A 56-year-old man was involved in an automobile accident and had abdominal blunt injury with the steering wheel. He developed slight abdominal distension and pulseless, cold sensation and peripheral cyanosis of the left lower extremity which suggested acute arterial occlusion. DSA revealed complete obstruction of the common iliac artery near its origin and the leakage of contrast medium at peripheral branch of the SMA. We have described an infant in whom migration of a transected umbilical artery catheter resulted in sudden vascular ischemia of an extremity. Early operative retrieval was successful, but long-term effects of this complication were encountered. We have outlined an approach to the initial management of the complication. In addition, this case demonstrates the need for careful follow-up for identification and treatment of the long-term sequelae in the involved extremity.

The results of treatment of sixty-five symptomatic lesions in forty-three patients who had fibrous dysplasia were reviewed. For fourteen (93 per cent) of the fifteen times that a lesion in the upper extremity was treated non-operatively, the result was satisfactory. Prognosis of compound open fractures has been transformed by the use of plastic surgery to cover wounds. However, loss of substance of lower third of leg remains a delicate problem frequently requiring the use of microsurgical techniques by experienced teams. The sural cutaneo-aponeurotic distal base flap, the subject of a detailed anatomical study by the authors, can resolve the problem without affecting muscle capital and at the price of minor esthetic sequelae. Based on an analysis of the surgical treatment of 240 patients with traumas of the main extremity vessels (147 associated traumas included) the authors show that favorable outcomes of such injuries are due to such factors as timely diagnosis, successful struggle with the traumatic shock and blood loss, adequate and skillful technique of surgical measures, adequate treatment of concomitant injuries and character of therapy in the postoperative period.

A study of patients with serious lower limb injuries is presented. With these injuries it is important to decide whether the limb should be salvaged at the risk of losing a life. Seventy-six patients are described and compared with a group of patients with similar injury severity scores but without serious lower limb injury. Aspergillosis may complicate the course of the child in an immunocompromised state. Peripheral vascular injuries associated with falls from heights are uncommon.
