Pre-lacrimal window maxillary sinus surgery: a computed tomography analysis and classification.

BACKGROUND: The pre-lacrimal window (PLW) approach is a promising technique for accessing otherwise inaccessible maxillary sinus lesions. The objective of this study was to determine the computed tomography (CT) dimensions, measurements, and grading of the PLW. METHODS: One hundred paranasal CT scans were included in the study. For all subjects, axial images were obtained, and multiplanar reformats were used to obtain detailed views in the coronal and sagittal planes. The width of the PLW, the width of the nasolacrimal duct (NLD), and the angle between the axis of the NLD and the hard palate

A Clinical Case of Methotrexate Toxicity.

Methotrexate is a commonly prescribed immunosuppressant and chemotherapy agent, carefully monitored by healthcare providers due to its potential adverse effects. As a result, methotrexate toxicity is relatively rare. We present the case of a 79-year-old man followed in rheumatology for symmetrical polyarthralgia, who inadvertently took methotrexate 10 mg daily, instead of weekly, leading to methotrexate toxicity. The patient presented with erosive mucositis affecting the lateral tongue, buccal mucosa, and hard palate, as well as pustular lesions on the scalp (occipital and cervical regions) ex

Surgical Repair of Palatal Fistulae in Adults-Outcomes, Challenges, and Determinants of Recurrence.

BACKGROUND: Palatal fistulae remain one of the most challenging complications following cleft palate repair, with incidence rates reported between 2.4% and 35% in recent studies. They pose significant morbidity for patients, including oronasal regurgitation, speech impairment, and failure to thrive. Although extensively studied in pediatric populations, palatal fistula repair in adults remains poorly documented. Adult patients frequently present with more chronic defects, extensive fibrosis, and reduced tissue elasticity, necessitating a tailored surgical approach. This study aimed to evaluate

Mucosal Coverage of Palatine Spines: CT Morphometric Analysis and Clinical Relevance.

INTRODUCTION: The palatine spines (PSs) form the margins of the palatal grooves (PGs) of the hard palate, which contain the greater palatine arteries and nerves. Except for one overly simplistic account in the early 1990s, there are no detailed descriptions and dimensions for the mucosa covering the PSs and grooves. METHODS: We retrospectively studied the normative morphometric features of PSs and their overlying mucosa in selected 69 patients using CT imaging. We statistically analyzed variables among the demographics, morphometrics, and several other spine characteristics, with statistical

Palatal Abscess of Endodontic Origin with Extensive Radiolucency in Maxillary CBCT Imaging.

Palatal abscesses of endodontic origin are rarer than buccal ones due to maxillary anatomy. Their clinical appearance may resemble that of other palatal illnesses, complicating diagnosis and treatment. Prevention of problems requires early detection and endodontic treatment. A 26-year-old female patient presented with a 2 cm diameter palatal abscess, significant pulsatile discomfort, fever, and enlargement of the anterior hard palate. Clinical examination showed grade 1 mobility of the central and lateral incisors, percussion discomfort, and negative pulp vitality in the case of the lateral in

Sialadenopapillary ductal tumor: Unifying sialadenoma papilliferum-like intraductal papillary tumor and tubulopapillary hidroadenoma-like tumor of the mandible - Report of an additional case.

Sialadenoma papilliferum-like intraductal papillary tumor (SP-IPT) and tubulopapillary hidroadenoma-like tumor of the mandible (TPHLTM) are rare lesions with preferential intraoral involvement and variable clinical behavior. Several studies show that SP, SP-IPT, and TPHLTM share similar morphological features and identical genetic mutations, most commonly BRAF V600E mutation, suggesting close relationship. Recently, a study proposes classifying SP-IPT and TPHLTM under the term "sialadenopapillary ductal tumor" (SDT), which seems to best reflect its clinicopathological spectrum. To date, to the

Intraoral Lesions Following Nose and Nasolabial Fold Augmentation Using Hyaluronic Acid/Poly-l-Lactic Acid Fillers: A Case Report and Literature Review.

Vascular complications after facial filling with hyaluronic acid/poly-l-lactic acid (HA/PLLA) composite fillers are rare. Herein, the authors report a case of vascular occlusion after nose and nasolabial fold augmentation with HA/PLLA that resulted in necrosis of the gingival papilla and unilateral hard palate mucosa and pulpitis. To explain the pathogenesis of necrosis of the gingival papilla and unilateral hard palate mucosa, the authors hypothesized that an embolus was accidentally injected into the infraorbital artery and nasal septum branch of the upper lip artery and subsequently flowed

An Aggressive Juvenile Nasopharyngeal Angiofibroma With Intracranial and Orbital Extension in an Adolescent Female: A Case Report and Focused Literature Review.

Juvenile nasopharyngeal angiofibroma (JNA) is a rare, benign but locally invasive vascular tumor of childhood that almost exclusively affects adolescent males. We present a very unusual case of a 14-year-old female with JNA, who presented with persistent headache, nasal obstruction, post-nasal drip, and intermittent epistaxis. Contrast-enhanced CT imaging revealed a highly enhancing mass located in the sphenopalatine fossa, coursing into the left nasal cavity, nearby soft tissues, paranasal sinuses, and eroding the hard palate. Additional imaging showed that the mass extended into the orbit an

Palatal bone healing after removal of miniscrew-assisted rapid palatal expansion devices.

