Gait pattern stratification in persons with incomplete spinal cord injury: a data-driven approach.

BACKGROUND: Incomplete spinal cord injury (iSCI) often causes heterogeneous locomotion dysfunctions, depending on remaining sensorimotor function. Clinical tests and traditional gait analysis have limited ability to quantify the diversity of gait impairments. Unsupervised learning techniques can objectively identify common gait patterns among the overall heterogeneity. Explainable artificial intelligence approaches, when combined with machine learning models, can reveal important features often missed by traditional gait analyses. This study presents a framework to characterize gait heterogene

Lower Extremity Compartment Syndrome Due to Capillary Leak Syndrome Following 60% Total Body Surface Area Burn Injury.

Lower extremity compartment syndrome (LECS) following burn injury in the absence of circumferential burns is rare. Capillary leak syndrome (CLS) is a condition characterized by systemic capillary hyperpermeability and can be triggered by the multisystem inflammatory response seen in extensive burns. The resulting intravascular fluid loss into the interstitial space can elevate compartment pressures and contribute to the development of compartment syndrome. In the case presented here, the patient developed LECS in the setting of severe burn injury with clinical and laboratory evidence consisten

Trichosporon asahii infection in a lower limb stump: A diagnostic challenge in a severely burned patient.

Trichosporon asahii is an opportunistic yeast that colonizes the skin and mucous membranes but may cause severe invasive infections in critically ill patients. We report the case of an 81-year old man with extensive second- and third degree thermal burns (30-39% total body surface area) who, during the third week of intensive care, developed a deep infection of a previously amputated femoral stump. Cultures repeatedly yielded multidrug-resistant Pseudomonas aeruginosa and T. asahii, identified by MALDI-TOF mass spectrometry. Combined surgical management and antimicrobial therapy with meropenem

Enhanced biomechanical outcomes of split-thickness skin grafts with a three-dimensional collagen-elastin matrix in deep second-degree burns: A 12-month comparative study.

BACKGROUND: Split-thickness skin grafting (STSG) remains the standard for deep second-degree burns, but grafted skin often shows suboptimal biomechanical properties. The dermal substitute MatriDerm® may improve elasticity, structural integrity, and long-term function. This study evaluated the safety, feasibility, and biomechanical outcomes of a single-stage application of 1 mm MatriDerm® combined with STSG versus STSG alone in lower-extremity burns. METHODS: In this single-center retrospective cohort study (January 2023-June 2025), 28 patients received 1 mm MatriDerm® with STSG (1:1 mesh, 0.2

Application of Novel Thrombosis Scale in Fracture Related Lower Extremity Deep Venous Thrombosis.

BackgroundLower limb fractures with peripheral deep venous thrombosis (DVT) are common, yet unified stratified treatment standards are lacking. Quantifying thrombus burden guides decisions like inferior vena cava (IVC) filter implantation or anticoagulation. Existing scoring systems rarely integrate peripheral vein anatomy and imaging features, limiting clinical utility.ObjectiveTo develop a dual-dimensional scoring scale for thrombus burden quantification and validate its value in guiding stratified treatment.MethodsA retrospective study of 68 fracture-related DVT patients (Suzhou TCM Hospita

Timing and Outcomes of Free Flap Reconstruction in High-Voltage Electrical Burns of the Extremities: A Retrospective Study.

BACKGROUND: High-voltage electrical burns of the extremities often result in deep tissue destruction with exposure of vital structures, necessitating free flap reconstruction. However, the optimal timing for reconstruction remains uncertain, particularly in the presence of evolving tissue viability. METHODS: This retrospective study included 23 patients with full-thickness high-voltage electrical burns involving the extremities who underwent free flap reconstruction. Patients were divided into two groups based on timing of flap coverage: early (<21 days) and delayed (>21 days, after at least

Beyond Injury: Length of Stay and Rehabilitation Disparities in Houseless Trauma Patients.

INTRODUCTION: Houseless trauma patients (HTPs) face triple the complications and nearly double the mortality of nonhouseless trauma patients (nHTPs), which may be related to a higher burden of undiagnosed or untreated chronic conditions. This study aims to explore whether HTPs have a longer length of stay (LOS) and encounter more difficulties in accessing rehabilitation services compared to nHTPs. We hypothesize that HTPs, as compared to nHTPs, have longer LOS and decreased access to rehabilitation services. METHODS: The 2021-2023 Trauma Quality Improvement Program database was queried for ad
