Reconstructive venous surgery is essential in lower limb injuries to avoid circulatory insufficiency when the main vein is severed. The repair requires a vein graft with adequate lumen--two segments of the saphenous vein are slit open and sutured together. A technique for atraumatic preparation of this composite graft is described. A peripheral temporary A-V fistula is advocated to prevent thrombosis of the graft.

We reviewed our total experience with arteriovenous (AV) fistulas for long-term hemodialysis. We are unable to show any significant difference in the survival of AV fistulas based either on the type of material used to create the fistulas or on their location. A patient with fracture-dislocation of D9 on D8 had considerable posterolateral displacement. The neurologic injolvement of his left leg proved to be progressive. Treatment consisted of posterior decompression, exploration, open reduction and stabilization by means of Harrington rods, and fusion. All neurologic findings resolved completely. Final evaluation 18 months later showed a good and serviceable spine with no residual neurologic deficits. In my opinion, the treatment used in this case under given circumstances represented a reasonable modality of therapy.

Twenty hip fractures in 19 children aged 2 1/2 to 18 were analyzed in a ten-year retrospective review at the University of Alabama Hospitals in Birmingham, Ala. There was one epiphyseal, one intertrochanteric, 4 subtrochanteric, and 14 intracapsular fractures. 30 fracture-dislocations of the hip were operated in the years 1967-1973. Indication, operative approach, X-ray diagnostic and complications are discussed. Postoperatively continuous traction is applied for some weeks, 26 patients were followed up with 11 very good, 10 good, 3 fair and 2 poor results. It is pointed out, that open reduction with interfragmentar compression - osteosynthesis is a real progress in the treatment of irreducable fracture-dislocations of the hip.

A series of 547 patients (678 extremities) subjected to reconstructive arterial surgery is presented, with a minimum observation period of 2 years. Based on the analysis of 45 cases of comminuted fractures of the lower extremity of the radius, this study points out the insufficiency of conventional treatments. The authors advocate a technique using a Kirschner's wire passed through the metacarpal bones and another one through the ulna, both beind embeded in plaster cast. This method was tested on 19 cases with encouraging results. The technique was precisely described and the authors think that it should be used in fractures incorrectly reduced by conventional methods.

A method of treatment of pyogenic arthritis of the adult knee employing wide open arthrotomy and early active motion, as well as synovectomy in some knees, was used in thirty-four patients with pyogenic arthritis of the knee. Of these patients, sixteen had a good result, twelve had a fair result, and six had a poor result. This method of treatment was used as a salvage procedure in most of the patients, but may have more general application in the treatment of pyogenic arthritis of the knee.
