Quantitative total serum IgG4 reporting in human paragonimiasis: a scoping review.

BACKGROUND: Paragonimiasis is a food-borne parasitic disease caused by lung flukes belonging to the genus Paragonimus. It can produce pulmonary, pleural, and other organ manifestations that overlap clinically, radiologically, and histopathologically with malignancy, tuberculosis, and immune-mediated disorders. Elevated total serum immunoglobulin G4 (IgG4) concentrations and IgG4-positive plasma cell infiltration may mimic IgG4-related disease (IgG4-RD), but quantitative serum IgG4 levels in paragonimiasis have not been systematically characterized. METHODS: We conducted a multi-database scopi

Microbiologically Confirmed Concomitant Tuberculous and Pneumococcal Meningitis in an Adult: A Case Report.

Concurrent infection of the central nervous system by Mycobacterium tuberculosis and Streptococcus pneumoniae is rare and poses substantial diagnostic and therapeutic challenges due to the overlap of clinical symptoms and cerebrospinal fluid (CSF) findings. A 45-year-old male presented with complaints of fever, headache, vomiting, generalized seizures, and altered sensorium. Magnetic resonance imaging of the brain was normal, and CSF analysis showed lymphocytic pleocytosis, protein elevation, and elevated levels of adenosine deaminase. Spectrum of Neuroimaging Findings in CNS Lymphoproliferative Disorders.

Central nervous system (CNS) lymphoproliferative disorders comprise a heterogeneous group of rare entities that present major diagnostic and therapeutic challenges. Primary CNS large B-cell lymphoma (LBCL) is the most common form and the second most frequent primary CNS tumor after gliomas. The 5th edition of the World Health Organization Classification of Hematolymphoid Tumours introduced updated terminology, including primary LBCL of immune-privileged sites and lymphomas arising in immunodeficiency/dysregulation, along with less common variants such as intravascular large B-cell lymphoma, ly

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Recent advances in nanomedicine for the diagnosis and therapy of thyroid disorders.

INTRODUCTION: Inappropriate antibiotic prescribing drives antimicrobial resistance. Although the Swiss Society of Infectious Diseases has introduced national guidelines for common infectious diseases starting from 2019, it remains unclear whether family physicians and paediatricians adhere to them and what factors influence their prescriptions. The aims of this study were to assess whether Swiss family physicians and paediatricians make appropriate antibiotic choices in accordance with national guidelines and to identify physician- and patient-related factors associated with the prescribing of

Schaumann bodies deposited along myenteric plexus of the muscularis propria is a unique histopathological feature of Crohn's disease.

AIMS: Schaumann bodies were first identified in sarcoidosis by Dr Schaumann in 1941. They were also detected in 10% of Crohn's disease (CD) cases in a study involving patients with surgically resected CD. However, the characteristics and significance of Schaumann bodies in CD have yet to be fully elucidated. This study aimed to determine the pathological features and diagnostic significance of Schaumann bodies in various bowel diseases. METHODS: Overall, 278 bowel specimens were collected from patients with CD, intestinal tuberculosis, ulcerative colitis, intestinal schistosomiasis, diverticu

Universal driving restrictions beyond 4 weeks appear unnecessary following CAR-T therapy in multiple myeloma.

INTRODUCTION: Tuberculosis (TB) is a major global cause of morbidity and mortality. TB meningitis (TBM) is an implication of systemic dissemination of a primary TB infection that indicates a poorer disease prognosis with various long-term neurological sequelae. Consequently, it is crucial to understand the different clinical presentations and manifestations of such condition. In TBM, vision loss, which is one of the most devastating complications, may result from optic nerve inflammation and atrophy. Although unilateral optic neuritis as a primary presenting symptom of TBM has been broadly rep

Distinct features of ANCA-associated hypertrophic pachymeningitis compared with other etiologies: findings from a study of 74 patients.

OBJECTIVES: We aimed to characterize the clinical and radiological features, and outcomes, of a large cohort of hypertrophic pachymeningitis (HP) patients from a single centre. METHODS: We conducted a retrospective study at a tertiary referral centre, encompassing patients diagnosed with HP between 2003 and 2022. The diagnosis of HP relied on the identification of thickening of the dura mater via MRI of the brain or spine. RESULTS: We included 74 patients, with a mean age of 43.6 ± 14.2 years, of whom 37 (50%) were male. Among the 74, 32 (43.2%) had an immune-mediated origin, including 21 wi

Serum microRNA Biomarker Expression in HIV and TB: A Concise Overview.

Non-coding RNAs (ncRNAs), specifically MicroRNAs or miRNAs, are now understood to be essential regulators in the complex field of gene expression. By selectively binding to certain mRNA targets, these tiny RNA molecules control the expression of genes, leading to mRNA degradation or translational repression. The discovery of miRNAs has significantly advanced biomedical research, particularly in elucidating the molecular mechanisms underlying various diseases and exploring innovative therapeutic approaches. Recent progress in miRNA research has provided insights into their biogenesis, functiona

Optimizing Pyrazinamide Use: A Low-Hanging Fruit in Improving Outcomes with Tuberculous Meningitis? Narrative Review.

