Microsurgical reconstruction of the lower extremity presents a difficult problem to plastic surgeons; the rate of failure is higher than any other anatomical site. We reviewed our recent experience with lower extremity microsurgical reconstruction using the 3M vascular coupling device. We believe the excellent patency rate of the coupler may minimize the well-described problem of venous thrombosis in this challenging group of patients. The purpose of this study is to assess the long-term success rate and functional results of limb-sparing therapy in a group of 156 patients with soft tissue sarcomas of the extremities in the Netherlands Cancer Institute, treated according to a standard protocol of surgery and radiotherapy, if indicated. In the years 1985 to 1989 75 children and adolescents presented to an urban children's hospital for treatment of 76 incidents of extremity gunshot wounds. Although the population ranged widely, the "typical" patient was a preteen or teenager (n = 70) who was shot in the lower extremity (n = 53) with a low velocity handgun (n = 74). Forty-nine skeletally mature patients who had either a non-union or a malunion of a fracture of the tibia or the femur had correction of the deformity and acute axial lengthening of the fractured bone. Reports of accessory soleus muscle are rare, perhaps as a result of it often remaining unrecognized. One hundred and forty-two consecutive patients with proximal femoral fractures were audited prospectively over a 1-year period. Mobility, age and sex were recorded along with timing of surgery, complications, 'will to live', length of admission, mortality, mobility and housing requirements on discharge. Osteoporosis was assessed at the hip (Singh's index) and calcaneus (calcaneal index) using conventional radiography in 80 patients (31 men, 49 women) with fracture neck of femur. Forty four patients had intracapsular fracture neck of femur (ICFN), and 36 extracapsular fracture neck of femur (ECFN). Forty acute blunt fractures of the humeral diaphysis were treated with Seidel nails (9-mm diameter) between April 1988 and August 1992. Follow up was available for 36 patients. Eleven patients (with 12 supracondylar femur fractures) with knee extension contractures were followed for an average of 24.8 months (range 12-45) after Judet quadricepsplasty. An increase in motion of 53 degrees with active postoperative flexion of 91 degrees was noted. There was an average loss of 14 degrees of flexion from maximum intraoperative correction. One patient had an extensor lag of 30 degrees, which did not improve after quadricepsplasty, but no other patients developed an extensor lag, infection, or skin breakdown after the procedure.

Based on all patients with tibial shaft fracture (n = 767) treated in the department of orthopedics in Malmö from 1955 to 1963, a retrospective cohort study was performed in 1990. Two hundred sixty-nine of the patients were still living in Malmö or had died in the city.
