Genetic developmental and epileptic encephalopathies in adulthood: Gaps in evidence and care.

Developmental and epileptic encephalopathies (DEEs) are a heterogeneous group of the most severe epilepsies, characterized by seizure onset and neurodevelopmental delay in infancy or childhood, often with additional neurological and systemic comorbidities. Up to 50% of cases have an identifiable genetic cause, with structural etiologies representing a small minority. Although associated with early mortality, many individuals now live into adulthood. Over the past 15 years, advances in genetic diagnosis and understanding of disease mechanisms have created realistic prospects of precision therap

Competitive spreading dynamics under sentinel intervention in higher-order community networks.

Misinformation spreading on social networks is jointly shaped by community structure, higher-order reinforcement, and intervention. We formulate a sentinel-triggered competitive susceptible-infected-susceptible-AB model on a simplicial complex with two equal-sized communities. Sentinels respond to local misinformation exposure by temporarily becoming truth sources, coupling local detection and corrective response with higher-order reinforcement and cross-community invasion. We derive a heterogeneous mean-field description and compare its predictions with Monte Carlo simulations. The results re

A multiplicity of neolemnane-type sesquiterpenoids from the soft coral Lemnalia sp.

Terpenoids are a large class of structurally diverse secondary metabolites that have attracted the attention of chemists and pharmacologists due to their unique skeletal structures and wide range of biological activities. Octocorals are major contributors to the diversity of terpenoids in the ocean, and soft corals are one of the representative species of octocorals. An investigation into the chemical composition of a soft coral identified as Lemnalia sp., yielded six previously undescribed neolemnane-type sesquiterpenoids (1-6) and twelve known ones (7-18). The elucidation of their structures

Curriculum-Aligned Conversational AI to Support Pathology Self-Directed Learning in Preclinical Medical Students: A Crossover Randomized Pilot Study.

BACKGROUND: Artificial intelligence (AI) is reshaping the landscape of medical education, yet the widespread adoption of general-purpose AI tools in student learning raises substantial concerns about curricular alignment, information accuracy, and the absence of pedagogical scaffolding. OBJECTIVE: This crossover pilot study evaluated a curriculum-aligned custom GPT that was developed to support AI-scaffolded self-directed learning (SDL), preparation that remains student-initiated but is supported by structured Socratic prompting, compared with fully unguided traditional preparation in second-

Discovery of benzimidazole derivatives as LDHA inhibitors with antitumor activity.

LDHA is aberrantly overexpressed in various malignancies and drives enhanced glycolytic flux. Although LDHA inhibitors have shown promising antitumor activity in preclinical studies, no such targeted antitumor drugs have been approved for clinical use at present. In this study, we designed and synthesized a series of benzimidazole derivatives bearing benzoate substituents. We evaluated their antitumor activity in vitro and in vivo, along with their LDHA inhibitory potency. Compounds 8h and 8d emerged as the primary candidates. Both exhibited LDHA inhibition activity. They inhibited proliferati

From social isolation to frailty: the chain mediating roles of depression and cognitive decline among Chinese older adults.

BACKGROUND: As global aging accelerates, frailty has emerged as a critical public health challenge. Although social isolation is recognized as a prevalent psychosocial risk factor among older adults, the longitudinal mechanisms linking it to frailty-specifically via the chain mediating pathways of depression and cognitive decline-remain underexplored. OBJECTIVE: This study aimed to investigate the presence and magnitude of the "social isolation → depression → cognitive function → frailty" chain mediation pathway in the Chinese elderly population. METHODS: Using four waves of data (2011-2018)

Biased 5-HT1A agonist NLX-101 improves executive function under NMDA receptor hypofunction and preferentially enhances cortical ERK2 phosphorylation.

Cognitive inflexibility and working memory deficits are major contributors to functional impairment in neuropsychiatric disorders and remain insufficiently addressed by current treatments. NLX-101, a preferential postsynaptic 5-HT1A receptor biased agonist, engages cortical serotonergic signaling and ERK1/2-related pathways, offering a mechanistically distinct approach to improving prefrontal cortex-dependent cognition. Here, we examined whether NLX-101 counteracts executive function deficits induced by NMDA receptor blockade in BALB/c mice. Working memory was assessed using the 8-arm radial m

Gamma-distributed transmission memory in a reaction-diffusion epidemic model with nonlinear incidence.

We extend the delayed reaction-diffusion SI model with nonlinear incidence proposed by Song et al. by replacing the fixed transmission lag with weak and strong gamma-distributed memory kernels. Because the model has no exposed compartment, the memory term is interpreted phenomenologically as a distributed timing of transmission-related effects of past infectious states rather than as an explicit latent-stage mechanism. Using the same parameter set adopted by Song et al., we show that the distributed-memory models have a stability structure that is qualitatively different from that of the discr

Health outcomes and design elements of dementia-friendly gardens: A narrative review.

