Arrow Revision Palatoplasty: A Novel Technique for Soft-Palate Revision in Patients with Velopharyngeal Insufficiency and a Large Pharyngeal Gap.

Up to 30% of patients undergoing primary palatoplasty require secondary surgery due to velopharyngeal insufficiency (VPI). In patients with a large pharyngeal gap seen on nasoendoscopy, surgical intervention may risk the development of obstructive sleep apnea. Many surgical techniques are available to address VPI, but for patients with VPI and a large pharyngeal gap, there are limited surgical options available that do not require obstruction of the pharyngeal airspace with tissue flaps or implants. The authors describe arrow revision palatoplasty, a novel technique that combines revision pala

Tensor veli palatini muscle and palatine aponeurosis: Their independent fetal development and delayed meeting.

To demonstrate the prenatal morphology of the palatine aponeurosis (AP) and establish how the tendon of the tensor veli palatini (TVP) muscle meets the aponeurosis, we examined serial histological sections of 25 heads from human fetuses at approximately 9-16 weeks of gestational age (GA). At 9 weeks GA, the initial AP was identified as mesenchymal condensation that extended anteriorly to attach to the palatine bone. The TVP muscle extended inferiorly and curved along the greater palatine nerve; however, the TVP tendon does not reach the AP. At 10 weeks GA, the AP had developed as a tendinous o

Persistent foot-and-mouth disease virus infection of the bovine nasopharynx is associated with suppression of innate and cellular immunity.

Foot-and-mouth disease is a devastating disease of cattle that is caused by foot-and-mouth disease virus (FMDV). After acute infection, FMDV persists in the upper respiratory tract of about 50% of infected cattle. The persistent infection is characterized by very localized viral replication in the absence of clinical signs, but the underlying mechanisms are still not clear. In our study, we investigated tissue samples collected from 20 cattle which had been experimentally infected with FMDV O/FRA/1/2001. In 17 animals, the infection persisted for longer than 28 days. Epithelial tissue from the

Double-Opposing Z-Plasty for Velopharyngeal Insufficiency: Technical Advancements and Outcomes.

BACKGROUND: Furlow's double-opposing Z-plasty (DOZ) can be used to effectively treat velopharyngeal insufficiency (VPI) with minimal complications, but its success rate remains modest. This article presents modifications made to improve DOZ outcomes. METHODS: This retrospective study included 114 patients with marginal VPI who had undergone DOZ performed by a single senior surgeon between 2007 and 2024. Patients with a closure ratio of ≥0.7 on nasopharyngoscopy were selected for this study. The DOZ procedure was modified to maximize soft palate lengthening and include fat grafting. Patients w

Comprehensive Analysis of the Influence of Soft Palate Inflammation in Brachycephalic Dogs with BOAS III.

Brachycephalic obstructive airway syndrome is a group of abnormalities that primarily affect the upper respiratory tract, particularly in dogs-especially in English and French bulldogs, pugs, and Boston terriers. The description and the consequences of these anomalies are well known. We performed a detailed histopathological analysis of soft palate samples taken from brachycephalic dogs with BOAS III. We examined the impact of the severity and composition of inflammatory infiltrates on individual histological structures, such as mucosal membrane, serosal and mucosal glands, and muscles. The st

Computational fluid dynamics simulation of upper airway flow and soft tissue deformation in a mouth-breathing patient: a case study.

OBJECTIVE: The primary objective of this study was to establish a three-dimensional fluid-structure interaction (FSI) model to compare the biomechanical effects of three distinct breathing modes-nasal (NA), oronasal (ONA), and oral (OA) ventilation-on upper airway dynamics in mouth-breathing patients. A key aim was to investigate a potential biomechanical link between mode-specific hydrodynamic forces and the soft tissue remodeling associated with adenoidal facies. METHODS: Mouth-breathing volunteer was selected, and cone-beam computed tomography (CBCT) and magnetic resonance imaging (MRI) im

Minor modification of soft palate surgical technique resulted in a considerable increase of residual cleft dimensions for unilateral cleft lip and palate.

