Trends in mortality from tuberculosis in England and Wales: effect of age on deaths from non-respiratory disease.

BACKGROUND: Although mortality from tuberculosis has continued to fall in recent years, there has been little change in the case fatality rate for tuberculosis over the same period. This has previously been shown to be due to the increasing proportion of cases of tuberculosis occurring in the elderly. Tuberculosis mortality and case fatality were therefore analysed to determine if this disappointing trend in case fatality rate has occurred from disease in all or only certain sites. METHODS: A retrospective analysis of the tuberculosis mortality and case fatality rates in England and Wales for

Headache associated with aseptic meningitis.

A retrospective analysis of all patients admitted with the diagnostic codes of aseptic or viral meningitis was performed at two institutions over 3 years. Forty-one patients with cerebrospinal fluid confirmation of aseptic meningitis (increased protein; increased white count; negative gram stain; and negative fungal, tuberculosis, and bacterial cultures) were analyzed. All the patients had headache, which was typically severe and bilateral in 39 of the 41 patients. The headache was of abrupt onset or the worst of the patient's life in 24 of the patients. The quality of the headache, when descr

Intracranial tuberculoma: comparison of MR with pathologic findings.

PURPOSE: To compare the MR signal intensity patterns and enhancement pattern of intracranial tuberculomas with their histopathologic features. METHODS: MR images of six patients with surgically proved intracranial tuberculoma were reviewed retrospectively and were compared with histologic findings of the resected specimen. Detailed histologic examination was performed to look for the extent and characteristics of caseation necrosis, fibrosis, and inflammatory cellular infiltrates at each area of different signal intensities and at the enhancing areas on MR. Signal intensities for T1- and T2-w

Leptomeningeal and brain biopsy in chronic meningitis.

BACKGROUND: The cause is not found in one third of patients presenting with chronic meningitis. Biopsy of the leptomeninges and brain is often recommended in these patients, but the value of a biopsy is uncertain. AIMS: To review the results of leptomeningeal and brain biopsies and their effect on diagnosis and management in patients with chronic meningitis, if the diagnosis was uncertain after clinical assessment, lumbar puncture and radiological investigations. METHODS: The clinical features, investigations and the results of leptomeningeal and brain biopsies were reviewed retrospectively

The use of adenosine deaminase activity as a biochemical marker for the diagnosis of tuberculous meningitis.

The diagnostic value of adenosine deaminase (ADA) activity was studied to evaluate its use in the differential diagnosis of tuberculous meningitis in the local setting. Cerebrospinal fluid (CSF) from 119 patients with meningitis and other conditions with central nervous system symptoms were collected and ADA activity determined by the colorimetric method of Guisti read at 628 nm. The CSF was also subjected to other laboratory examinations so as to provide the aetiological diagnosis. All 14 tuberculous meningitis patients had ADA activity greater than the cut off value of 9.0 IU/L. High ADA act

[Tuberculosis of the CNS--once again a current neurosurgical disease].

Tuberculosis is relative uncommon in developed countries. The incidence is still considerable in India, in the Middle East and South America. Neurosurgically important manifestations of tuberculosis are intracranial tuberculomas, tuberculous meningitis with hydrocephalus and tuberculous spondylitis. Therefore experiences in the management of these three kinds of neurotuberculosis are described and an overview of the literature is given. The diagnosis of intracranial tuberculomas is difficult because the symptoms are similar to those of many other types of intracranial tumor and the CT-findings

Central nervous system tuberculosis: MRI.

The MRI findings of 18 proven cases of central nervous system (CNS) tuberculosis were reviewed; 10 patients were seropositive for HIV. All had medical, laboratory, or surgical proof of CNS tuberculosis. Eleven patients had meningitis, of whom two also had arachnoiditis. Five patients had focal intra-axial tuberculomas: four brain masses and one an intramedullary spinal lesion. Two patients had focal extra-axial tuberculomas: one in the pontine cistern, and one in the spine. In all 11 patients with meningitis MRI showed diffuse, thick, meningeal enhancement. All intraparenchymal tuberculomas sh

[A case of intracerebral tuberculoma: clinical characteristics and MRI findings].

A case of intracerebral tuberculoma treated surgically was reported. A 47-year-old man was admitted to our hospital because of progressive left hemiaparesis over the previous 5 months. A computerized tomography scan showed a well enhanced mass associated with a marked perifocal edema. T1 weighted magnetic resonance imaging (MRI) revealed an isosignal ring around the heterogenous low intensity mass. A low intensity area just interior to this ring was also visualized both in T1 and T2 weighted images. Although the clinical course was unusually long, this was diagnosed as a metastatic brain tumor

Tuberculous radiculomyelitis (arachnoiditis): myelographic (and CT myelographic) appearances.

