Transnasal Penetrating Intracranial Injury with a Retained Foreign Body: Diagnostic Challenges and Endoscopic Surgical Management-A Case Report.

Introduction: Transnasal penetrating intracranial injuries represent an exceptionally rare form of traumatic brain injury, characterized by high mortality and a substantial risk of severe neurosurgical and infectious complications. Self-inflicted injuries involving the penetration of a foreign body through the nasal cavity into the brain are exceedingly uncommon and require prompt diagnosis and a multidisciplinary treatment approach. Objective: This study aimed to present a rare clinical case of transnasal penetration of a foreign body (a ballpoint pen refill) into the brain parenchyma during

Renal Dysfunction in Chronic Obstructive Pulmonary Disease Lung-Kidney Interorgan Crosstalk with Cardiac Mediation.

Chronic obstructive pulmonary disease (COPD) is increasingly recognized as a systemic disorder with clinically significant extrapulmonary manifestations. Among these, renal dysfunction-manifesting as chronic kidney disease (CKD) and acute kidney injury (AKI)-is highly prevalent, frequently underdiagnosed, and strongly associated with adverse clinical outcomes. Meta-analytic data indicate that COPD is associated with more than a twofold increase in CKD prevalence, independent of shared risk factors such as age, smoking, hypertension, and diabetes. CKD in COPD is associated with increased mortal

Excision of calcified left atrial myxomas: a surgical challenge.

Atrial myxomas are the most frequent primary cardiac tumors and are typically cured by surgical resection. Prognosis is excellent in most cases, but complex tumors that involve adjacent valvular or septal structures may lead to challenging repairs and adverse outcomes. We present two patients with atrial myxomas who underwent surgery with strikingly different results. The first, a 72-year-old woman, had a broad-based, calcified left atrial myxoma encroaching on the mitral annulus. Following resection, annular stability was preserved with annuloplasty and patch reinforcement, resulting in succe

Glomerular Injury Findings in Patients with Thalassemia Minor.

Renal involvement in β-thalassemia minor (β-TMin) has been described mainly in case reports and small observational studies, and its clinical significance remains incompletely characterized. Using real-world data from routinely collected electronic medical records, we performed a retrospective cohort study including 1516 adult patients with β-TMin insured by Clalit Healthcare Services to explore renal abnormalities identified during routine clinical care. Urine testing for hematuria, microalbuminuria, and proteinuria was not performed systematically but was ordered at clinicians' discretion, r

Escalation to transplantation in refractory stone heart syndrome: a case report of advanced mechanical circulatory support failure.

BACKGROUND: Stone heart syndrome was first described by Denton Cooley in 1972 and represents an extremely rare but severe complication following cardiac surgery. The condition is characterized by an irreversible, concentric, spastic contracture of the myocardium occurring during reperfusion which does not respond to either pharmacological or mechanical interventions. The underlying pathophysiology remains poorly understood, but an association with suboptimal cardioprotection and prolonged ischemia time has been suggested. CASE PRESENTATION: A 22-year-old otherwise healthy patient was admitted

Acupuncture for Poststroke Dysphagia: An NTS-Centered Hypothesis-Generating Framework Linking the Brainstem Swallowing Network, Neuroimmune Regulation, and Vagal Anti-Inflammatory Signaling.

BACKGROUND: Poststroke dysphagia (PSD) may reflect not only focal motor pathway injury but also instability of the brainstem swallowing network under neuroimmune, glial, and vascular microenvironmental stress. METHODS: This mechanism-oriented narrative review synthesized evidence on swallowing central pattern generator (CPG) organization, nucleus tractus solitarius (NTS)-related sensory integration within the medullary swallowing network, poststroke neuroinflammation, altered glial responses, blood-brain barrier/neurovascular unit (BBB/NVU) dysfunction, vagus-related anti-inflammatory regulat

Double-row wired anchors combined with "8" cross tension bands: an innovative procedure for the treatment of inferior pole fractures of the patella and its finite element and biomechanical evaluation.

OBJECTIVE: To propose and verify the biomechanical properties of a new double-row suture anchor combined with figure-of-eight cross tension band technology for the treatment of inferior pole fractures of the patella, in order to overcome the shortcomings of traditional fixation methods in achieving stable fixation and reducing implant-related complications. METHODS: (1) Based on clinical CT data, a three-dimensional finite element model of patellar inferior pole fracture was reconstructed, and the new fixation technology (deep double-row anchors in the proximal bone fragment combined with figu

Risk Factors for Recurrence and Reoperation after Surgical Treatment of Cubital Tunnel Syndrome: A Narrative Review.