Tuberculous meningitis (TBM) disables more than a third of its sufferers. Recent research has focused on optimizing the antitubercular regimen, mainly by increasing the dosage of rifampicin. However, pyrazinamide, with higher penetration into the central nervous system, is generally overlooked. We discuss the potential clinical impact of using pyrazinamide throughout antitubercular therapy in TBM, in contrast to only the intensive phase. This approach may improve the treatment outcomes and reduce disability in TBM. We summarize the available data regarding this approach from in vitro studies,

Unveiling Pediatric Neurosarcoidosis Mimicking Central Nervous System Tuberculosis: Diagnostic Challenges.

Neurosarcoidosis is a rare chronic inflammatory disease affecting the nervous system. Owing to its varying manifestations that can mimic other central nervous system infectious or autoimmune diseases, and scarcity of literature, it proves to be a challenging diagnosis. We report two cases of possible neurosarcoidosis in the pediatric age group. Our first patient presented to us with seizures at the age of 13 years, whereas our second patient presented with headaches and vomiting at the age of 10 years. Both patients had elevated cerebrospinal fluid protein levels and leptomeningeal enhancement

Neurological Complications of Cupping Therapy: A Comprehensive Review.

BACKGROUND: Cupping therapy, a traditional treatment method, has been shown to be effective in various studies. However, there have been reports of significant neurological complications following cupping therapy. This comprehensive review aimed to investigate the important and potentially severe neurological complications documented in the literature. SUMMARY: The review analyzed case reports and case studies published in three major databases (PubMed, Google Scholar, and Cochrane) up to June 2024. The search strategy involved a three-step process, where two traditional medicine experts inde

Pharmacokinetic and Pharmacodynamics of Clofazimine Nano-in-Microparticles: Enhanced Brain Delivery and CNS Tuberculosis Amelioration via Intranasal Administration.

Mycobacterium tuberculosis (Mtb) demonstrates a proclivity for infecting extrapulmonary sites, notably the brain. Treating these extrapulmonary tuberculosis (TB) manifestations is challenging due to the difficulty of drug delivery across the blood-brain barrier. Clofazimine (CLF) has exhibited promising activity against Mtb, including multidrug-resistant variants, in vitro and in preclinical animal models. However, its clinical implication is restricted owing to poor physicochemical and pharmacokinetic properties. This study aims to develop CLF nano-in-microparticles (CLF-NIMs) for brain drug

Ovarian Actinomycosis Mimicking Ovarian Malignancy: A Case Report.

Actinomyces spp. are non-spore-forming anaerobic bacteria that can be part of the normal flora of human oral, intestinal, and urogenital tracts. Mucosal disruption can lead to an infection characterized by granulomatous inflammation leading to abscess formation and sinus tracts classically draining pus with sulfur granules. Most actinomycosis cases are polymicrobial, involving various aerobic and non-aerobic bacteria. Cervicofacial actinomycosis is by far the most common, accounting for more than half of its presentations; however, cases of thoracic, abdominal, pelvic, and central nervous syst

A Rare Case of a Solitary Central Nervous System Tuberculoma Mimicking an Intracranial Tumor.

Tuberculosis (TB) can sometimes involve the central nervous system (CNS), especially among immunocompromised patients, but it is an infrequent manifestation. Among immunocompetent individuals, CNS TB has only scarcely been described in the literature; when it is manifested as a solitary lesion (tuberculoma), it can mimic other CNS tumors, appearing with clinical manifestations of increased intracranial pressure, such as headaches and vomiting. In this manuscript, we describe a case of a CNS tuberculoma, which presented as a cerebellum tumor with signs of increased intracranial pressure in an i

Pontine tuberculoma presenting as a true-localising abducens palsy.

An immunocompetent woman in her early 30s presented with a 3-day history of nausea, vomiting and horizontal diplopia. Examination revealed left-sided abducens paresis, with normal visual acuity, pupillary reactions and fundus. There was no papilloedema, neck rigidity or positive neurological signs. Magnetic resonance imaging (MRI) showed a central pontine T2 hypointense ring-enhancing lesion causing compression of the fourth ventricle. Magnetic resonance spectroscopy showed a lipid lactate peak leading to a diagnosis of pontine tuberculoma. The diagnosis was supported by a positive Mantoux and

A Case of Tuberculous Meningitis With Concomitant Spinal Co-infection With Tuberculosis and Aspergillosis.

Co-infection with Mycobacterium tuberculosis and Aspergillus in an immunocompetent host is rare but can occur. In this case, we present a patient with central nervous system tuberculosis (TB) and biopsy-proven spinal co-infections with TB and Aspergillosis. We highlight the complexities of treating a TB-Aspergillosis co-infection given drug-drug interactions between standard therapy for both conditions. Using susceptibilities for the Aspergillus and carefully monitoring drug levels of the antifungal agents, we were able to optimally treat the co-infection. This patient was ultimately discharge
