Cleft Palate Repair Guided by Cleft Gap: An Institutional Protocol Using Modified Asymmetric Short Limb Z-plasty (MASLZ) and Straight-line Repair.

No single soft palate repair technique is universally optimal for cleft palate reconstruction; the ideal approach depends on cleft gap width and palatal configuration. Wide-gap clefts incur excessive tension with double opposing Z-plasty, whereas narrow-gap clefts may not require the complexity of intravelar veloplasty (IVV). We developed an institutional protocol selecting the repair technique based on intraoperative cleft gap measurement at the uvular neck: modified asymmetric small limb Z-plasty (MASLZ) for V-shaped gaps ≤ 6 mm, and straight-line repair (SLR) with overlapping IVV for gaps w

Effect of Semi-sitting Position on Airway Clearance Obtained by Jaw Thrust During Oral Fiberoptic-Guided Intubation.

BACKGROUND: Oral fiberoptic-guided intubation can be challenging in anesthetized patients due to loss of pharyngeal muscle tone, resulting in posterior displacement of the tongue, soft palate, and epiglottis. Maneuvers such as jaw thrust can improve patency, but their effectiveness may vary with patient position. Semi-sitting positioning has been shown to reduce upper airway collapsibility. This study compared airway clearance obtained by jaw thrust in the semi-sitting versus supine position during oral fiberoptic intubation. METHODS: This prospective randomized comparative study included adu

Nasopharyngeal Hairy Polyp Causing Pediatric Sleep-Disordered Breathing.

A rare congenital nasopharyngeal hairy polyp was incidentally identified during an attempted adenoidectomy in a 15-month-old female. The patient presented with snoring and intermittent apnea concerning for upper airway obstruction. Magnetic resonance imaging (MRI) revealed a 2.5-cm partially enhancing nasopharyngeal mass along the soft palate, likely originating near the left nasopharyngeal wall or Eustachian tube orifice. The lesion was excised transorally without complications. Histopathological analysis confirmed a benign hairy polyp composed of mature adipose tissue, skeletal muscle, and c

Foot-and-Mouth Disease Virus Persistence Divergence Within Serotypes and Vaccine Doses in Vaccinated Cattle.

Foot-and-mouth disease virus (FMDV) persistence in ruminants constitutes the potential risk of disease transmission. While vaccination effectively protects animals against clinical disease, more than 50% of vaccinated cattle can still develop persistent infection. Investigating the potential factors that influence the establishment of FMDV persistence in vaccinated cattle is critical for improving control and eradication strategies in epidemic regions. Here, cattle vaccinated with different doses of serotype O and serotype A vaccines were experimentally challenged with the homologous virus. Th

Lower cranial nerve responses after laryngeal stimulation in posterior fossa surgery: a study of medullary complex motor functions.

OBJECTIVE: Complex motor responses (CMRs) can be recorded from lower cranial nerves after laryngeal stimulation. Long latency responses (LLRs) are observed in the soft palate and tongue. This study aimed to describe these responses, evaluate their intraoperative reproducibility, and assess correlations between signal loss and postoperative outcomes. METHODS: The laryngeal adductor reflex (LAR) was elicited while LLRs were recorded bilaterally from the soft palate and tongue. Permanent signal loss was defined as an amplitude reduction greater than 50% affecting two LLRs. Postoperative outcomes

Clinical assessment and diagnostic features of fellatio-associated palatal lesions in forensic sexual abuse investigations: a systematic review.

OBJECTIVE: Sexual abuse is a global public health crisis impacting millions across all demographics. In forensic evaluations, relying only on anogenital injuries may create a diagnostic gap, as their absence does not exclude abuse. This highlights the importance of including a comprehensive intraoral assessment. Even though palatal lesions do not exclusively indicate sexual trauma, they remain a crucial yet frequently overlooked component when evaluating suspected oral sexual contact. Clinically, fellatio-associated trauma typically manifests at the junction of the hard and soft palate as eryt

Integrated Surgical Repair of an Isolated Congenital Palatal Fistula: Case Report and Brief Review.

Congenital palatal fistula is a rare embryologic anomaly, occurring in one percent of cleft cases, often causing hypernasality and nasal regurgitation. We report a 3-year-old male with an isolated fistula at the hard-soft palate junction. Repair was performed using a combined technique: a turnover hinge flap for the nasal layer and Z-plasty flaps for the oral layer. A fistula at this area is difficult to manage because of paucity of tissue. The two layer closure that we describe is a refinement of technique with practical applications.

Photobiomodulation Therapy for Symptom Management in Essential Palatal Myoclonus: Two Case Studies with Long-Term Follow-Up Two Cases: 7-Year/2-Month Follow-Up.

BACKGROUND: Palatal myoclonus (PM) is a rare movement disorder characterized by involuntary rhythmic contractions of the soft palate and adjacent musculature. Conventional therapies, including pharmacologic agents and botulinum toxin injections, frequently provide inconsistent or temporary relief and may be associated with adverse effects. This report evaluates photobiomodulation therapy (PBMT) as a noninvasive neuromodulatory treatment. CASES: The two cases were managed by different clinicians in separate clinical settings under standardized protocol guidance from the primary author, allowin

A rare case of severe hypertension following unilateral iatrogenic glossopharyngeal nerve injury during tonsillectomy.

