BACKGROUND: Tibial plateau fracture is one of the common fractures in the lower limb, mostly caused by high-energy injuries, which may be accompanied by different degrees of compression and displacement of the joint surface, affecting the knee joint alignment, stability, and sports function, and improper treatment may cause various complications, which are a more difficult problem in the clinic. The objective of this study was to investigate the biomechanical mechanisms underlying effective closed reduction in the treatment of tibial plateau fractures, particularly focusing on the performance

PURPOSE: Segmental fractures, being associated with high energy trauma, are quite intimidating for the treating surgeon due to wide incidence of complications. The soft tissue damage and the fracture pattern in segmental fractures further disrupt the precarious nutrition of the floating intercalary segment, affecting fracture healing. Our study analyses the parameters influencing time to union in segmental fractures of femur.
METHODS: A retrospective analysis of all patients who presented to our level 1 trauma centre between January 2015 and December 2020 with traumatic segmental fractures of

INTRODUCTION: Intertrochanteric femoral fractures (IFF) are among the most common lower limb fractures in the older population. Our team has developed a new cephalomedullary nail design called the proximal femoral universal nail (PFUN). This study aimed to compare the clinical outcomes of PFUN and proximal femoral nail anti-rotation (PFNA) internal fixation for the treatment of IFF in older patients.
METHODS: We retrospectively reviewed the clinical records of 435 older patients with IFF who were treated at Peking University Third Hospital between January 2022 and January 2024. Among them, 21

BACKGROUND: Transcutaneous osseointegration following amputation (TOFA) is an alternative to standard prosthesis by directly anchoring a metal implant to the skeletal residuum. However, there is a paucity of data concerning whether TOFA is safe and reasonable for patients with lower limb amputation to manage complications of sepsis. The primary aim of this study was to record adverse post-TOFA events for nine patients (15 limbs) whose index amputations were performed to manage sepsis-related complications. The secondary aim was to compare the pre- and post-TOFA mobility and quality of life (Qo

BACKGROUND: Periodontitis is a chronic inflammatory condition that affects the oral cavity and has systemic implications beyond its localized impact, potentially increasing the risk of various health issues. This study aimed to investigate the role of periodontitis as a risk factor for fracture-related infection (FRI), a severe complication encountered in orthopaedic trauma.
METHODS: A prospective cohort study was conducted to analyze data from 235 patients with severe open fractures (grades II, IIIA, or IIIB) who were monitored for at least 12 months. During the perioperative period, the per

Background: Hip fractures in geriatric patients represent a major public health burden, with a clinically important subset presenting with concomitant upper extremity (UE) fractures. The independent impact of these dual injuries on clinical outcomes remains incompletely characterized. Methods: A retrospective study of patients aged ≥65 years who underwent surgical treatment for hip fracture at tertiary medical center, between January 2010 and January 2024. Patients were stratified based on the presence of a UE fracture sustained at the same time as the hip fracture. Multivariable regression mo

INTRODUCTION: Lower extremity fractures are common orthopaedic injuries and often require surgical intervention, which has been shown to significantly impact both the physical recovery and psychological well-being of patients. Given the high prevalence of psychiatric disorders following lower extremity injuries, understanding the relationship between postoperative weight bearing restrictions and psychiatric outcomes is important for improving patient recovery. It was hypothesized that patients with lower extremity fractures who were allowed early weight bearing postoperatively would have lower

UNLABELLED: Treatment of both bone forearm fractures (BBFFs) in skeletally immature patients includes both operative and nonoperative strategies. However, there is no literature-supported consensus as to which intervention provides optimal patient outcomes. This systematic review and meta-analysis compares the outcomes of surgical and conservative treatment in the management of BBFFs in skeletally immature patients. A systematic review following Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines was performed across Medline, Scopus, PubMed, and CENTRAL from

For the osteosynthetic treatment of fractures of the femur and optimal exposure of the fractured region including radiological fluoroscopy, it is often recommended to carry out positioning on an extension table, with positioning of the contralateral lower limb on a leg holder. It is not uncommon for this to result in peri- and postoperative damage of varying severity as a result of patient positioning. Some cases of damage due to improper positioning can be found in the literature in the context of urological, general and gynaecological surgery. This is often associated with the responsible su

