Clinical risk factors associated with the need for tracheostomy in traumatic cervical and high thoracic spinal cord injury.

BACKGROUND: Our objective was to assess the association of completeness and level of spinal cord injury (SCI) with the need for tracheostomy and identify additional risk factors predictive of tracheostomy. METHODS: This was a retrospective review of patients with SCI between January 2017 and December 2022. RESULTS: Patients with complete SCI were roughly thirty-three times more likely to have a tracheostomy when compared to incomplete injury (82 % vs 12 %, p < 0.001, OR = 32.9). The rate of tracheostomy did not differ between spinal cord levels for complete (p = 0.68) or incomplete (

Transformative Deep Vein Thrombosis Prophylaxis With Sequential Compression Devices in the Care of Hospitalized Patients.

Deep vein thrombosis (DVT) is a critical complication and concern in hospitalized patients due to its significant morbidity and mortality. Given the complex and multifaceted pathophysiology surrounding DVT formation, patients who have had surgical interventions faced acute or chronic trauma and prolonged immobility are at substantially high risk. Identifying these risk factors early is essential for early intervention and prophylaxis. Current standard-of-care prophylaxis for DVT includes pharmacological agents such as anticoagulants, and recently, there has been an increase in the use of mecha

Surgical Management of Acute Forearm Compartment Syndrome Due to Suspected Edematous Cutaneous Loxoscelism.

BACKGROUND Compartment syndrome of the forearm has been associated with a variety of etiologies, including fractures, snake bites, complications of certain infections, and, very rarely, spider bites. Loxoscelism is the venom-related clinical manifestation of the bite of spiders of the genus Loxosceles, also called brown or fiddler (violinist) spiders. It manifests locally/regionally with pain, erythema, and edema, with subsequent necrotic plaque formation at the site of the bite. This condition can threaten the function and integrity of the limbs and, in severe cases, can be life-threatening.

Navigating the Role of Surgery in Optimizing Patient Outcomes in Traumatic Brain Injuries (TBIs): A Comprehensive Review.

Traumatic brain injuries (TBIs) present with symptoms ranging from a mildly altered level of consciousness to irreversible coma and death. The most severe stage of TBIs is diffuse axonal injury and swelling affecting the whole brain. Management strategies are based on the classification of TBIs by severity and type and range from cognitive therapy sessions to complex surgeries. Neuroimaging modalities, predominantly magnetic resonance imaging, and the clinical Glasgow Coma Scale are principal indicators to diagnose and assess a patient's condition and neurological status and decide optimal tre

Time to Onset of Dysphagia Following Head and Neck Radiation.

To evaluate the time of onset of dysphagia in a cohort of head and neck cancer patients treated with radiation or chemoradiation. Retrospective chart review of adult patients. 237 patients met inclusion criteria for the study. The average age at cancer diagnosis was 62 years (± 12.6) in a predominantly male cohort (n = 198, 83.5%). The most common subsite was oropharyngeal (n = 146, 60.8%) and squamous cell carcinoma in origin (n = 232, 97.9%). Of head and neck cancer patients diagnosed with new onset dysphagia or a dysphagia related diagnosis, nine (3.8%) were diagnosed at six months to 1 yea

Non-foreign body-associated risk factors for complications associated with esophageal foreign-body removal and timing of endoscopic treatment: a single-center retrospective study.

BACKGROUND: Complications from prolonged esophageal foreign body impaction are well-documented, yet the significance of non-foreign body factors has not been thoroughly explored. This study aims to investigate non-foreign body risk factors for complications during esophageal foreign-body removal and to evaluate the impact of treatment timing. STUDY: We conducted a retrospective evaluation of patients diagnosed with esophageal foreign bodies requiring gastroscopic removal in our hospital between January 2019 and December 2020. Non-foreign body factors, such as whether endoscopic treatment was

Influence of Trauma Patients Aged ≥55 With PEC in Long Stay in the Hospital and Intensive Care Unit.

BackgroundTrauma and pre-existing conditions (PECs) can independently impact patient hospital length of stay (H-LOS) or intensive care unit (ICU) ICU-LOS. Pre-existing conditions impact on LOS has rarely been studied in older trauma patients aged ≥55. Our purpose is to examine the relationship between PEC status and ICU-LOS and H-LOS in this population.MethodsThis is a 3-year retrospective study, for calendar years 2020 through 2022. Multiple linear regression was used for analysis. Confounding factors were controlled for. Statistical significance was defined as P < 0.05.ResultsThere were 5168

From Prone to Prepared: Airway Management in a Patient With Penetrating Thoracic Trauma.

Perioperative and critical care management following penetrating thoracic trauma represents a complex challenge. Those who survive the early trauma approach and reach the hospital alive often remain in critical condition, with cardiocirculatory complications and major pulmonary injuries. Additional difficulty arises from the presence of a weapon in situ, particularly in a dorsal location, which limits patient positioning, and the safe manipulation of both the weapon and the patient. We present the case of a 47-year-old man, who suffered a stabbing assault, resulting in a deep dorsal thoracic w

Identifying key risk factors for acute compartment syndrome in tibial diaphysis fracture patients.

Acute compartment syndrome (ACS) is a severe orthopedic issue that, if left untreated, can result in lasting nerve and muscle damage or even necessitate amputation. The association between admission laboratory blood test indicators and the occurrence of ACS in patients with tibial diaphysis fractures is currently a subject of debate. The objective of this research was to identify the contributing factors for ACS in individuals suffering from tibial diaphysis fractures. In this retrospective study, we collected data on a total of 705 individuals from our hospital, comprising 86 ACS patients and

Characterizing trauma patients with delays in orthopedic process measures.