Iatrogenic injuries of the nervus glossopharyngeus are usually seen during procedures such as tonsillectomy, adenectomy, carotid endarterectomy, and endotracheal intubation due to its close anatomical neighborhood. Damage to the nervus glossopharyngeus is characterized by specific and nonspecific symptoms such as sensory deficits, especially in the posterior 1/3 of the tongue and soft palate, sore throat, swallowing disorder, and decreased parotid gland secretion. The glossopharyngeal nerve has a close anatomical relationship with the palatine tonsil and is at risk during tonsillectomy. Thus,

Anatomy of the Soft Palate and Its Role in Upper Airway Function: A Narrative Review.

The soft palate is a complex anatomical structure, located at the posterior aspect of the oral and nasal cavities, able to dynamically create valve closure of the nasal and the oral cavities. Its precise movement is critical for speech, swallowing, and respiration. Despite its functional importance, detailed characterization of its constituent muscles-the levator veli palatini (LVP), palatopharyngeus (PP), tensor veli palatini (TVP), musculus uvulae (MU), and palatoglossus (PG)-as well as the palatine aponeurosis (PA) remains limited and inconsistent. This narrative review synthesizes findings

A large unilateral tonsillolith.

Tonsilloliths or tonsil stones are singular or multiple calculi found in tonsillar crypts that are located unilaterally or bilaterally. A review report determined the exact location of the tonsillolith, showing that in 69.7% of cases they are located in the tonsillar tissue, 21.2% in the tonsillar fossa, and 9% in the soft palate. Our research aims to document a case of unilateral tonsillolith that we removed under general anesthesia. His orthopantomography revealed a large radiopacity with respect to the anterior border of the left ramus and the angle area, measuring about 23.1 mm × 21.5 mm ×

Velopharyngeal Anatomy and Speech Production in Cleft Palate Surrounding Primary Palatoplasty: Protocol for a Prospective Observational Pilot Study.

BACKGROUND: Children with cleft palate often experience impaired speech due to atypical velopharyngeal anatomy following palate repair surgery. While surgical repair aims to restore the function of the palate, more than one-third of cases result in continued velopharyngeal insufficiency (VPI) and require reoperation, which can result in negative psychosocial impacts and financial burden. Common surgical approaches for primary palatoplasty, including intravelar veloplasty and double-opposing Z-plasty, have similar reported rates of VPI. Furthermore, procedure selection occurs without any presur

Sublingual Caruncle Taste Buds Drive Taste Perception and Salivary Reflexes.

Taste buds (TBs) are the fundamental units of taste perception and are distributed throughout the oral cavity, including the tongue, soft palate, epiglottis, pharynx, and retromolar pad. Despite their widespread presence, a complete and systematic understanding of TB distribution and functional organization remains incomplete. In this study, we focus on a small population of TBs located in the mandibular sublingual caruncle of mice, adjacent to the openings of the Wharton's ducts, a site previously noted in anatomical and physiological studies that has not been comprehensively characterized. T

Quercus persica lozenges versus placebo for reducing gag reflex severity in adult dental patients: a randomized double-blind controlled trial.

The gag reflex is a common challenge during dental procedures, often impairing clinical performance and causing patient discomfort. While pharmacological interventions are commonly employed, they can lead to adverse effects or be poorly tolerated. Herbal alternatives with local mucosal activity have gained increasing attention as potentially safer adjuncts. Quercus persica (Persian oak), rich in tannins, exhibits astringent and mucosal-desensitizing properties that may contribute to reducing gag reflex sensitivity. To evaluate the efficacy of a Quercus persica extract lozenge in reducing gag r

Dynamics of oropharyngeal structures during respiration with the volume-altered tongue base of minipigs.

OBJECTIVE: This study aimed to analyze the functional adaptations of tongue base volumetric enlargement and reduction on the kinematics of oropharyngeal structures during respiration in a minipig model. METHODOLOGY: Six same-sex sibling pairs of 8-to-9-month-old Yucatan minipigs were studied. Of each pair, one was diet-induced obese with a BMI>50 (obesity-associated volume enlargement) while the other was normal-weight and underwent partial ablation of the tongue base volume (volume-reduction). Real-time X-ray video fluoroscopic images (30 frames/s) were recorded under sedation, at baseline (

Robot-Assisted Transoral Trans-hard Palate Corridor to the Ventral Skull Base: A Cadaveric Feasibility Study.

BACKGROUND: Robot-assisted surgery offers high-precision, minimally invasive access to confined spaces. Although transoral approaches using soft-palate splitting can reach the sella and clivus, they may be associated with velopharyngeal dysfunction. To date, no robotic technique reportedly preserves the soft palate while providing continuous access from the anterior skull base to the craniovertebral junction. To evaluate, in a cadaveric model, a robotic transoral trans-hard-palate approach that creates a single midline corridor from corridor permits representative deep skull base maneuvers usi
