Postoperative sural nerve entrapment following lateral heel surgery: a case report.

BACKGROUND: Sural nerve injury is an uncommon complication following foot and ankle surgery, including procedures performed for Haglund deformity. Postoperative sural nerve entrapment remains rarely reported and may be challenging to diagnose. CASE PRESENTATION: A 56-year-old woman presented with persistent lateral ankle pain and paraesthesia 12 months after open lateral heel surgery for Haglund deformity. Clinical examination revealed a trigger point at the distal portion of the surgical scar associated with a positive Tinel's sign. A diagnostic local anaesthetic injection provided temporary

Popliteal artery injury associated with multiligament knee injuries and tibial plateau fractures: A 20-year retrospective cohort study.

PURPOSE: Popliteal artery injury (PAI) around the knee is a limb-threatening condition typically associated with high-energy trauma, particularly multiligament knee injuries (MLKIs) and tibial plateau fractures. This study aimed to analyse injury patterns, management, complications and functional outcomes, with the hypothesis that fracture-related PAI would be associated with better functional outcomes than MLKI-associated PAI. METHODS: A retrospective review was conducted of patients treated for PAI at a tertiary referral centre from 2003 to 2023. Patients were eligible if computed tomograph

Study of the Spread of Scarring along the Damaged Area of the Limb Nerve after Gunshot and Explosive Injuries.

BACKGROUND: Determining the true size or extent of the nerve defect after damage and excision of the posttraumatic neuroma is fundamental in the microsurgical treatment of the patient. It is assumed that the length of the scar in the nerve may depend on the nature of the combat wound of the limb, gunshot, or mine-explosive. Determining the extent of scarring in the nerve in cases of these two types of injury may provide an opportunity to establish a relationship with the type of injury or to disprove such a relationship. This should help in the surgical treatment tactics. METHODS: Fifty sampl

Blood-activating and collateral-unblocking traditional Chinese medicine formulas combined with mecobalamin for peripheral neuropathy in type 2 diabetes: a systematic review and Bayesian network meta-analysis.

BACKGROUND: Diabetic peripheral neuropathy (DPN) is a leading cause of neuropathic pain, disability and lower-limb amputation, and the pharmacological options currently available do little to reverse established nerve injury. Compound traditional Chinese medicine preparations that activate blood circulation and unblock the collaterals have shown promise as add-on therapy to neurotrophic treatment, improving nerve conduction and relieving symptoms, but head-to-head comparisons between individual formulas are lacking. METHODS: PubMed, Embase, the Cochrane Library, CNKI, Wanfang and other databa

Systematic Review and Meta-Analysis Comparing the Efficacy of CHIVA and Conventional Stripping and Ablation Techniques.

BACKGROUND: Chronic venous disease, particularly primary lower extremity varicose veins, is a prevalent global health issue. The Conservatrice et Hémodynamique de l'Insuffisance Veineuse en Ambulatoire (CHIVA) technique is a hemodynamic-based, vein-preserving strategy proposed as a minimally invasive alternative to traditional treatments like high ligation and stripping or endovenous thermal ablation (e.g., radiofrequency or laser). The CHIVA technique is designed to treat varicose veins while preserving the great saphenous vein, which is the first-choice conduit for potential future arterial

Peripheral nerve schwannomas: Anatomical distribution, biopsy risks and surgical outcomes in a contemporary large-volume case series.

BACKGROUND: Schwannomas are benign peripheral nerve sheath tumours that may cause pain, sensory disturbance and functional deficits. Despite their typically indolent nature, management decisions carry the risk of neurological morbidity. This study presents the largest single-centre series of schwannomas reported to date, evaluating clinical presentation, imaging, management and outcomes. METHODS: A retrospective review was conducted of solitary schwannomas treated at a tertiary peripheral nerve injury unit between 2017 and 2020. Data collected included anatomical location, imaging features, p

Safety and Efficacy of Tunneled Sciatic Nerve Catheters in Combat Injuries: Ukrainian Experience.

BACKGROUND: Surgical treatment of combat injuries often requires continuous analgesia for many weeks, which can be achieved with continuous peripheral nerve blocks. Existing data on the safety and efficacy of sciatic nerve catheterization in civilian and military populations are difficult to extrapolate to the population of combat-injured patients in Ukraine. PURPOSE: To add to the body of evidence on the safety and efficacy of tunneled sciatic nerve catheters for pain control in combat-injured patients treated at Role 3 facilities. PATIENTS AND METHODS: This retrospective study included pat

A Narrative Review of Perioperative Peripheral Nerve Injuries After Major Surgery: Clinical Recognition, Electrodiagnostic Evaluation, and Rehabilitation Implications.

