Disseminated Histoplasmosis Presenting as Mucocutaneous Lesions in an Immunocompetent Patient.

A case of an immunocompetent male patient in his 50s, an oil field worker from the northeastern part of India who presented with fever, weight loss over 4 months, and multiple umbilicated papulonodular lesions on his face and nodular lesions over the hard palate, is reported. The initial suspicion was talaromycosis, which is endemic in that part of the country. Periodic acid-Schiff and Grocott-Gomori's methenamine silver staining of the skin biopsy specimen from the forehead revealed numerous yeast-like bodies morphologically resembling Histoplasma within the histiocytes. A culture of the skin

Nasopharyngeal Leiomyosarcoma: Diagnostic Challenges and Management of a Rare Head and Neck Sarcoma.

Leiomyosarcoma (LMS) is a rare, smooth-muscle malignancy and an uncommon sinonasal tumor, often diagnosed at a late stage due to non-specific symptoms and complex anatomy. We report an 82-year-old man with a two-month history of facial pain, epistaxis, and right-sided hearing loss. Imaging and endoscopic evaluation showed a friable mass involving the posterior nasal cavity and nasopharynx. Biopsy confirmed a giant cell-rich spindle cell LMS. He underwent endoscopic resection with positive margins. Postoperative complications and comorbidities delayed the initiation of adjuvant radiation therap

A Rare Case of Nodular Fasciitis of the Hard Palate with Complete Clinical Resolution.

Nodular fasciitis is a rare, benign, and self-limiting proliferation of fibroblasts and myofibroblasts. It is occasionally encountered in the head and neck region, but only a few cases were reported in the palate (1-3). Case: We report the case of a 24-year-old female patient presenting with an exophytically growing, pedunculated palatal mass. According to the patient, the lesion had developed overnight and measured 12 mm in diameter at clinical presentation two weeks later. An initial incisional biopsy was interpreted as a pyogenic granuloma. The lesion then enlarged to 30 × 20 mm within thre

Mucoepidermoid Carcinoma of the Hard Palate: A Case Illustrating Grading Uncertainty and Bone-Sparing Surgical Management.

Mucoepidermoid carcinoma (MEC) of the hard palate presents diagnostic and surgical challenges, particularly regarding histological grading and preservation of the underlying palatal bone. Histological grading remains controversial because of multiple grading systems and inter-observer variability. A 42-year-old man presented with a painless 10-12 mm right paramedian hard-palate swelling that had been present for approximately 12 months, with slight enlargement over the preceding two months, identified incidentally during routine dental examination. Biopsy confirmed MEC. MRI and CT demonstrated

Maxillary oral mucosa exhibits region-specific but No direction-dependent mechanical tensile properties.

This study investigated the regional and direction-specific tensile properties of native human oral mucosal tissues. Tissue specimens were harvested from three distinct maxillary regions, namely the hard palate, attached gingiva, and alveolar mucosa from 11 partially dentate donors, totaling 34 specimens. Using the osmotic stress protocol to standardize water content, specimens underwent uniaxial tensile testing. Mechanical parameters, including elastic modulus, Cauchy stress and stretch at failure, were calculated and statistically analyzed. The attached gingiva yielded a higher elastic modul

Radiological and Functional Outcomes of Joshi External Stabilization System (JESS) in Proximal Tibia Fractures: A Case Series.

INTRODUCTION: External fixators have a limited role in the management of tibial plateau fractures (TPF). They are typically used as an initial management strategy. The Joshi External Stabilisation System (JESS) is a Kirschner wire-based fixator and has been used in TPF where plating may not be feasible due to poor skin conditions or delayed due to other soft tissue complications. This retrospective case series aimed to evaluate the radiological and functional outcomes of TPF managed using the JESS fixator. MATERIALS AND METHODS: Records of patients operated on with the JESS fixator for TPF be

50 years of estimating exposures for different jobs: the past, present and future of developing and using job exposure matrices (JEM).

INTRODUCTION: During treatment for malignant lymphoma, cytopenia can develop for several reasons. This can range from mild cytopenias leading to infection and bleeding to full-blown drug-induced aplastic anaemia. While aplastic anaemia affects individuals of all genders and ages, here, we describe aplastic anaemia after chemotherapy exposure to bendamustine in a 65-year-old female with non-Hodgkin's lymphoma. Liver fibrosis is a disease that causes high morbidity and has become a major health problem. Liver fibrosis can lead to the end stage of liver diseases (livercirrhosisand hepatocellularcarcinoma). Currently, liver transplantation is the only effective treatment for end-stage liver disease. However, the shortage of organ donors, high cost of medical surgery, immunological rejection and transplantation complications severely hamper liver transplantation therapy. Mesenchymal stem cells (MSCs) have been regarded as promising cells for clinical applications in stem cell therapy in the treatment of

The Role of Proinflammatory Biomarkers in Oral Cancer Pathophysiology.

