Four cases of rheumatoid arthritis with a total of 12 undisplaced stress fractures of the lower extremities are described. Pain, swelling and disability arising from these fractures were initially mistaken for rheumatoid synovitis. Three patients had osteoporosis, and one had osteomalacia. A high index of suspicion is necessary in the diagnosis of these fractures. Patients undergoing operations on the hip for either replacement or fracture were chosen for this study. Fifty-two subjects received cyclic sequential compression as well as elevation of the lower limb while another fifty-two patients were treated with only elevation of the lower limb. The vascular-pedicle "wrap-around" free flap was used in five patients for reconstruction in amputations distal to the metacarpophalangeal joint. In five other patients who had normal thumbs but had lost all fingers, the technique was expanded to include the second toe with great toe wrap-around flap. All patients did well, but significant bone peg resorption occurred in six patients and in three others the bone peg fractured.

A false aneurysm of the posterior tibial artery associated with a fracture of the tibia and fibula is described. A review of the English language literature of the last 15 years revealed only six other similar cases. A case of avulsion fracture of the lateral sesamoid is reported in association with dislocation of themetatarsophalangeal joint of the hallux. The radiographic criteria to establish the diagnosis of fracture in sesamoid bones are outlined. Review of the literature reveals only one other reported case associated with a hallux dislocation, and in that case, the radiographic diagnosis was not clearly established.

One in six patients receiving renal transplants has orthopaedic problems. Osteoporosis with accompanying fractures is common, as is avascular necrosis of the hips, knees, and other bones. Immunosuppressive therapy with steroids is implicated and its mode of action discussed. No means of prophylasix is known. If treated conservatively, aseptic necrosis of bone is much more disabling than is commonly believed as judged by the critical assessment technique of Charnley. The authors therefore recommend that a surgical approach should be adopted whenever possible.

The frequency and nature of exertion pains of the leg in athletes were studied in 2,750 cases of overuse injuries treated at the Sports Clinic of the Deaconess Institute of Oulu, Finland, during the years 1972-1977. 465 cases of exertion pain (18%) were located in the shin. Traumatic bowing of the forearm or lower leg is reported in 31 children. It is a relatively rare condition. Bowing occurs most frequently in combination with fracture of the other bone in the same extremity. In a minority of cases a bowing deformity is a single lesion. Age distribution, degree of deformity, mechanism of origin and therapy are presented and discussed.
