[Recommendations and Protocols for the Safe Use of Tourniquets in Upper- and Lower-Extremity Surgeries].

The tourniquet (TNQ) interrupts blood flow to a given anatomical segment and has critical applications in orthopedic surgeries by providing a blood-free operating field. The risks and complications attributed to its use are increased pain, reperfusion injury, edema, deep venous thrombosis, and peripheral nerve injury. The main recommendations for TNQ use and to reduce the occurrence of complications include adequate limb padding; TNQ inflation to pressures of 50 mmHg and 100 mmHg above the perfusion pressure for the upper and lower limbs respectively; avoid TNQ use in children and patients wit

Array pin placement in robotic-assisted total hip arthroplasty: Optimal trajectory to avoid neurovascular injury.

BACKGROUND: Array pin placement into the pelvis is a necessary step for robotic-assisted total hip arthroplasty (RATHA). Despite the increase in RATHA being performed, there is limited literature investigating safe zones and pin trajectory to avoid neurovascular injury. METHODS: This study utilized lower extremity magnetic resonance images of nine patients. The trajectory of three array pins placed at caudal angles of 35, 45, and 55 were recreated, and each pin was divided into four equidistant zones. For each pin, the distance was determined from each zone to nearby neurovascular structures:

Risk Factors of Positional Peripheral Nerve Injury in Robotic Laparoscopic Radical Prostatectomy.

BACKGROUND AND OBJECTIVES: One of the responsibilities of the anesthesiologist is to maintain the physiologic anatomic position during surgery. Postoperative positional peripheral nerve injury (PPPNI) inevitably may occur during robot-assisted laparoscopic radical prostatectomy (RARP) in steep-Trendelenburg-lithotomy positioning. The primary aim of the study was to identify incidence and risk factors for the development of PPPNI in the RARP and the secondary aim was to identify the most common types and duration of PPPNI. METHODS: After ethics committee and patients approval, patients who und

Tourniquet-related complications in extremity injuries: a scoping review of the literature.

BACKGROUND: Tourniquets are crucial for controlling life-threatening hemorrhage and, therefore, in preventing avoidable deaths in both military and civilian settings. Its increased use since the launch of the Stop the Bleed campaign, however, has raised concerns regarding possible complications associated with its application, including limb ischemia and amputation. The objective of this study was to synthesize the existing evidence regarding complications associated with the use of tourniquets for extremity injuries and identify gaps in knowledge to guide future research on this topic. Pericapsular Nerve Group Block for Hip Fracture Pain Management: A Narrative Review of Emergency Applications.

The pericapsular nerve group (PENG) block is a regional anaesthesia technique for acute pain management that is becoming frequently employed for emergency lower limb fractures, particularly hip injuries. While current evidence has been predominantly derived from postoperative randomized trials and case series, the results of recent clinical reports support the preprocedural utility of the PENG block in emergency settings for rapid analgesia 40-60% pain score reduction and preservation of motor function. This review synthesizes mechanistic insights and clinical outcomes from the literature, com

Preserved fascicular architecture predicts neuroma pain: a morphometric study.

Painful neuromas remain a major clinical challenge after limb amputation and peripheral nerve trauma. While histological features such as inflammation, fibrosis, and axonal sprouting have been proposed as contributors to neuropathic pain, direct clinicopathological correlations remain inconsistent. The role of internal nerve architecture, particularly the proportion of preserved, organized fascicular tissue, has not been quantitatively assessed in relation to pain intensity. To address this gap, this study investigates whether the relative amount of organized versus unorganized nervous tissue

Comparative Outcomes of Direct Versus Connector-Assisted Peripheral Nerve Repair.

Background: Peripheral nerve injuries affect a significant proportion of patients with upper extremity trauma, with transections frequently requiring surgical intervention. While direct repair (DR) remains the historical standard, connector-assisted repair (CAR) has been proposed to improve functional outcomes by addressing limitations inherent to DR, such as fascicular misalignment and tension at the repair site. Objectives: The purpose of this systematic review is to evaluate and compare the clinical effectiveness and complication rates of DR versus CAR in upper extremity peripheral nerve in