As the global population ages, the prevalence of older adults with dementia continues to rise, and non-pharmacological interventions for this population are receiving increasing attention. The existing literature suggests that garden environments may influence patients' well-being and health outcomes through various mechanisms, but their actual effectiveness depends on appropriate design and use. This narrative review aims to systematically synthesize the published evidence, focusing on the potential health benefits of gardens for older adults with dementia, and to identify garden design eleme

Influence of pea protein on structural modulation of tapioca starch emulsion gels: Vitamin E release behavior and potential as a dysphagia-friendly food matrix.

This study investigated the influence of pea protein (PP) on the structural modulation and vitamin E (VE) release behavior of tapioca starch (TS)-based emulsion gels and evaluated their potential as a dysphagia-friendly food matrix. TS/PP-based emulsion gels were prepared with different PP concentrations. FE-SEM and CLSM suggested a more compact gel network and finer oil-phase dispersion at higher PP concentrations, and FT-IR and XRD indicated structural changes and possible TS-PP interactions, potentially involving hydrogen bonding. The encapsulation efficiency of VE increased from 80.0% to 9

Antibacterial and immunomodulatory core-shell nanofibrous membranes accelerate skin wound repair and regeneration.

Complex skin wounds are complicated by infection, inflammation, and delayed regeneration, necessitating wound dressings with antibacterial, immunomodulatory, and regenerative properties. Here, we developed a coaxial core-shell nanofibrous membrane that integrates structural and bioactive components within a multifunctional dressing. The membrane comprised poly(lactic acid) (PLA) reinforced with nano-hydroxyapatite (nHA) as the structural core and poly(vinylidene fluoride-co-hexafluoropropylene) (PVDF-HFP) as the bioactive-loaded outer shell. Formulations included: PLA/PVDF-HFP (PPN0B0), PLA-3%

Intravenous administration of a PHP.eB vector overexpressing neuronal Caveolin-1 delays motor neuron and neuromuscular degeneration in a TDP-43 mouse model of ALS.

TDP-43 proteinopathy is observed in over 95% of ALS cases, yet effective disease-modifying therapies remain elusive. We previously showed that neuron-specific overexpression of caveolin-1 via SynCav1 extended survival and delayed neurodegeneration in an SOD1 mouse model of ALS. Here, we further assessed the effect of systemic delivery of AAV-PHP.eB-SynCav1 in TDP-43A315T mice. At 24 weeks, SynCav1-treated mice showed preserved motor neuron (MN) morphology, maintained neuromuscular junction (NMJ) innervation, and normalized pathologically driven neurotrophic transcription in the spinal cord. Integrated evidence-based assessment of in vitro and in vivo data supports that the effects of phenmedipham in Xenopus laevis are consistent with generalised toxicity rather than thyroid disruption.

Phenmedipham (PMP), an herbicide registered for various crops across different geographies, is under re-approval in Europe and should be evaluated for potential endocrine-disrupting properties according to the current legislation on pesticide active substances. This paper describes an evidence-based assessment of potential thyroid-mediated effects of PMP in Xenopus laevis as model species for thyroid modality. As an indication of thyroid activity, rat thyroperoxidase inhibition was reported in a single in vitro assay in the US-EPA ToxCast database. The results of a 27-day Extended Amphibian Me

Optimal Scheduling of Dengue Vector Control.

Dengue transmission is shaped by the population dynamics of the Aedes aegypti mosquito, making vector control a central strategy for disease mitigation. The impact of interventions such as larvicide, adulticide, and breeding-site reduction depends critically on their timing under fluctuating environmental conditions. We build on a high-fidelity, non-Markovian mechanistic model of the Aedes life cycle that captures stage-structured, temperature-dependent developmental delays, and mortality, and extend it to incorporate multiple vector control measures. Rather than using continuous abstract cont

Health Policy for Obesity and Metabolic Syndrome in Asian Health Systems: Three Design Principles, Three Binding Constraints, and the Governance That Decides between Them.

BACKGROUND: Obesity drives metabolic syndrome and cardiometabolic disease in Asia, where complications develop at lower levels of adiposity than in Western populations. We reviewed Asian obesity policies to identify design features that support their durability and how access to incretin-based medications fits within these policies. METHODS: This narrative review used a structured search of PubMed/MEDLINE and Embase (January 2000 to March 2025) supplemented by grey literature from public health agencies. Of 1,884 records, 612 underwent full-text review and 236 met inclusion criteria. Two revi

Obesity, Weight Loss, and Facial Plastic Surgery.

Obesity is an increasing problem in today's patient population. Obesity creates well-documented physiologic effects on local tissues, including proinflammatory states, impaired microvascular circulation, and changes to the extracellular matrix. These cellular level changes lead to the potential for impaired wound healing. Increased adipose tissue leads to changes in the facial structure with predominantly increased thickness of the superficial fat, and inconsistent findings on deeper fat pad volumes. The rising popularity of injectable weight loss drugs has created new challenges for the facia