Tuberculous radiculomyelitis (arachnoiditis) remains one of the important causes of paraplegia in India. The diagnosis usually rests on clinical history and examination, and on laboratory findings in the cerebro-spinal fluid (CSF). Few descriptive reports are available of the myelographic appearance, with water-soluble contrast media, in tuberculous radioculomyelitis (arachnoiditis). A retrospective review of 21 myelograms and 10 computed tomographic (CT) myelograms, in 14 patients with tuberculous radiculomyelitis, was carried out, with a view to describing, in detail, the radiographic featur

Intracranial tuberculoma: clinical and computerized tomographic findings.

Seven cases of intracranial tuberculoma diagnosed in northern Turkey over a 2-year period are presented, with emphasis on the CT and clinical findings. CT alone cannot differentiate tuberculoma from an abscess or other granulomatous processes, but it is still one of the most sensitive techniques for detecting tuberculomas. It provides information on the nature of the lesion and the number of loci.

[An investigation on risk factors relating to the treatment difficulty in originally treated pulmonary tuberculosis cases].

In order to investigate whether so-called risk factors relating to treatment difficulty are true risk or not, treatment results of 520 in-patients originally treated for pulmonary tuberculosis during 12 years' period from 1980 to 1991 in our hospital were analyzed. The proportion of cases with so-called risk factors among total 520 cases was as follows: Aged patients (70 years of age and over) 31.5%. Cases discharging abundant bacilli in sputum (Gaffky scale VII or above or culture, +3 positive) 29.4%. Adverse reactions to drugs 18.1%. Far advanced cavitary lesions (GAKKAI Classification I or

[Bacterial CNS infections in adults in Southern Lower Saxony. A retrospective study of the Göttingen Neurologic University Clinic].

All 155 patients with suspected bacterial central nervous system (CNS) infections treated from 1986 to 1991 at the Department of Neurology, University of Göttingen, were evaluated in a retrospective study. According to the clinical symptoms presented at admission, 7 cases were classified as encephalitis, 44 as meningitis, 15 as radiculitis, 19 as ventriculitis, 61 as meningoencephalitis and 9 as meningoradiculitis. In 78% of these cases, the causative bacteria were either isolated from cerebrospinal fluid (CSF), or other relevant sources (blood, wound swabs) or identified by serological method

Intramedullary tuberculoma of the spinal cord: case report.

A case report of intramedullary tuberculoma is presented. Only one other case of intramedullary tuberculoma has been reported in the North American literature in the last 30 years. It is also unique because it is the only documented case of the paradoxical enlargement of an intramedullary tuberculoma after the initiation of antituberculous chemotherapy. Excellent results were obtained with a combination of medical and surgical management.

[Nontraumatic subdural hematoma secondary to dural metastasis of lung cancer: case report and review of the literature].

In this paper, a case of nontraumatic subdural hematoma secondary to dural metastasis of lung cancer was presented and 43 cases in the literature were reviewed. A 75-year-old man who had been affected with lung tuberculosis for the previous 7 years was admitted to our hospital because of right hemiparesis and aphasia. No history of head injury was discovered. On admission, he was fully conscious and no other neurological abnormalities were noticed except for right hemiparesis and motor aphasia. The chest X-ray revealed abnormal shadows on the right lung, which were most suggestive of tuberculo

Predictive value of computed tomography-based diagnosis of intracranial tuberculomas.

The need to obtain histological diagnoses of intracranial tuberculomas, before initiating therapy, is not universally accepted, because some clinicians believe that an image-based diagnosis is fairly accurate in patients from endemic regions. To evaluate the sensitivity, specificity, and predictive value of computed tomography (CT)-based diagnosis of an intracranial tuberculoma, we prospectively compared the preoperative imaging diagnoses with histological diagnoses in 105 consecutive patients with intracranial masses. CT differential diagnoses (first or second) of tuberculomas were considered

Control of disease by selective iron depletion: a novel therapeutic strategy utilizing iron chelators.

Recognition of the central role of iron in the generation of toxic, oxygen-derived species through the Haber-Weiss reaction, the ability of desferrioxamine (DFX) to prevent the damage associated with free radical generation in reperfusion injury, and its inhibitory effect on cell proliferation by inactivation of the iron dependent enzyme ribonucleotide reductase, resulted in an increasing number of studies exploring the novel therapeutic applications of iron chelating drugs: (a) Animal models of reperfusion injury have shown that DFX is able to decrease post-anoxic damage to the brain and hear

Paradoxical enlargement or development of intracranial tuberculomas during therapy: case report and review.

Intracranial tuberculomas can sometimes develop or increase in size despite administration of appropriate therapy. We report the case of a child whose intracranial tuberculomas paradoxically enlarged while therapy was being administered, and we review 23 other cases in which tuberculomas increased in size or number and 17 cases in which tuberculomas appeared during therapy. These phenomena generally occurred within 3 months of the start of therapy. All but four patients had neurological deterioration that prompted obtaining a repeated computed tomographic scan. One patient died, about one-four
