Dermatomal Contact Heat Evoked Potentials for the Detection of Subclinical Cervical Spinal Cord Injury in Asymptomatic Degenerative Cervical Cord Compression.

BACKGROUND: Asymptomatic degenerative cervical cord compression (ADCC) represents a premyelopathic stage of degenerative cervical myelopathy (DCM), with high prevalence in older age and unclear management. Contact heat-evoked potentials (CHEPs) assess the integrity of the thermo-algesic somatosensory pathway and have shown the highest sensitivity among neurophysiological methods in detecting dysfunction in the centromedial and anterior spinal cord in DCM. METHODS: This cross-sectional cohort study evaluated the utility of upper extremity dermatomal C4, C6 and C8 CHEPs (dCHEPs) compared to sta

Assessing nonunion and morbidity in pediatric clavicula pro humero reconstruction with vascularized fibular grafts.

The proximal humerus is a common site for pediatric malignant bone tumors, often necessitating complex surgical approaches when the tumor extends into the glenohumeral joint. Limb-salvage techniques like clavicula pro humero (CPH) reconstruction aim to balance limb function with oncological safety. However, nonunion and functional limitations remain significant challenges. This study evaluates whether adding a vascularized fibular graft (VFG) reduces nonunion in CPH reconstructions following proximal humeral tumor resection. A retrospective review of six pediatric patients (mean age 12 years)

Migration of a Nelaton Catheter Into the Pulmonary Artery During Spinal Surgery Requiring Surgical Removal: A Regrettable Case.

Perioperative complications in spinal surgery include dural injury, postoperative epidural hematoma, and surgical site infections. More severe complications involve paralysis, sensory deficits due to nerve injury, and bladder or bowel dysfunction. During fusion surgery, attention must also be paid to risks such as guide wire breakage, cage migration or dislodgement, and major vascular injury during intervertebral disc gauge placement. A common empirical practice during bone drilling with a high-speed drill involves placing a 15 mm segment of a Nelaton catheter over the tip of the suction tube

Brown-Sequard syndrome caused by posterior full-endoscopic cervical discectomy: A case report.

BACKGROUND: Posterior full-endoscopic cervical discectomy (PFECD) is an effective and safe technique for cervical radiculopathy. The primary complications of PFECD include temporary nerve root paralysis and dural rupture, while spinal cord damage is exceedingly rare. This study describes a rare case of Brown-Sequard syndrome (BSS) occurring following PFECD and investigates its potential etiologies and pathomechanisms associated with this procedure. METHODS: Notes and images were reviewed and the relevant literature was analyzed. RESULTS: A 50-year-old woman underwent PFECD for cervical radic

Optimizing proximal fibula tumor resection: "The open door technique" for better surgical access and outcomes - A case series.

OBJECTIVE: Proximal fibula tumors pose significant surgical challenges due to their proximity to vital neurovascular structures, including the common peroneal nerve. Traditional resection techniques often require extensive tissue manipulation, which can lead to complications such as peroneal nerve injury and knee instability. This study introduces and evaluates the "open door technique," designed to enhance surgical visualization and access, thereby reducing complications while achieving oncologic control. METHODS: We conducted a retrospective cohort study of 31 patients who underwent proxima

Low Posterior Electromyographic Threshold and Functional Outcomes After L4-5 Lateral Lumbar Interbody Fusion.

BACKGROUND AND OBJECTIVES: The transpsoas approach for lateral lumbar interbody fusion (LLIF) places the surgical retractor near the lumbar plexus, particularly at the L4-5 level. To mitigate risk to these nerves, surgeons rely on intraoperative neuromonitoring using stimulation to approximate the distance between the plexus and retractor. The aim of this study was to review a single-institution series of lateral-position LLIF procedures to determine whether the stimulation threshold was associated with plexus injury. METHODS: Single-level LLIF procedures performed in the lateral decubitus po

MR Neurography of Lower Extremity Sports-Related Nerve Injuries.

Lower extremity peripheral nerve injuries leading to sensorimotor disturbances can significantly impede athletic performance and return to play. Electrodiagnostic studies are limited by operator dependency, limited sensitivity in the acute phase, and sometimes inability to localize a lesion, particularly if only partial (ie, fascicular) or multifocal injury is present. This article highlights the clinical value of MR neurography in diagnosing and managing sports-related peripheral nerve injuries, including iatrogenic nerve-related complications following sports surgeries.

Nonsurgical segment "spreading-type" symptomatic epidural hematoma formation after lumbar fusion surgery: A case report and literature review.

Although the incidence of epidural hematoma formation after lumbar fusion surgery is low, it is a severe complication. If not managed promptly, it may cause compression of the spinal cord or nerve roots, leading to neurological dysfunctions such as limb numbness, weakness, and abnormal bowel or bladder function. We conducted a comprehensive retrospective review of the medical records, test results, and imaging data of a patient with a rare "spreading-type" symptomatic epidural hematoma occurring in the nonsurgical segment after lumbar fusion surgery. The patient was a 66-year-old male who, due

Comparative efficacy and safety of endoscopic, open, and mini-open techniques for carpal tunnel release: A meta-analysis.

BACKGROUND: Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy of the upper extremity. In addition to traditional open carpal tunnel release (OCTR), endoscopic (ECTR) and mini-open (MOCTR) approaches have been developed as minimally invasive alternatives. However, comparative evidence regarding their clinical efficacy and safety remains inconsistent. METHODS: A systematic review and meta-analysis were performed according to PRISMA guidelines, including 44 comparative studies (38 comparing ECTR vs OCTR and 6 comparing ECTR vs MOCTR) involving adult patients with idiopathic C

Patient-reported risk acceptance for complications of carpal tunnel release.

BACKGROUND: Carpal tunnel release (CTR) is a common surgery with a well-described complication profile, but it is unclear whether patients' self-reported risk acceptance is higher than, equal to, or lower than actual surgical risks. We aimed to investigate patients' self-reported levels of risk acceptance for complications of CTR, factors associated with higher risk acceptance, and whether PROMIS Upper Extremity, PROMIS Pain Interference, and PROMIS Pain Intensity scores correlate with risk acceptance. METHODS: A prospective, cross-sectional study was conducted of 47 adult patients with idiop
