Angiographic perfusion outperforms large artery vasospasm for predicting the impact of rescue therapy in subarachnoid hemorrhage.

Aneurysmal subarachnoid hemorrhage (aSAH) contributes disproportionately to stroke-related disability due to its prevalence in younger patients. Large vessel vasospasm complicates aSAH and is often treated with endovascular rescue therapy (ERT). However, clinical trials have not demonstrated a clear link between vasospasm improvement and better outcomes. We hypothesized that improving vessel caliber alone may not ensure better blood flow to brain parenchyma, and we studied how vessel caliber relates to perfusion in the context of ERT. We measured the internal carotid artery (ICA) caliber and t

Time against tissue: case reports of fungal necrotizing fasciitis and the cost of delayed aggressive interventions.

BACKGROUND: Necrotizing fasciitis is a rare disease that causes the necrosis of subcutaneous tissues and muscle fascia in patients, and if not diagnosed and treated in due time may lead to the extreme morbidity and even death, with the mortality rate ranging from 6 to 33%. Fungal necrotizing fasciitis is even more rare and life threatening because of aggressive infection and less response to medical treatment. Literature has reported three times higher mortality rate in patients, who had positive fungal cultures and requiring multiple surgical detriments. CASE PRESENTATIONS: We are reporting c

Therapeutic Dilemmas in the Management of Stroke Among Patients With Infective Endocarditis (IE): A Literature Review.

Infective endocarditis (IE) is an infection of the endocardial lining of the heart, with high morbidity and mortality. Neurological complications, mainly stroke, represent the most frequent and devastating extracardiac manifestations. Despite continued advancements in diagnostics and management, the optimal strategies and guidelines to prevent and treat IE-associated stroke remain controversial and scarce. We conducted a detailed narrative review of recent literature focusing on the epidemiology, risk factors, pathophysiology, diagnostic approaches, and therapeutic strategies for stroke in the

PERi-operative Selective Decontamination of the digestive tract to prevent severe infectious complications after Esophageal Resection: study protocol of the PERSuaDER-trial, a multicenter randomized controlled trial in patients with primary resectable esophageal carcinoma.

BACKGROUND: Esophagectomy is a highly complex surgical procedure, and it is associated with significant morbidity and mortality. The postoperative complication rate is high, with pneumonia being the most common. These are thought to arise from (micro-)aspiration of bacteria residing in the oropharyngeal and gastrointestinal (GI) tract. Selective decontamination of the digestive tract (SDD) is a prophylactic antibiotic strategy aimed at preventing these infections. SDD decolonizes the oropharyngeal and GI tract from pathogenic aerobic gram-negative rods, fungi and yeasts, while anaerobic, prote

SGLT2 inhibitor or metformin as standard treatment in early-stage type 2 diabetes? Baseline data in SMARTEST, a novel, decentralised, register-based randomised trial on prevention of diabetic complications.

AIMS: Metformin has hitherto not been proven superior to other type 2 diabetes (T2D) medications for the prevention of organ complications. The aim of this study is to report baseline data and blinded interim analyses in the register-based randomised clinical trial (RRCT) SMARTEST, which compares metformin and the SGLT2 inhibitor dapagliflozin in early T2D. We also present learnings from the novel decentralised methodology of this first RRCT in diabetes care. MATERIALS AND METHODS: Participants with T2D since <4 years and without major cardiovascular or renal disease were included at 36 centr

Gangrenous Cholecystitis in a 54-Year-Old Male With Schizophrenia and Other Barriers to Care: A Case Report.

Schizophrenia is a chronic psychiatric condition that can contribute to delays in the diagnosis and treatment of health conditions. Factors like poor judgment, decreased medical literacy, cognitive barriers, and delayed seeking of therapy can result in worse outcomes of the disease. Acute gangrenous cholecystitis is a severe complication of acute cholecystitis, requiring immediate surgical intervention. We present the case of a 54-year-old Spanish-speaking male with schizophrenia who presented with seven days of progressively worsening right upper quadrant (RUQ) pain, as well as associated nau

Single-incision laparoscopic endorectal pull-through for Hirschsprung disease: a prospective institutional study and systematic review with meta-analysis.

BACKGROUND: Single-incision laparoscopic endorectal pull-through (SILEP) has emerged as an alternative to conventional laparoscopic surgery for Hirschsprung disease (HSCR), offering improved cosmetic outcomes with comparable efficacy. However, its adoption remains limited, and evidence supporting its long-term safety and functional outcomes is sparse. This study presents our institutional experience with SILEP, alongside a systematic review and meta-analysis evaluating perioperative outcomes, postoperative complications, bowel function, and cosmesis. Influence of Multiple Prior Endoscopic Sinus Surgeries on Dupilumab Efficacy Including Smell Function in the DUPIREAL Population.

INTRODUCTION: Chronic rhinosinusitis with nasal polyps (CRSwNP) is a persistent inflammatory condition that significantly reduces the quality of life due to nasal obstruction, loss of smell and rhinorrhea. Although endoscopic sinus surgery offers relief of symptoms and improvement of quality of life, recurrences are common and multiple surgeries are often required. The aim of this post hoc analysis is to verify the hypothesis that previous multiple surgeries may be associated with poor outcomes with dupilumab in a real-life setting in terms of quality of life, reduction in nasal polyps and imp

Comparative assessment of endoscopic and microsurgery resection for intracranial ventricular tumors: a meta-analysis of 3059 patients.

BACKGROUND AND AIM: Surgical resection of ventricular brain tumors is traditionally performed via microsurgery, which requires open craniotomy and can result in significant complications. Endoscopic approaches have emerged as minimally invasive alternatives. There is a lack of consensus regarding the preferred surgical strategy for ventricular tumor resection; the technique with the optimal rates of recurrence, mortality, and complications is debatable. OBJECTIVES: This article aims to evaluate the efficacy of endoscopic resection in comparison to microsurgical resection, focusing on identify
