Primary Hyperparathyroidism Presenting as a Distal Femur Pathologic Fracture in an Adolescent: A Case Report.

CASE: A 17-year-old adolescent boy presented with a pathologic distal femur fracture and bilateral distal femoral metadiaphyseal lytic lesions. Multidisciplinary evaluation confirmed bilateral distal femoral brown tumors associated with primary hyperparathyroidism secondary to a parathyroid adenoma. Treatment included bone biopsy, curettage, grafting, and retrograde locked intramedullary nail, followed by definitive partial parathyroidectomy. The contralateral metadiaphyseal lesion was treated with corrective osteotomy and prophylactic intramedullary stabilization. CONCLUSION: Primary hyperpa

Intramedullary Nailing of Femoral Shaft Fractures: Implications of Interlocking Screw Breakage.

Introduction Femoral shaft fractures are commonly treated with intramedullary nailing (IMN), though complications such as delayed union, nonunion, and hardware failure remain significant challenges. Interlocking screw breakage has unclear implications for fracture healing. Our study examined patients with femoral shaft fractures treated with IMN who developed interlocking screw breakage to evaluate outcomes and complications. Methods The retrospective cohort study included 551 patients with femoral shaft fractures (AO Foundation (AO)/Orthopaedic Trauma Association (OTA) 32A/B/C) treated with I

[Multidisciplinary Management of Bone Metastases in Clinical Practice].

Bone metastases occur in approximately two-thirds of patients with advanced cancer and are associated with pain, fractures, and neurological complications. The recently revised Dutch multidisciplinary guideline on bone metastases emphasizes early diagnosis, personalized treatment, and coordinated multidisciplinary care. We present three clinical cases highlighting key aspects of management: delayed diagnosis leading to femoral destruction and loss of mobility; a missed opportunity for prophylactic fixation in a patient with limited prognosis; and curative-intent stereotactic radiotherapy for a

Prehospital pelvic binder use and transfusion requirements in patients with pelvic fractures: A nationwide propensity-matched cohort study.

PURPOSE: Pelvic binders are recommended for suspected pelvic fractures to provide early stabilization and hemorrhage control, but robust comparative evidence regarding their real-world impact on transfusion requirements and subsequent hemorrhage-control strategies remains limited. We evaluated the association between prehospital pelvic binder use, early transfusion requirements, and pelvic angioembolization in a nationwide trauma registry. METHODS: We conducted a retrospective cohort study using the Japan Trauma Data Bank from 2019 to 2023. Adult patients with blunt pelvic fractures were incl

Is Adolescent Age a Contraindication to Titanium Elastic Nailing for Femoral Shaft Fractures? A Retrospective Comparative Study.

BACKGROUND: Titanium elastic nailing system (TENS) is widely used for pediatric femoral shaft fractures in school-age children, but its use in adolescents remains debated because greater age and body weight may increase mechanical demand on the elastic construct. This study compared outcomes of TENS in children aged 5-11 years and adolescents aged 12-16 years with isolated closed femoral shaft fractures. MATERIALS AND METHODS: This retrospective comparative study reviewed existing hospital records and serial radiographs of 39 patients treated with TENS between January 2022 and December 2024.

Association of coexisting type 2 diabetes and DXA-defined low muscle mass with prevalent radiographic vertebral fracture in community-dwelling older adults: a cross-sectional study.

BACKGROUND: Prevalent radiographic vertebral fracture is frequently clinically unrecognized in older adults, and areal bone mineral density does not fully characterize its distribution. Whether coexisting type 2 diabetes mellitus (T2DM) and objectively measured low muscle mass identify a subgroup in which this burden is concentrated remains unclear. METHODS: This community-based cross-sectional analysis included 1,642 adults aged 65 years or older recruited through 16 community health service centers covering 36 administrative communities in Changchun, China, between July 2021 and March 2023.

CT-defined sarcopenia as a predictor of early mortality following hip fracture: a retrospective cohort study.

PURPOSE: Sarcopenia is increasingly recognised as a marker of physiological vulnerability. Computed tomography permits objective quantification of skeletal muscle mass, yet the prognostic significance of CT-defined sarcopenia after hip fracture remains incompletely characterised. This study aimed to determine sarcopenia prevalence in patients with hip fractures and evaluate associations with early postoperative outcomes. METHODS: This single-centre retrospective cohort study included patients with hip fracture during 2015 to 2025 who had abdominal computed tomography imaging, including the th
