a great toe caused by stubbing. The first four children presented with cellulitis and osteomyelitis. The last two were treated promptly with antibiotics, and no infection developed. Analysis of the anatomic relationship of the distal phalanx and nail indicates that these fractures were compound despite the absence of usual clinical signs. Recognition of these occult compound fractures and treatment with antibiotics should prevent infectious complications.

Utilization of a free omental transfer with a skin graft in two lower limbs for soft tissue and skin cover in difficult situations is reported. In 1 patient, it was used to cover open and damaged joints around the ankle, and in the other, for the reconstruction of a circumferential defect in a lower limb. The results were quite satisfactory.

The stable closure of an indolent wound below the knee may require the placement of well vascularized flap tissue. Recent advancements in axial and microvascular flap transfer have nearly obviated the use of the cumbersome traditional cross-leg flap. Thirty-two flaps in 25 below-knee defects illustrate a variety of surgical options. Closed stable wounds were achieved in 24 of the 25 defects, and bony union was accomplished in 11 of 12 unhealed fractures.

In 15 patients traction followed by cast brace was an acceptable method of treatment for ipsilateral fractures of the tibia and femur. A retrospective study of 260 cases of motorcycle accidents which occurred over a 4 1/2 year period is reported. Approximately 8% died of their injuries. Sixty-seven patients with 86 fractures of the calcaneum were re-examined 2 to 6 years after their injury. The authors analyse a series of 70 bimalleolar fractures treated surgically with a follow up of 3 to 8 years. The late functional and radiological result was favourable in 85.7% of cases.