INTRODUCTION: Early operative intervention in orthopedic injuries is associated with decreased morbidity and mortality. Relevant process measures (e.g., femoral shaft fixation <24 hours) are used in trauma quality improvement programs to evaluate performance. Currently, there is no mechanism to account for patients who are unable to undergo surgical intervention (i.e., physiologically unstable). We characterized the factors associated with patients who did not meet these orthopedic process measures. METHODS: A retrospective cohort study of patients from 35 American College of Surgeons Committ

Ipsilateral femoral head and intertrochanteric fractures with posterior subluxation of the hip: A rare case report.

INTRODUCTION: Hip fractures are common in elderly patients, often accompanied by comorbidities. These fractures can be combined with other injuries, such as a femoral head, neck, or shaft dislocation. The cause of this complex injury is not well-established, but factors like high-energy trauma or falls from a certain height may contribute. CASE PRESENTATION: A 55-year-old man presented with an ipsilateral femoral head and left intertrochanteric fracture with posterior hip subluxation after a 3-m fall. This rare and complex injury was diagnosed using X-rays and CT scans. Due to the risk of com

A clinical study on the effect of axillary approach in the treatment of Ideberg type II scapular glenoid fractures.

BACKGROUND: Due to its deep position and complex surrounding anatomy, the scapular glenoid fracture was relatively difficult to deal with especially in cases of severe fracture displacement. Improper treatment may lead to failure of internal fixation and poor fracture reduction, severely affecting the function of the shoulder joint. Inferior scapular glenoid fracture was Ideberg type II fracture, and posterior approach was commonly used to deal with inferior scapular glenoid fracture. However, there are shortcomings of above surgical approach for inferior scapular glenoid fracture, such as ins

Management of urinary stones: state of the art and future perspectives by experts in stone disease.

AIM: To present state of the art on the management of urinary stones from a panel of globally recognized urolithiasis experts who met during the Experts in Stone Disease Congress in Valencia in January 2024. Options of treatment: The surgical treatment modalities of renal and ureteral stones are well defined by the guidelines of international societies, although for some index cases more alternative options are possible. For 1.5 cm renal stones, both m-PCNL and RIRS have proven to be valid treatment alternatives with comparable stone-free rates. The m-PCNL has proven to be more cost effective

Paediatric Hand Fractures - A Review.

Paediatric hand fractures account for approximately 25% of hand-related emergency department visits, making them the second most common childhood fractures after distal forearm fractures. These injuries are more prevalent in boys and peak ages between 9 and 14 years. Hand fractures in children are primarily caused by crushing injuries and, increasingly in older children, by sports-related activities. The little finger (52%) and thumb (23%) are the most frequently fractured digits, with phalangeal fractures, especially of the proximal phalanx, being common. Metacarpal fractures are more prevale

Spinal subdural haemorrhage secondary to strenuous exercise and warfarin, complicated by acute ischaemic stroke.

Spinal subdural haemorrhage or haematoma (SSDH) is a rare condition that is often overlooked and missed on initial presentation due to its non-specific features that may mimic other more common pathologies. It is associated with high morbidity and mortality rates, with few evidence-based management principles, particularly during the subacute stages of recovery. In this report, we detail a case of SSDH associated with exercise and anticoagulation therapy, which was complicated by acute ischaemic stroke. SSDH should be suspected in cases of acute back pain without a clear alternative cause, par

Undiagnosed vertebral fragility fractures in patients with distal radius fragility fractures: an opportunity for prevention of morbimortality in osteoporotic patients in developing countries.

UNLABELLED: Our study investigates vertebral fractures in individuals with distal radius fractures. Among 512 patients, 41.21% had vertebral fractures, predominantly in the lumbar spine. These findings highlight the importance of screening for vertebral fractures in this population, informing early intervention strategies to mitigate risks associated with osteoporosis. PURPOSE: This study's main goal was to look into the frequency, location, kind, and severity of asymptomatic vertebral fragility fractures (VFF) in people who had fractures of the fragility of the distal radius. Although VFF is

Age at First Fracture and Later Fracture Risk in Older Adults Undergoing Osteoporosis Assessment.

IMPORTANCE: Fragility fractures are often defined as those that occur after a certain age (eg, 40-50 years). Whether fractures occurring in early adulthood are equally associated with future fractures is unclear. OBJECTIVE: To examine whether the age at which a prior fracture occurred is associated with future fracture risk. DESIGN, SETTING, AND PARTICIPANTS: This observational, population-based cohort study included individuals from the Manitoba Bone Mineral Density Registry with a first bone mineral density (BMD) measurement between January 1, 1996, and March 31, 2018, with and without pri

L4-L5 Facet Dislocation Treated in a Delayed Fashion with Excellent Early Clinical Results.

INTRODUCTION: Introduction: Lumbar facet fracture-dislocations are rare injuries that are generated from a significant trauma. Literature regarding these injuries is limited to case reports, and there are even more limited reports concerning whether the delay of operative intervention in neurologically intact patients can achieve good clinical results if concomitant injuries and/or medical issues preclude urgent operative intervention. There has been no consensus on which operative techniques are effective in achieving an anatomic reduction of these injuries. OBJECTIVES: A case report of an L
