Delayed Pseudoaneurysm Formation after Carotid Artery Stenting in a Heavily Calcified Lesion: A Possible Mechanism of Mechanical Vessel Injury.

OBJECTIVE: Pseudoaneurysm formation after carotid artery stenting (CAS) is a rare but potentially life-threatening complication. Herein, we report a case of delayed pseudoaneurysm formation associated with a heavily calcified lesion and discuss the possible underlying mechanisms. CASE PRESENTATION: An 87-year-old man presented with symptomatic left carotid artery stenosis. Carotid ultrasonography demonstrated a heavily calcified plaque without marked flow acceleration, and DSA revealed 69% stenosis according to the North American Symptomatic Carotid Endarterectomy Trial (NASCET) criteria. CAS

Laparoscopic and Robotic Device Malfunction: A Scoping Review.

Laparoscopic and robotic surgeries have become the gold standard for many operations. Device malfunctions during these procedures represent a preventable source of patient harm; however, the trends in malfunction reporting and their implications for surveillance systems, surgical training, and reporting infrastructure have not been comprehensively reviewed. Our aim is to conduct a scoping review of device malfunctions during minimally invasive abdominopelvic surgeries. Four databases (Web of Science, Scopus, Embase, and PubMed) were searched for relevant articles published between January 1, 1

Suture Anchor Fixation Combined With Transosseous Tunnels for a Large Patellar Osteochondral Fracture After Acute Patellar Dislocation.

Patellar osteochondral fractures (OCFs) are common concomitant injuries in acute patellar dislocation, particularly in adolescents. Large free fragments (>15 mm in diameter) are relatively rare and require surgical reduction and fixation to restore articular surface congruity and prevent mechanical symptoms and secondary patellofemoral osteoarthritis. This article reports the case of a 16-year-old female athlete who sustained an acute patellar dislocation with a large patellar OCF. The osteochondral fragment was reduced and fixed using a suture anchor combined with transosseous tunnel techniqu

A Classic Cutaneous Presentation of Thymoma-Associated Multiorgan Autoimmunity and Review of the Literature.

Thymoma-associated multiorgan autoimmunity (TAMA) is a rare paraneoplastic syndrome characterized by graft-versus-host disease (GVHD)-like manifestations. The disorder is thought to arise from a disruption to central immune tolerance of T cells secondary to neoplastic growth of thymic epithelial cells. Thymic epithelial cells are crucial in T-cell education: distinguishing between self-antigens and foreign antigens. This process relies on a protein, the autoimmune regulator (AIRE), which negatively selects and eliminates T cells that are self-reactive. With neoplastic transformation of thymic

Prioritization and Severe Primary Graft Dysfunction after Lung Transplantation.

BACKGROUND: Primary graft dysfunction (PGD) is the leading cause of early morbidity and mortality after lung transplantation (LTx). Critically ill candidates may receive priority on the transplant waiting list; however, the relationship between prioritization and the development of severe PGD remains unclear. METHODS: We performed a retrospective analysis of a prospectively maintained cohort including all consecutive LTx performed at a tertiary center between January 2016 and June 2025. Recipients were categorized according to prioritization status. Severe PGD was defined as grade 3 PGD accor

A Novel Case Report of Closed-Loop Spinal Cord Stimulation for Neuropathic Pain in Cervical Spinal Cord Injury.

BACKGROUND: Neuropathic pain after spinal cord injury (SCI) is common and often refractory. Closed-loop spinal cord stimulation (CL-SCS) is a novel intervention that adjusts stimulation based on evoked compound action potentials in real time, but its use for pain in complete SCI is not well described. CASE REPORT: A 45-year-old man with complete C6-C7 SCI developed severe, diffuse neuropathic pain below the injury level refractory to multiple medications, an intrathecal fentanyl pump trial, and ketamine infusions. With extensive epidural scarring, a staged buried CL-SCS trial with a combined

Hesperidin attenuates methotrexate-induced cardiotoxicity: Modulation of Nrf2 antioxidant defense and NF-κB/iNOS proinflammatory signaling.

Methotrexate (MTX) is a commonly used chemotherapeutic and immunosuppressive agent, although serious adverse effects, including cardiotoxicity, sometimes compromise its clinical use and effectiveness. This work was designed to investigate the mechanisms underlying MTX-induced cardiac injury and to evaluate the potential cardioprotective efficacy of hesperidin (HES), a bioflavonoid derived from citrus fruits, known for its potent antioxidant and anti-inflammatory properties. Thirty-two adult male Sprague-Dawley rats were randomly divided into four groups (n = 8/group) and treated for six weeks:

Unexpected Identification of Two Medialized Tympanostomy Tubes in an Adult: A Case Report.

Tympanostomy tube placement is one of the most commonly performed otologic procedures and is widely used in the management of chronic otitis media with effusion (OME), recurrent acute otitis media (AOM), and Eustachian tube dysfunction (ETD). Although generally safe, complications may occur, and medialization of a tympanostomy tube into the middle ear space is a rare but recognized complication that may be associated with surgical removal due to the potential for chronic inflammation, conductive hearing loss, or middle ear injury. We present a case of two medialized tympanostomy tubes identifi

Early hematoma expansion provides incremental prognostic information beyond admission characteristics for 6-month functional outcome after adult traumatic brain injury.

BACKGROUND: Early hematoma expansion (HE) reflects interval progression of traumatic intracranial hemorrhage, but its association with long-term outcome and its prognostic contribution beyond admission characteristics remain incompletely defined. We evaluated whether HE was independently associated with 6-month functional outcome and provided incremental prognostic information beyond admission characteristics. METHODS: This single-center retrospective study included 640 adults with traumatic brain injury, including 285 with HE. HE was determined from the first two in-hospital head computed to

Quinolinic acid contributes to maladaptive kidney injury and acute kidney injury-to-chronic kidney disease transition.

The transition from acute kidney injury (AKI) to chronic kidney disease (CKD) remains a major clinical problem with unclear underlying mechanisms. Emerging evidence suggests that alterations in tryptophan metabolism, particularly in the production of downstream metabolites such as quinolinic acid (QA), play a role in renal pathophysiology. QA, an intermediate in de novo nicotinamide adenine dinucleotide biosynthesis, is metabolized by quinolinate phosphoribosyltransferase (QPRT). We investigated whether QA contributes to kidney injury using models of folic acid-induced AKI-to-CKD transition an
