Three-Point Bedside Congestion Score (IJV Collapsibility + IVC Collapsibility + Central Venous Pressure) in Elective Mitral Valve Surgery: A Prospective Observational Study.

BACKGROUND: Systemic venous congestion and right-ventricular (RV) dysfunction are key drivers of postoperative hemodynamic instability, prolonged ventilation, and acute kidney injury (AKI) after mitral valve surgery. Comprehensive echocardiography is informative but may be impractical in high-throughput pre-anesthesia workflows. We evaluated a pragmatic three-point bedside congestion score combining internal jugular vein (IJV) collapsibility, inferior vena cava (IVC) collapsibility, and invasively measured central venous pressure (CVP) for preoperative risk stratification. METHODS: In this pr

Bentall Procedure in a Patient with Loeys-Dietz Syndrome: A Case Report.

Loeys-Dietz syndrome (LDS) is a rare autosomal dominant connective tissue disorder characterized by aggressive aortopathy, craniofacial anomalies and systemic involvement. Rapid aortic root dilatation and valvular dysfunction mandate an early surgical intervention. We report a case of 15-year-old boy with LDS, rapidly progressive aortic root dilatation and severe aortic regurgitation (AR) who underwent successful Bentall procedure. The case highlights the need of early detection, multidisciplinary involvement and meticulous anesthetic care.

Acromegaly as the Initial Presentation of Multiple Endocrine Neoplasia Type 1: A Case Report.

BACKGROUND Multiple endocrine neoplasia syndrome type 1 (MEN-1) is a rare autosomal-dominant disorder, caused by MEN-1 gene mutations, leading to tumors in multiple endocrine organs, classically the parathyroid glands, the anterior pituitary, and the pancreatic islet cells. Although primary hyperparathyroidism is usually the earliest manifestation, growth hormone-secreting pituitary adenomas rarely present as the initial clinical feature, potentially delaying recognition of the syndrome. CASE REPORT A 34-year-old woman presented with acromegalic features and hypercalcemia. Laboratory testing a

Association of pre-operative opioid use disorder and postoperative outcomes: a systematic review and meta-analysis.

INTRODUCTION: Opioid use disorder may increase the risk of adverse postoperative outcomes, but the magnitude and consistency of this association remain unclear. We performed a systematic review and meta-analysis to evaluate the impact of opioid use disorder on postoperative outcomes across surgical populations. METHODS: We searched electronic databases for studies comparing adults (aged ≥ 18 y) with and without opioid use disorder undergoing surgery. Primary outcomes were the incidence of any postoperative pain and composite adverse respiratory outcomes. Secondary outcomes included re-admissi

Postoperative Vasoactive Exposure Patterns and Outcomes After Non-Cardiac Surgery: A Prespecified Analysis of the SQUEEZE Cohort.

BACKGROUND: Postoperative vasoactive therapy is usually summarised by its presence or duration, although haemodynamic support evolves over time. We investigated whether reproducible postoperative vasoactive exposure patterns provide prognostic information beyond the duration of postoperative vasoactive therapy. METHODS: We performed a prespecified substudy of the international SQUEEZE cohort. Adults receiving postoperative vasoactive infusion within seven postoperative days after non-cardiac surgery were included. Unsupervised k-means clustering identified exposure patterns using duration of

Intelligent thermoregulation strategy versus routine thermal management on myocardial injury after noncardiac surgery during gastrointestinal tumor resection in elderly patients (SAGE): a multicenter, parallel-group, superiority trial.

BACKGROUND: Inadvertent perioperative hypothermia is associated with adverse cardiovascular outcomes, but adequately powered trials using high-sensitivity troponin endpoints is lacking. The SAGE trial evaluates whether an intelligent, closed-loop thermoregulation strategy reduces myocardial injury after noncardiac surgery (MINS) in high-risk elderly patients. METHODS: This prospective, multicenter, parallel-group, randomized controlled superiority trial conducted at seven tertiary hospitals in China. Participants aged ≥65 years scheduled for elective gastrointestinal tumor resection under gen

Prediction of Acute Kidney Injury with Venous Excess Ultrasound (VExUS) Scoring Versus Renal Resistive Index (RRI) in Patients Undergoing On-Pump Coronary Artery Bypass Grafting: A Prospective Observational Study.