INTRODUCTION: Miniscrew-assisted rapid palatal expansion (MARPE) is an effective approach for correcting maxillary constriction in adults. This study aimed to evaluate palatal bone healing after miniscrew removal in MARPE-treated patients and to identify key factors influencing the healing process. METHODS: This retrospective study included 38 patients (18 men, 20 women; mean age at ≥6 months postremoval [T2], 24.7 ± 4.2 years) and analyzed 152 removal sites (76 anterior and 76 posterior). The average number of MARPE screw turns and expansion width were 29.5 ± 6.1 mm and 5.9 ± 1.2 mm, respect

Costal Cartilage Chip Grafting for High-Arched Narrow Palate Deformity Secondary to Palatal Bone Fracture.

Palatal bone fractures are often associated with vertical maxillary fractures, but direct reduction of the palatal bone itself is rarely performed. We report a case in which a high-arched narrow palate deformity following palatal bone fracture associated with multiple facial fractures was successfully corrected by grafting costal cartilage chips to the hard palate. The patient was a 25-year-old man. At the age of 19, he sustained a cervical spinal cord injury at the C4 level and multiple facial fractures-including maxillary fracture with zygomatic and palatal bone fractures, as well as mandibu

Long-Term Speech Outcomes in Unilateral Cleft Lip and Palate: A Comparative Study of Early and Delayed Hard Palate Closure.

BACKGROUND: Evidence on the comparative effectiveness of surgical protocols for cleft lip and palate remains limited, especially regarding long-term speech outcomes. Therefore, this study evaluates speech outcomes at 5, 12, and 22 years in patients with unilateral cleft lip and palate (UCLP) to guide protocol selection. METHODS: This prospective cross-sectional study included 285 UCLP patients treated at 2 Dutch academic hospitals. Patients were assessed at 1 of 3 predefined ages (5, 12, or 22 y) during routine follow-up; only a minority were assessed at more than one time point. Four protoco

Beyond failure: a case report on brain state changes during virtual reality-induced hypnosis in pediatric patient.

BACKGROUND: Management of fear in anxious patients is challenging, particularly in children. Virtual reality induced hypnosis may help during the procedures, changing vital parameters and brain states. Modifications in brain activity can be easily traced with wearable instruments. CASE PRESENTATION: An 11-years old boy was scheduled for avulsion of teeth 15 and 25, which were misplaced in the hard palate. Due to his fear of procedure, he was exposed to virtual reality-induced hypnosis. The brain state was continuously monitored, showing light sedation, associated to low spectral edge frequenc

Mesenchymal stem cells undergoing apoptosis recover cyclophosphamide-induced cleft palate through modulation of NLR signaling mediated pyroptosis.

BACKGROUND: Cleft palate is a common developmental disorder in craniofacial region, which cause the severe postnatal oral dysfunction. Mesenchymal stem cell (MSC) therapy has been revealed as a promising therapeutic approach in fetal developmental defects. However, since placental barrier blocks the transfer of cells from maternal circulation, how the MSC exert therapeutic effects and biological function to repair developmental injuries and maintain tissue homeostasis remains elusive. METHODS: Cyclophosphamide (CP)-induced cleft palate mice were used with replenishment of adipose-derived MSC

A Neonate with FNAIT Supported by Placental Chronic Histiocytic Intervillositis and Confounded by Maternal Preeclampsia: A Case Report.

INTRODUCTION: Fetal and neonatal alloimmune thrombocytopenia (FNAIT) occurs in the setting of maternal anti-human platelet antigen (anti-HPA) antibodies against paternally derived fetal platelet antigens. Recent studies have also demonstrated an association between chronic placental inflammation and FNAIT, specifically low-grade chronic histiocytic intervillositis (CHI). We present a neonate with profound thrombocytopenia after delivery with co-occurring CHI, whose platelet counts recovered rapidly with platelet transfusions, born to a primigravida mother with late-onset preeclampsia. Oral Candidiasis Mimicking Mucosal Lesions of Bullous Pemphigoid During Systemic Corticosteroid Therapy: A Case Report.

Bullous pemphigoid (BP) is an autoimmune blistering disorder mainly affecting the elderly. Although primarily cutaneous, bullous pemphigoid can occasionally involve the oral mucosa, which complicates diagnosis and management. We report an 88-year-old woman with diabetes and dementia who presented with tense bullae on her limbs. Systemic corticosteroid therapy improved the skin lesions, but new painful erosions appeared on the hard palate. Although mucosal BP was considered, the concurrent improvement of cutaneous symptoms suggested an opportunistic infection instead. Culture confirmed Candida

Radiological and Anatomical Evaluation of the Hard Palate in Healthy Adults: A Retrospective Study.

OBJECTIVES: Due to the hard palate's structure and position, it serves as one of the main structural components in the oral sensorimotor system. This study aimed to examine the hard palate angle, inclination, depth types, and the presence of S-shaped projection in healthy individuals. METHODS: Cone-beam computed tomography (CBCT) images of 130 healthy individuals, aged between 18 and 58 years, were retrospectively analyzed. Four parameters, such as hard palate angle (HPA), hard palate inclination (HPI), hard palate depth types (HPDT), and hard palate S-shaped projection, were statistically ev
