Metastasizing Pleomorphic Adenoma: A Systematic Review and Pooled Case-Report Analysis.

Background/Objectives: Metastasizing pleomorphic adenoma (MPA) is a rare salivary gland tumor characterized by benign pleomorphic adenoma morphology despite regional or distant metastatic behavior. Because most evidence derives from case reports and small series, this systematic review and pooled case-report analysis aimed to summarize published patient-level data and to explore clinicopathologic features, metastatic distribution, treatment, and survival without inferring causal treatment effects. Methods: PubMed, Scopus, Google Scholar, and reference lists were searched for English-language r

Advances in Intranasal CNS Targeting: Integrating Formulations, Devices, Computational Fluid Dynamics, and 3D Printing.

Nose-to-brain (N2B) delivery is a practical, non-invasive strategy for CNS targeting that can increase brain exposure while limiting systemic exposure. This review integrates three milestones in N2B delivery, formulations, devices, and quantitative evaluation strategies, to define design rules for effective olfactory/trigeminal deposition and enhance translational relevance. Formulations emphasize mucoadhesive systems, nanoparticle carriers (polymeric, lipid-based, and hybrid), nano-emulsions, and stimuli-responsive "smart" gels that prolong nasal residence. Regarding device advancements, the

The Furlow Double-opposing Z-Plasty: History, Technique, Modifications, and Outcomes.

Primary palatoplasty represents an important procedure in pediatric plastic surgery, with long-term implications for speech, feeding, and craniofacial development. Historical techniques often struggled to balance anatomic closure with functional restoration, resulting in high rates of velopharyngeal insufficiency. The Furlow double-opposing Z-plasty (DOZ) simultaneously lengthens the soft palate and reconstructs the levator muscular sling to address these challenges. This review examines the historical evolution of cleft palate repair and analyzes the Furlow DOZ in terms of its design, surgica

Definitive Radiation Therapy for Schneiderian Inverted Papilloma of the Soft Palate and Uvula.

Schneiderian inverted papilloma is a benign, locally aggressive tumor that typically arises in the respiratory mucosa of the nasal cavity and paranasal sinuses. These tumors are often asymptomatic but can cause unilateral nasal obstruction, discharge, hyposmia and epistaxis. They warrant attention particularly due to the potential for malignant transformation. They are treated most often with surgical excision. This case report describes a patient with Schneiderian inverted papilloma arising outside its usual location and managed with definitive radiation therapy instead of surgical excision d

Resolution of painful refractory soft palate mucoceles following post-surgical onset of satellite lesions: a rare three-year course.

Painful refractory mucoceles erupting along the soft palate are uncommon and management pathways are not well established. In general, oral mucoceles represent common non-neoplastic, non-infectious lesions of the minor/accessory salivary glands and are categorized histopathologically as mucus extravasation phenomena or mucus-retention cysts. We report a rare 3-year treatment-resistant course of painful soft palate mucocele clusters in an adult male causing significant impairment in quality of life, affecting speaking, swallowing, and psychological well-being. The case began as two isolated sof

An endotracheal tube inserted to the level of the hypopharynx is an easy, safe, and effective technique for ventilation of pediatric patients: A prospective observational study.

BACKGROUND: Difficult intubation and facemask ventilation in infants and small children can lead to rapid oxygen desaturation. An endotracheal tube (ETT) with its tip positioned in the hypopharynx is readily available and bypasses the soft palate, tongue, and epiglottis of the child, thus saving time by providing safe and efficient hypopharyngeal ventilation to ensure proper oxygenation. METHODS: This prospective, observational study included 85 infants and children aged >1 month and ≤ 6 years old, undergoing elective surgery under general anesthesia with orotracheal intubation. The effective

Persistent Oropharyngeal Hemangioma Causing Progressive Upper Airway Compromise: Diagnostic and Therapeutic Challenges.

Hemangiomas are rare benign vascular tumors of the oral cavity. Lesions involving the soft palate and oropharynx may cause significant functional impairment and, in advanced cases, life-threatening upper airway compromise. We report the case of a 15-year-old girl with a longstanding palatal mass first identified in early childhood who presented with progressive enlargement of the lesion, accompanied by dyspnea, odynophagia, and globus sensation. Physical examination and flexible nasolaryngoscopy revealed an extensive violaceous vascular lesion causing partial upper airway obstruction. Contrast

Perioperative management in primary palatoplasty-an international survey.

Orofacial clefts are common congenital defects. Primary palatoplasty aims to address the functional problems associated with the underlying deformity and is necessary for normal speech acquisition. International consensus on the perioperative management of primary cleft palate surgery is limited. A 34-question online survey was disseminated via email between June and September 2022, to identify differences and similarities in the perioperative management of children undergoing primary palatoplasty. Overall, 297 cleft surgeons were invited to take part, of whom 72 (24.2%) responded to the surve