BACKGROUND: The diagnosis of acute kidney injury (AKI) using conventional markers such as serum creatinine and urine output is often delayed, prompting interest in bedside ultrasound-based predictors. This study compared renal resistive index (RRI) and venous excess ultrasound (VExUS) scoring for early prediction of AKI after on-pump coronary artery bypass grafting (CABG). METHODS: In this prospective observational study, 180 adult patients were enrolled. RRI and VExUS assessments were performed preoperatively and within 3 hours of cardiothoracic ICU admission. AKI was diagnosed within 48 hou

Application of bedside ultrasound guidance in difficult diagnostic lumbar puncture in infectious diseases: Technical specifications, clinical evidence, and practical prospects.

Diagnostic lumbar puncture is a core procedure in the diagnosis and management of infectious diseases. However, in patients with complex anatomical conditions, such as obesity and spinal deformities, the traditional blind puncture method is often challenging to perform, resulting in a substantially higher failure rates and increased complication risks. Bedside ultrasound guidance provides a precise, visual solution to these challenges and represents an emerging tool for improving clinical practice. This narrative review aimed to comprehensively examine the clinical value of ultrasound-guided l

Association between tuberculosis, HIV infection, and diabetes mellitus: should we integrate screening and care? A systematic review.

Tuberculosis (TB) and HIV/AIDS are among the leading infectious diseases worldwide, while diabetes mellitus (DM) is a major chronic endocrine and metabolic disorder. Together, these conditions contribute substantially to the global burden of disease and mortality and are increasingly recognized as interconnected comorbidities. The combination of these three diseases can worsen patients' health conditions, increase mortality risk, and complicate treatment, which is why this study aims to summarize the relationship between HIV, TB, and DM. To achieve this, a systematic literature search was cond

GLP-1 Receptor Agonists and Insulin Versus Insulin for Perioperative Glycemic Control in Patients with Type 2 Diabetes Undergoing Cardiac Surgery: A Systematic Review.

Perioperative hyperglycemia is common in patients with type 2 diabetes undergoing cardiac surgery and is linked to adverse outcomes. Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) offer glucose-dependent insulinotropic effects and potential cardiac benefits, but their perioperative role remains uncertain. This review evaluated the efficacy and safety of GLP-1 RA therapy as an adjunct to insulin in this setting. Three electronic databases (PubMed, Cochrane, and Google Scholar) were searched for relevant articles up to October 2025. The primary endpoint was blood glucose levels during the

The Effect of Intranasal Dexmedetomidine on Hemodynamic Responses during Double-Lumen Tube Insertion: A Randomized Controlled Trial.

BACKGROUND: Thoracic surgeries require controlled lung collapse for optimal visualization, achieved through Double-Lumen Tube (DLT) intubation for one-lung ventilation (OLV). DLT insertion triggers significant sympathetic activation, potentially causing cardiovascular complications and cerebrovascular events. This study aimed to investigate the efficacy of intranasally administered Dexmedetomidine in attenuating hemodynamic perturbations associated with laryngoscopy and DLT placement. METHODS: This randomized controlled study was done in a tertiary care center in South India. Patients coming

[Stratified and graded diagnosis and treatment strategies for patients with primary aldosteronism].

Primary aldosteronism (PA) is the most common endocrine cause of secondary hypertension and carries a greater risk of cardiorenal complications than essential hypertension. Recent domestic and international guidelines and consensus statements have proposed screening all patients with hypertension for PA. To cope with the massive patient base resulting from universal screening, this article explores practical schemes for the stratified and graded diagnosis and treatment of PA. By employing "non-drug withdrawal" and "minimal-drug withdrawal" testing protocols, PA screening can be feasibly implem

Minimally Invasive Coronary Artery Bypass Grafting in Patients With Prior Myocardial Infarction: A Propensity Score-Matched Retrospective Study.

BACKGROUND: Patients with prior myocardial infarction (MI) represent a high-risk subgroup for coronary artery bypass grafting (CABG). The primary goal of CABG is durable myocardial revascularization to prevent recurrent ischemic events and improve survival. Minimally invasive CABG (MICS-CABG) may accelerate recovery, but evidence specifically in patients with a history of MI remains limited. METHODS: We performed a single-center retrospective cohort study of adults with prior MI who underwent isolated CABG from 2017 to 2024. Patients with new or recurrent MI within 3 months were excluded. Pat

The Nurse as an Early Warning System: A Longitudinal Surveillance Model in Pediatric Transplantation.

Posttransplant complications in pediatric patients, including rejection, infection, drug toxicity, and nonadherence, are still frequently identified at a late stage, when biomarkers have already changed and clinically meaningful harm to the graft or the patient may already be established. Current follow-up models remain overly dependent on laboratory tests, episodic visits, and reactive responses to overt deterioration, while underrecognizing subtle clinical signals perceived by nurses and caregivers over time. This viewpoint proposes the Nurse-as-Sensor Framework, in which nurses act as clini