INTRODUCTION: A two-stage surgical approach, starting with the soft palate, has been practiced in the Gothenburg cleft team for unilateral cleft lip and palate (UCLP) since decades. To improve the velopharyngeal function, a modified soft palate closure (SPC) technique was introduced. Clinical observations did however indicate that the novel technique resulted in a deviant residual cleft shape, potentially contributing to future compromised dental arch form. AIM: This study aimed to compare residual cleft dimensions (area, width and length) between different SPC techniques and at various age,

Atypical soft palate pyogenic granuloma in an elderly anticoagulated patient presenting with acute oropharyngeal hemorrhage.

Pyogenic granuloma (PG) is a benign lobular capillary proliferation of mucocutaneous tissues that usually arises in response to local irritation. It typically occurs on the gingiva, with extragingival sites such as the soft palate being very rare. We report a 69-year-old female on Apixaban who presented with sudden, profuse oropharyngeal bleeding. Examination revealed a friable, sessile erythematous mass on the right soft palate that was actively bleeding. The lesion was surgically excised with electrocautery under local anesthesia after appropriate management of anticoagulation. Histopatholog

Management of an Oral Squamous Cell Papilloma in an Unusual Maxillofacial Location: A Case Report.

According to the World Health Organization (WHO) classification, oral squamous cell papilloma is a benign, hyperplastic, exophytic proliferation characterized by a verrucous or cauliflower-like morphology, with either a sessile or pedunculated base. It is among the most common benign epithelial lesions of the oral cavity, showing a predilection for the hard and soft palate. Although human papillomavirus (HPV) infection has been implicated in its pathogenesis, the precise viral role remains controversial. We describe a rare case of oral squamous cell papilloma arising in the pterygomandibular r

[Equine laryngeal dysplasia - overview of variable clinical presentation based on 6 cases].

Equine laryngeal dysplasia (ELD) is the umbrella term for various malformations of laryngeal structures that arise from the 4th and 6th branchial arches during embryogenesis. The disease is rare and may be subclinical. In symptomatic patients with clinical signs, the clinical presentation is highly variable and is the result of functional limitations of the larynx as well as of the esophageal sphincter. The spectrum of signs may range from respiratory changes such as abnormal respiratory noise, coughing, and at times dyspnea all the way to poor performance, dysphagia, aerophagia, and colic. Th

Two case reports of soft palate perforation as a complication of video laryngoscope-assisted tracheal intubation: mechanism analysis and preventative strategies.

BACKGROUND: Video laryngoscopes have gradually replaced direct laryngoscopes as an aid for tracheal intubation in various scenarios, increasing intubation success rates and facilitating difficult airway management. Unlike direct laryngoscopy, video laryngoscopy carries specific risks during the intubation process. CASE PRESENTATION: We report two cases of soft palate perforation, a rare oropharyngeal complication associated with video laryngoscopy. In the first case, video laryngoscopy was used for teaching endotracheal intubation during otolaryngology nasal bone fracture surgery, and soft pal

Anatomical analysis of the muscle layers of the soft palate in velopharyngeal closure: A cadaveric study.

BACKGROUND: In general, the levator veli palatini is essential for velopharyngeal closure as it elevates the velum. However, reconstructing the levator veli palatini alone does not fully restore velopharyngeal closure, suggesting that surrounding muscles-particularly the palatopharyngeus and superior constrictor-are also essential. We aimed to clarify the spatial relationships between the muscle bundles of the soft palate and pharynx macroscopically and histologically, particularly the palatopharyngeus, superior constrictor, and levator veli palatini. METHODS: We examined 15 heads from adult

Hairy Polyp Originating From the Lateral Nasal Wall of the Nasal Cavity.

Hairy polyps are benign lesions with limited growth potential. They usually occur in newborns, infants and young children, although occasional cases have been reported in adults, with a higher prevalence in females. Clinical symptoms include stridor, respiratory difficulty, snoring, and difficulty in oral intake. Histopathologically, hairy polyps consist of tissues derived from both mesodermal and ectodermal components. The nasopharynx and oropharynx are known to be the most common sites of origin, followed by less common sites such as the hard and soft palate, tonsils, tongue, auditory tube,
