Many complications can occur following insertion of silicone elastomer implants into the foot. Postoperative infection may be difficult to distinguish from other conditions such as dislodgment, fracture, ectopic and heterotopic new bone formation, synovitis, and bursitis. White blood cell scintigraphy, in conjunction with the clinical scenario, may prove to be an invaluable tool in the diagnosis of postoperative osteomyelitis, subsequent to implant arthroplasties.

The choice between regional versus general anaesthesia for elderly patients undergoing hip surgery is debated. Complete dislocation of the tarsal cuboid is presented along with regional anatomic considerations. Thorough literature search revealed only three other reported cases of dislocation of the tarsal cuboid. The operative procedure described encompasses an interesting method of reduction of this dislocation. It should be emphasized that A-P, lateral, and oblique X-rays should be obtained for injuries of the mid and forefoot. The rarity of such a dislocation is attributed to the structural integrity of the supporting ligamentous elements.

Chronic joint pain in rheumatoid arthritis may mask that from other causes. Stress fractures, particularly in osteoporotic lower limb bones of such patients, are well described but can still cause diagnostic difficulty. Soft-tissue injuries and defects associated with severe fractures of the tibia treated with the Hoffmann external fixator were reviewed. Of 84 cases of open tibia fractures (classified as Gustilo's Type II and III), 54 patients with 55 injured limbs required soft-tissue reconstruction other than skin grafts or small rotational flaps. The incidence of pain and other complications associated with the untreated spastic dislocated or subluxed hip was investigated in 80 institutionalized patients.
