Compressive cervicothoracolumbar epidural haematoma following lead insertion for spinal cord stimulator trial.

Spinal cord stimulators (SCS) are used as treatment for back pain and associated radicular symptoms recalcitrant to both conservative measures and spinal surgery. This report reviews a case of a male in his 40s with a medical history pertinent for pulmonary hypertension presenting with lower extremity weakness and sensory deficits, inability to void and lower extremity pain after SCS trial procedure. MRI revealed a spinal epidural haematoma from C6 to L2 and cord compression from T10 to T11 inferiorly to L2. Coagulation studies were within normal limits. This profound bleeding is suspected to

Effect of preoperative liposomal bupivacaine single-injection pericapsular nerve group (PENG) block on lower extremity deep vein thrombosis in elderly patients with hip fractures: a randomised controlled, double-blind, prospective clinical study protocol.

INTRODUCTION: Deep vein thrombosis (DVT) of the lower limbs has a significantly higher incidence among elderly populations than that observed in other types of fractures, prolonged immobilisation and the systemic inflammatory response triggered by preoperative pain are the main risk factors. Liposomal bupivacaine (LB) single-injection pericapsular nerve group (PENG) block has demonstrated effective analgesia both before and after surgery, while preserving motor function in patients with hip fracture. Although regional nerve block is a well-established component of preoperative multimodal analg

5-Factor Modified Frailty Index predicts 30-day complications after amputation for open lower-extremity fracture: an American College of Surgeons Trauma Quality Improvement Program database study.

PURPOSE: Open lower-extremity fractures are high-energy injuries, with severe cases necessitating amputation. While frailty predicts adverse surgical outcomes, its role in postoperative risk among patients undergoing amputation for open lower-extremity fractures has not been defined. This study aimed to determine whether frailty, measured by the 5-Item Modified Frailty Index (mFI-5), predicts 30-day postoperative complications following surgical amputation for open lower-extremity fractures. METHODS: This retrospective cohort study utilized the American College of Surgeons Trauma Quality Impr

3D guided large osteochondral allograft reconstructions for post-traumatic osteoarticular bone loss of the knee: a technical note.

PURPOSE: Post-traumatic osteoarticular bone loss of the knee remains challenging to treat. Allograft reconstruction can yield favourable outcomes, and 3D virtual surgical planning may further improve results by enabling precise templating, optimal graft selection, and accurate fixation. This study aimed to introduce and evaluate 3D-assisted planning for knee allograft reconstruction. METHODS: In two patients with severe post-traumatic knee osteoarticular bone loss—one in the medial distal femur and one in the proximal tibial plateau—3D-assisted resection and allograft reconstruction were perfo

Diagnostic performance of a gravity varus stress computed tomography protocol in detecting instability in isolated coronoid fractures.

BACKGROUND: Instability associated with isolated coronoid fractures can be subtle and frequently missed on routine imaging. A stress-based computed tomography (CT) protocol may improve detection of occult instability in isolated coronoid fractures. The purpose of this study was to evaluate the diagnostic performance of a gravity varus stress CT protocol in detecting instability in isolated coronoid fractures and compare it with routine static CT. METHODS: A retrospective cohort study of patients with isolated coronoid fractures who underwent either gravity varus stress CT or routine static CT

What factors influence outcomes in olecranon fractures in older adults? A cohort study of operative and nonoperative management.

BACKGROUND: Olecranon fractures occur more commonly in older individuals. For patients aged ≥70 years, operative management is often considered standard of care, although recent evidence has supported nonoperative treatment in frail and/or elderly patients. With evolving treatment indications, more granular evidence is beneficial to guide patient-specific decision-making. The aim of this investigation was to explore patient and treatment factors associated with outcomes after displaced, closed olecranon fractures in older individuals. METHODS: This retrospective cohort study with prospective

Knee Osteoarthritis (KOA) Severity Influences Proximal Femoral Biomechanics and Predicts Pertrochanteric Fracture Instability: A Retrospective Radiographic Study.