Background Patients with neck of femur (NOF) fractures experience high morbidity and transfusion rates. Tranexamic acid (TXA) routinely reduces perioperative blood loss in elective lower limb arthroplasties; however, its use in fragility fracture management remains variable, and there is no nationally agreed guideline for its use in hip fracture surgery. This study aims to evaluate the impact of TXA administration on transfusion rates, postoperative hemoglobin drop, in-hospital complications, and 30-day mortality in patients with NOF fractures at a UK-based teaching hospital. Methods A retrosp

BACKGROUND: Pelvic fractures are caused by high-energy trauma, consisting of traffic accidents, falls from heights. They are associated with a high percentage of complications, as well as the need for rapid surgical repair. The least invasive anastomosis for this type of injury is the TiFi method, which shows minimal postoperative complications and the possibility of early rehabilitation and weight-bearing, mobilising patients after surgery.
METHODS: Review of thematically available articles from the last 20 years on TiFi pelvic fracture anastomosis after high-energy trauma and ongoing rehabi

We present a 46-year-old patient who developed acute psychotic and neurological symptoms associated with the use of intravenous ertapenem, a carbapenem antibiotic. He was hospitalized following a road traffic accident where he sustained an open fracture over the right lower limb requiring intravenous antibiotics. He started to exhibit hallucinations, odd beliefs and twitching movements two weeks after the initiation of ertapenem. Other organic causes for this acute change in behaviour were ruled out. His symptoms continued despite the use of antipsychotics but quickly resolved within three day

BACKGROUND: Older adults represent a growing proportion of motorcyclists and experience poor outcomes after crashes, but the role of frailty has not been characterized. We hypothesized that certain injuries and adverse outcomes would be more common with frailty.
METHODS: The Trauma Quality Improvement Program database was retrospectively reviewed for patients aged ≥50 years with motorcycle collisions from 2017 to 2021. Injury patterns were compared by frailty status and multivariable regression assessed the effect of frailty on mortality and adverse discharge.
RESULTS: Among 58,149 patients,

Vascular injury leading to pseudoaneurysm formation is a rare but potentially limb-threatening complication of lower limb trauma and postoperative recovery. We report a case of a 58-year-old man who developed progressive swelling of the right thigh one week after sustaining a road traffic accident and undergoing intramedullary nailing for a distal femoral shaft fracture. The swelling, initially presumed to represent a postoperative hematoma, progressed to cause severe tension and pain with early signs of threatened compartment syndrome. Upon transfer to our center, urgent surgical exploration

BACKGROUND: Postoperative pain following lower limb fracture surgery is frequently severe, yet the correlation between this acute pain trajectory and patient satisfaction with care remains poorly understood. This study investigated whether effective early pain relief correlates directly with improved patient satisfaction within the first 14 postoperative days.
METHODOLOGY: A prospective, longitudinal observational study was conducted on 58 adult patients undergoing surgical fixation for lower limb fractures. Pain levels (visual analog scale, VAS) and patient satisfaction (five-point Likert sc

Hip fractures in older adults present considerable challenges in health outcomes and management. Deciding between surgical and nonsurgical treatment is especially complex in patients with serious comorbidities, those receiving palliative care, or individuals nearing the end of life. This review compares operative versus nonoperative treatment of hip fractures, discussing the implications related to mortality, quality of life (QOL), and harm versus burden. While surgical treatment is often considered the standard of care, it carries significant risks in frail older adults, such as postoperative

Use of autologous and prosthetic grafts is one of the key concepts in open vascular reconstruction surgery. Despite taking different measures, there is still a chance of vascular graft infections due to various patient and surgery-related factors. Some of the vascular grafts might lose the overlying soft tissue cover, leading to graft exposure. Both these complications pose a significant threat to the graft viability. A graft with threatened viability poses a risk to the supplying end organ, limb, or life. Therefore, vascular graft exposure and graft infections should be promptly addressed and