Oral cancer (OC) is one of the most commonly diagnosed head and neck cancers and includes malignancies arising from the tongue, lips, floor of the mouth, buccal mucosa, hard palate, alveolar ridge, gingiva, and oropharynx. The most common histological type is squamous cell carcinoma, which originates from squamous cells lining the mouth and throat and accounts for over 90% of all OC cases. Notably, OC, with a five-year survival rate of approximately 68% represents a significant health burden worldwide. Biomarkers are used as molecular signatures, enabling the objective differentiation between

Successful Management of a Foreign Body Impacted in the Hard Palate: A Case Report From a Rural Setting.

Foreign body ingestion and impaction in children pose significant challenges, particularly in atypical locations like the hard palate. This case report details the successful management of a doll's eye embedded in the hard palate of an 18-month-old female child in a rural setting, highlighting the importance of clinical acumen, resourcefulness, and prompt intervention. Despite limited specialized care access, the foreign body was safely extracted under intravenous anesthesia using basic surgical facilities. This case underscores the risks associated with foreign body impaction in young childre

Buccal Sulcus Myomucosal Flap-an Approach for the Closure of Anterior Palatal Fistulae: A Retrospective Cohort Analysis of 132 Patients.

Anterior palatal fistulae (APFs) present a considerable challenge following cleft palate repair, particularly when involving the alveolus and the anterior half of the hard palate. These defects are notoriously difficult to close due to limited local tissue availability, scarring, restricted flap mobility, and altered vascularity, all contributing to high recurrence rates. This study evaluates the effectiveness of the buccal sulcus myomucosal flap (BSMF) as a practical and reliable option for APF closure. A retrospective review of 132 patients treated between January 2021 and December 2023 was

Foreign Body Impaction in the Pediatric Palate: A Case Report of a Frequently Misdiagnosed Condition.

This case report aims to increase awareness about foreign body (FB) impaction in the pediatric palate. The significance of this clinical entity lies in the fact that it is frequently misdiagnosed as a palatal fistula or mass. Patients are therefore subjected to a variety of unnecessary investigations, for example, computed tomography scans and x-rays. In this case report, a 13-month-old boy was referred for a newly developed oronasal fistula. Clinical evaluation, however, correctly identified a FB impacted in the palatal vault. The FB (button cap) was successfully removed under general anesthe

Distinct coexisting pathologies of oral malignant peripheral nerve sheath tumor with melanin pigmentation and indolent small lymphocytic lymphoma.

Spindle cell malignancies of the oral cavity are uncommon and pose considerable diagnostic challenges due to overlapping histomorphology and immunohistochemical features. Malignant peripheral nerve sheath tumor (MPNST) with heterologous differentiation is particularly rare. Moreover, its coexistence with a synchronous hematological malignancy such as small cell lymphoma (SLL) is exceedingly unusual. A male 62 year, presented with a progressively enlarging, pigmented, ulcero-proliferative painless mass over the right upper alveolus extending to the hard palate. Imaging revealed a heterogeneousl

Case reports that high-dose-rate brachytherapy using custom-designed mouthpiece mold for early-stage squamous cell carcinoma of the hard palate is useful.

BACKGROUND: Surgery is the primary therapy for hard palate cancer, but this sometime worsens oral function and unavailable for patients with several comorbidities. Radiotherapy represents an alternative curative treatment option to preserve oral function, however, there are few reports on the use of radiotherapy for squamous cell carcinoma of the hard palate (SCCHP). High-dose-rate (HDR) brachytherapy has been known as an effective treatment for SCCHP, which seems to be less invasive when mold technique is used. In our hospital, the HDR customized mold brachytherapy (HDR- customized MBT) is av

Panoramic Landmarks: Comparing LLM-Assisted, Manual Tracing, and Self-Directed Learning in Dental Education.

BACKGROUND: Accurate identification of anatomical landmarks on panoramic radiographs is a foundational yet challenging skill in dentistry. Traditional didactic teaching often requires supplementation to achieve proficiency. This study evaluates and compares the efficacy of three supplementary learning modalities: self-directed learning (SDL), traditional manual tracing (MT), and an AI-driven approach using ChatGPT. METHODS: In this prospective study, 63 third-year dental students were assigned to one of three groups (n = 21 each): SDL, MT, or ChatGPT-assisted learning. Following a theoretical