Perioperative peripheral nerve injuries (PPNIs) are complications of surgical and procedural care and may cause neuropathic pain and motor or sensory deficits that delay functional recovery. This narrative review summarizes common patterns of PPNI involving the extremities, practical bedside recognition, the role and timing of electrodiagnostic testing, and key rehabilitation considerations. Upper-limb involvement most often affects the brachial plexus, ulnar nerve, and median nerve. In contrast, lower-limb presentations commonly involve the lateral femoral cutaneous, sciatic, and common fibul

Preoperative risk factors associated with patient outcomes following arthroscopic elbow contracture release.

BACKGROUND: Elbow contractures can occur due to several causes including age-related degenerative disease, inflammatory conditions, and post-traumatic injury. The resulting pain and reduced range of motion can significantly interfere with activities of daily living and impede patient functionality. One treatment option for elbow contractures is arthroscopic contracture release. The purpose of this study is to evaluate the preoperative risk factors associated with inferior clinical outcomes following arthroscopic elbow contracture release. METHODS: This retrospective cohort study included pati

Influence of Local Anesthetic Techniques on Patient-Reported Lateral Numbness Following Total Knee Arthroplasty.

Total knee arthroplasty (TKA) results in significant postoperative pain, requiring effective analgesia to enable early mobilization and reduce complications. While local infiltration analgesia (LIA) is effective, it may cause prolonged lower limb numbness if sensory nerves are affected. This study evaluated the incidence of patient-reported inferolateral knee numbness at 2 and 6 weeks and 3 months post-TKA using different LIA techniques. A retrospective cohort study of 1,142 patients undergoing TKA (2021-2024) compared two LIA techniques. Group 1 (n = 471) received a conventional multisite per

Post-Injection Sciatic Neuropathy: MR Neurography Findings in a Case of Iatrogenic Nerve Injury.

Post-injection sciatic neuropathy is a recognized iatrogenic complication of intragluteal injection. We report a case of a young adult presenting with progressive complete paralysis of the left lower limb following intragluteal injection. Magnetic resonance imaging (MRI) demonstrated focal thickening and T2/short-tau inversion recovery (STIR) hyperintensity of the sciatic nerve in the gluteal region, without neuroma or discontinuity and without associated muscle denervation changes. These findings were consistent with post-injection sciatic neuropathy, likely representing axonotmesis. MR neuro

Retroperitoneal Hematoma Following Lumbar Injection in a Patient Receiving Clopidogrel: A Case Report.

Lumbar spine injections are commonly used to manage spinal and radicular pain and have included techniques with varying anatomic depth and bleeding risk. Although hemorrhagic complications are rare, lumbar injections in patients receiving antiplatelet therapy-particularly when multiple deep lumbar procedures are performed-may result in clinically significant bleeding and neurologic injury. We report a case of a 56-year-old man receiving clopidogrel who underwent a lumbar medial branch block and a psoas compartment-related block during the same session at an outside hospital and subsequently de

Distribution of Infralesional Lower Motor Neuron Injury in Traumatic Cervical Spinal Cord Injury Candidates for Upper Limb Reconstruction: A Single Center Experience.

INTRODUCTION/AIMS: Patients with cervical spinal cord injury (SCI) prioritize recovery of upper extremity function. Upper extremity reconstruction can restore function in carefully selected individuals. Infralesional cord damage, observed below the neurological level of injury (NLI), may affect postoperative outcomes. We characterized the extent of infralesional lower motor neuron (LMN) injury in patients with traumatic cervical SCI. METHODS: Each patient underwent electromyography (EMG) of potential donor and recipient muscles based on preoperative clinical evaluation and specific candidacy

Holding on to bypass in endovascular times? A narrative review on the role of surgical LSA revascularization in zone 2 TEVAR and how to achieve optimal results.

Thoracic endovascular aortic repair (TEVAR) has become the preferred treatment for most descending thoracic aortic pathologies. In Zone 2 procedures, intentional coverage of the left subclavian artery (LSA) is frequently required to achieve an adequate proximal seal, but this carries a risk of stroke, spinal cord ischemia, and upper extremity ischemia. Preventive LSA revascularization is therefore recommended in current guidelines, although the level of supporting evidence remains limited. This narrative review summarizes the indications, timing strategies, techniques, and outcomes of surgical