Background and Objectives: Intertrochanteric hip fractures (ITFs) are common in older adults and frequently coexist with knee osteoarthritis (KOA). Although both conditions share key biomechanical risk factors, the specific relationship between KOA severity and ITF stability has not been well defined. Recent evidence suggests that degenerative knee changes may alter lower-limb load distribution and increase susceptibility to unstable fracture patterns. This study evaluated whether KOA severity, graded using the Kellgren-Lawrence (KL) system, is associated with ITF stability according to the 20

Biological activation of prolonged delayed fracture healing following abaloparatide under stable circular external fixation: successful limb salvage of a Gustilo type III-C pilon fracture in severe immune thrombocytopenia-a case report.

BACKGROUND: Delayed fracture healing may persist despite adequate mechanical stability, reflecting biological insufficiency rather than mechanical failure. Although abaloparatide has osteoanabolic effects, its role in prolonged delayed fracture healing remains unclear. CASE PRESENTATION: A 59-year-old Japanese man with severe immune thrombocytopenia (platelet count approximately 5000/microL) sustained a Gustilo type III-C open pilon fracture after a crush injury. Emergency management included aggressive debridement, posterior tibial artery and vein repair, and circular external fixation. Beca

Upper extremity fractures and subsequent total knee arthroplasty incidence in knee osteoarthritis: a cross-continental propensity score-matched cohort study.

BACKGROUND: Knee osteoarthritis (KOA) represents a major clinical and economic burden globally, and total knee arthroplasty (TKA) remains the definitive treatment for end-stage disease. Upper extremity fractures in older adults often indicate skeletal fragility and reduced physiological reserve, but their influence on subsequent TKA incidence remains unclear. We investigated whether upper extremity fractures influence TKA incidence among patients with KOA. METHODS: We conducted a retrospective propensity score-matched cohort study using the TriNetX US Collaborative Network (2005–2024) and perf

Deltoid All-Inside Repair in SER IV Bimalleolar-Equivalent Ankle Fractures.

BackgroundAnkle fractures are the second most common fractures of the lower limb. In supination-external rotation (SER) stage 4 injuries, the medial malleolus or the deltoid ligament is disrupted. The stability of the deltoid ligament is essential for the functionality, however, the need for surgical repair remains debated. The aim of this study is to assess deltoid stability in 12 ankle fractures treated with an all-inside repair technique.MethodsA prospective case series of 12 patients (8 men,4 women aged between 25 and 53 years who had a stage 4 LH SER ankle fracture with unstable deltoid l

Comparison of muscle status and knee joint function between autologous hamstring tendon and peroneus longus tendon reconstruction of anterior cruciate ligament.

This study aims to compare knee and ankle muscle/function outcomes following anterior cruciate ligament reconstruction using autologous hamstring tendon (HT) versus peroneus longus tendon (PLT). In this retrospective study, patients who underwent anterior cruciate ligament reconstruction with either PLT or HT grafts between 2015 and 2024 were analyzed. Postoperative follow-up data included muscle properties (tone-F, stiffness-S, elasticity-D) around the knee, International Knee Documentation Committee and Lysholm scores, and thigh circumference difference. For the PLT group, muscle properties

Secondary Limb-Salvage Fifty Years After Vascular Injury: A Case Report.

A 73-year-old veteran presented to vascular surgery with a nonhealing left lower leg wound. Fifty years before the presentation, he sustained a gunshot wound to the left leg during combat. Emergent superficial femoral artery to popliteal artery bypass using vein graft and complex bone and soft-tissue repair resulted in successful primary limb salvage. He was entered into the Vietnam Vascular Registry at the time of his initial injury. On current presentation, he was diagnosed with infected myositis ossificans and bypass occlusion. He was originally offered wound care alone with possible amputa