Background. Cubital tunnel syndrome is the second most common compression neuropathy in the upper limb. The outcomes of operative treatment of cubital tunnel syndrome are predictable to some extent. Approximately 70-80% of patients are satisfied with the good long-term outcomes, but the remaining 20-30% benefits less or even are dissatisfied with the treatment. Moreover, in 7-24% recurrence occurs, or necessity of revision surgery due to lack of improvement after primary operation. The objective of this study was review of the literature identifying factors that affect the risk of recurrence

At the Other End of the Spectrum: Bigorexia as an Emerging Cardiovascular Phenotype in Adolescents.

OBJECTIVE: Bigorexia, or Muscle Dysmorphia, is an increasingly recognized body image disorder among adolescents, characterized by an excessive drive for muscularity and distorted self-perception. Although its psychological dimensions are well described, its cardiovascular implications remain underexplored. This review examines the association between bigorexia-related behaviors and early cardiovascular injury, and proposes a conceptual framework for this evolving phenotype. DATA SOURCES: A comprehensive narrative review was conducted using PubMed, Scopus, and Web of Science. Search terms incl

Early laboratory indicators of acute metabolic decompensation during emergency presentations in pediatric maple syrup urine disease.

UNLABELLED: Acute metabolic decompensation (AMD) is the most life-threatening complication of maple syrup urine disease (MSUD). Recognizing AMD early in the emergency department (ED) is difficult because the most important clinical indicators-neurological manifestations-typically appear late during decompensation, and plasma leucine measurements are frequently delayed. This study aimed to identify laboratory biomarkers that can be obtained quickly and support early AMD detection at ED. This retrospective study included MSUD patients aged 28 days-18 years admitted to a tertiary pediatric ED bet

N-terminal proBNP adds prognostic value to high-sensitivity cardiac troponin I in elective thoracic surgery: an observational cohort study.

BACKGROUND: Perioperative myocardial injury (PMI) is a common complication following non-cardiac, particularly thoracic, surgery and is associated with increased cardiovascular risk. Although guidelines recommend cardiac biomarker monitoring to detect PMI, its implementation in routine clinical practice remains limited. OBJECTIVE: To evaluate the combined use of high-sensitivity cardiac troponin I (hs-cTnI) and N-terminal pro-brain natriuretic peptide (NT-proBNP) in predicting major adverse cardiovascular events (MACE) following elective thoracic surgery, and to determine whether NT-proBNP pr

Outcomes of vertebral column resection in rigid, severe adult and pediatric spinal deformities.

STUDY DESIGN: Retrospective cohort study. OBJECTIVES: Managing severe and rigid spinal deformities is challenging to correct. Vertebral column resection (VCR) is often necessary for adequate correction. Substantial risks remain with blood loss, prolonged operative time, and risk to the neural elements. We studied VCR in rigid spinal deformities, including deformity correction, intraoperative neurophysiological monitoring (IONM) alerts, spinal cord/nerve injuries, and perioperative adverse events. METHODS: We conducted a retrospective review of 55 patients who underwent VCR for severe and rig

Nutritional therapy in critically ill patients with acute kidney injury on renal replacement therapy: An evidence review.

Protein-energy wasting is common in critically ill patients with acute kidney injury. Renal replacement therapy (RRT) has negative effects on nutrient balance and can further deteriorate nutritional status. The prescribed dialysis modality is a determinant of micronutrient and amino acid losses, with continuous RRT (CRRT) leading to the greatest losses. It is essential to assess and monitor the nutritional status in any RRT modality, implementing conventional nutritional assessment tools (SGA) and technologies for monitoring hydration and body composition (BIA, US). Nutritional therapy should

Chronic Liver Disease, Liver Damage and Liver Failure After Hypoabsorptive Bariatric Surgery: A Dose-Dependent Relationship and Multisystemic Consequences.

BACKGROUND/AIMS: Metabolic bariatric surgery remains the most potent weapon we have against severe obesity and its metabolic consequences. Yet, its effects on the liver are far from uniform. Although restrictive and mixed procedures like sleeve gastrectomy and Roux-en-Y gastric bypass consistently improve metabolic dysfunction-associated steatotic liver disease (MASLD), hypoabsorptive operations carry a distinct and troubling risk of progressive liver injury. The available evidence, drawn predominantly from case series, registry data and retrospective analyses, suggests that this risk is not a

[Advances in growth impairment associated with pediatric respiratory diseases].

Pediatric respiratory diseases are significantly associated with growth impairment. Through core pathways including chronic hypoxia, systemic inflammation, metabolic dysregulation, and pharmacological interference, these conditions suppress the growth hormone-insulin‑like growth factor axis and peripheral signaling, resulting in growth retardation. This article summarizes mechanistic advances regarding growth impairment in obstructive sleep apnea, asthma, chronic lung injury, and recurrent respiratory infections. Accordingly, it discusses intervention strategies encompassing growth monitoring,
