[Comparative efficacy of island latissimus dorsi myocutaneous flap versus lower trapezius myocutaneous flap for repair of posterior neck skin and soft tissue defects].

OBJECTIVES: Posterior neck skin and soft tissue defects often result from electrical burns, post-tumor resection, infections, or trauma. The region has complex anatomy, high mobility, and proximity to the cervical spine, major vessels, and critical nerves; thus, reconstruction must balance functional restoration and aesthetic outcomes. The island latissimus dorsi myocutaneous flap, with its reliable blood supply, large harvestable area, and long rotation arc, is widely used for trunk and limb reconstruction and shows particular advantage for large defects in the thoracic, dorsal, and posterior

Evaluation of the effects of extremity elongation on brachial plexus nerves via intraoperative neuromonitoring in patients undergoing reverse total shoulder arthroplasty.

BACKGROUND: Reverse total shoulder arthroplasty (RTSA) is one of the most common treatment method for irreparable rotator cuff tear. One of the significant complications of this surgery is nerve injury. This study aimed to identify the patterns of brachial plexus nerve involvement and to determine the surgical stages associated with increased nerve vulnerability during reverse total shoulder arthroplasty using intraoperative neuromonitoring (IONM). Additionally, the relationship between extremity lengthening, acromiohumeral distance (AHD) changes and intraoperative nerve events were evaluated.

Robotics in Pediatric Surgery: Expanding Indications and Techniques.

Pediatric lower extremity reconstruction, especially following trauma or oncologic resection, presents unique challenges requiring a multidisciplinary approach. Management emphasizes early, thorough debridement, appropriate fixation-often with flexible intramedullary nails-and timely, well-vascularized soft tissue coverage. Nerve injuries are addressed with autografts or allografts, with children demonstrating better regenerative capacity than adults. Postoperative protocols focus on immobilization, pain management, psychosocial support, and structured rehabilitation, with family involvement c

Effect of preoperative liposomal bupivacaine single-injection pericapsular nerve group (PENG) block on lower extremity deep vein thrombosis in elderly patients with hip fractures: a randomised controlled, double-blind, prospective clinical study protocol.

INTRODUCTION: Deep vein thrombosis (DVT) of the lower limbs has a significantly higher incidence among elderly populations than that observed in other types of fractures, prolonged immobilisation and the systemic inflammatory response triggered by preoperative pain are the main risk factors. Liposomal bupivacaine (LB) single-injection pericapsular nerve group (PENG) block has demonstrated effective analgesia both before and after surgery, while preserving motor function in patients with hip fracture. Although regional nerve block is a well-established component of preoperative multimodal analg

Arthroscopic-assisted uni-portal spine surgery via modified interlaminar approach combined with annular suturing for L5/S1 disc herniation: a case report and technical note.

BACKGROUND: Lumbar disc herniation (LDH) is a common clinical spinal disorder, with the L5/S1 segment being a frequently affected site due to its unique anatomical and biomechanical characteristics. Conventional minimally invasive spinal endoscopic techniques, such as percutaneous transforaminal endoscopic discectomy (PTED), percutaneous endoscopic interlaminar discectomy (PEID), and unilateral biportal endoscopy (UBE), have inherent limitations in treating L5/S1 LDH. These include difficulty bypassing the high iliac crest (for PTED), a steep learning curve (for PEID), and potential impairment

The effect of J-shaped incision versus standard midline incision on numbness and pain in primary total knee arthroplasty: a prospective randomized controlled trial.

INTRODUCTION: Peri-incisional numbness is a common but often underestimated complaint after primary total knee arthroplasty (TKA) and may negatively affect early postoperative patient satisfaction. This randomized controlled trial aimed to determine whether a modified J-shaped skin incision, created by lateralizing the distal limb of the standard midline incision by 3 cm at the tibial tuberosity, reduces peri-incisional numbness and pain after primary TKA. MATERIALS AND METHODS: Sixty-two patients undergoing primary TKA for Kellgren-Lawrence grade 4 osteoarthritis were randomized to either a

Staged limb salvage of a Gustilo-Anderson type IIIC open distal tibial gunshot fracture using cross-leg pedicle flap and delayed intramedullary nailing with bone grafting: a case report.

INTRODUCTION AND IMPORTANCE: Gunshot injuries to the lower limb can cause severe damage to both bone and soft tissues, particularly when the injury occurs at close range. Open fractures of the tibia are especially difficult to manage because of the limited soft-tissue coverage and the high risk of infection, non-union, and other complications. Gustilo-Anderson type IIIC fractures represent the most severe form, as they involve arterial injury that requires urgent repair. Successful limb salvage depends on early debridement, restoration of blood flow, stable fixation, and reliable soft-tissue c

Predicted effects of hip distraction on knee ligament mechanics following total knee arthroplasty: A finite element analysis.

BACKGROUND: Hip distraction is essential for hip arthroscopy and other orthopaedic surgeries, but high traction forces often lead to complications like nerve injury and soft tissue impairment. Patients with total knee arthroplasties (TKAs) may face increased risk due to altered joint mechanics; however, the impact of specific TKA configurations on knee behavior during distraction is poorly understood. METHODS: A validated finite element model of the pelvis and lower extremity was developed from patient CT and MRI data. Three configurations were modeled: bi-cruciate retaining (BCR), posterior-

Analysis of Complications Associated with Basivertebral Nerve Ablation for Vertebrogenic Low Back Pain: A Multicenter Retrospective Cohort Study of 1,145 Patients.

OBJECTIVE: Low back pain (LBP) is a leading cause of pain and disability worldwide. Recent studies demonstrate a vertebrogenic model involving the basivertebral nerve (BVN) as a cause of chronic LBP. Vertebral endplates are highly innervated by the BVN, and literature shows type 1 and 2 Modic changes at vertebral endplates are a source of vertebrogenic low back pain (VBP). As a result, intraosseous radiofrequency ablation of the BVN is a minimally invasive outpatient procedure used to treat VBP. We conducted a retrospective review of 1,145 patients that have undergone BVN ablation to assess fo

A Rigid Spine at Risk: Chalk-Stick Fracture With Dural Sac Entrapment After Ground-Level Fall.

Ankylosing spondylitis predisposes patients to unstable spinal fractures following low-energy trauma because of progressive spinal ankylosis and loss of flexibility. These fractures may be difficult to recognize clinically and can be associated with delayed neurologic complications. We present the case of a 65-year-old male patient with ankylosing spondylitis and severe kyphosis who developed progressive left lower extremity pain, weakness, and numbness five days after a ground-level fall. Computed tomography (CT) of the lumbar spine revealed marked widening of the L1-L2 intervertebral disc sp

Ollier's Disease Regenerate Healing Rates: Is there Cause for Concern?

BACKGROUND: Ollier's disease, characterised by multiple enchondromas, can result in significant limb length discrepancy and angular deformities. Anecdotal evidence suggests faster healing rates and a risk of premature consolidation during lengthening. The aim is to evaluate healing rates in Ollier's and assess the need for change in practice. MATERIALS AND METHODS: A total of 12 patients with Ollier's disease underwent limb lengthening utilising an external fixator or intramedullary lengthening nail. A latency period of 6 days was followed by a lengthening rate of 1 mm per day. Parameters, in
