Clinical categories of neurobrucellosis. A report on 19 cases.

Brucellosis rarely can present with involvement restricted to the nervous system. We describe a total of 19 cases of neurobrucellosis in whom the clinical presentation lay in three distinct categories. The first was an acute presentation with meningoencephalitis. The disease also presented in a chronic form where the brunt of the illness can either be in the peripheral or the central nervous system (CNS). The chronic peripheral form is that of a proximal polyradiculoneuropathy. The central form is that of diffuse CNS involvement, predominantly with myelitis or cerebellar involvement with or wi

Production and characterization of high titer antibodies to galactocerebroside.

High titer antibodies primarily of the IgG class were produced against galactocerebroside (GalC) by including keyhole limpet hemocyanin (KLH) and supplemental M. tuberculosis in the adjuvant mixture used for immunization of rabbits. Antibody titers were determined by an ELISA in which microtiter wells were coated with liposomes containing lecithin, cholesterol and GalC. The antibodies showed reactivity with GalC and psychosine, but not glucocerebroside, sulfatide, mixed gangliosides or asialo GM1. Specificity was further demonstrated by absorption of antibodies with GalC. Binding was inhibited

[Stereotactic cerebral biopsy in acquired immunodeficiency syndrome (AIDS)].

Central nervous system (CNS) complications are currently described in patients with Acquired Immuno-Deficiency Syndrome (AIDS). CT-scan imaging is helpful in the description of the lesions. However, by CT-criteria alone, it is not possible to reliably distinguish toxoplasmosis from other CNS infections and from a primary CNS sarcoma. The authors consider that biopsy of the lesion is recommended to exclude other treatable diseases such as bacterial infections, tuberculosis, fungus and toxoplasmosis. Stereotactic technic gives the optimal precision and security.

Low serum antimycobacterial glycolipid antibody titers in the sera of patients with systemic lupus erythematosus associated with central nervous system involvement.

Antibodies which bind to glycolipids derived from Mycobacterium tuberculosis were sought in the sera of patients with systemic lupus erythematosus (SLE), with and without neurological disease, patients with active tuberculosis and normal controls. A significantly lower activity against the mycobacterial glycolipids was detected in the sera of patients with SLE with neurological involvement compared to other patients with SLE or controls. We suggest that low antibody levels against mycobacterial glycolipids may be useful in the diagnosis of patients with SLE in whom involvement of the central n

Disappearing CT lesions in epilepsy.

A striking but reversible computed tomographic (CT) lesion corresponding to seizure activity is reported in a series of 46 epileptic patients. None of these patients had evidence of tuberculosis or cysticercosis, and all were treated with antiepileptic drugs only. Maximal radiological changes occurred in the area of maximal epileptic discharge; however, multiple lesions were seen in three cases. The CT lesion reappeared with recurrence of the ictus in four cases during follow-up, and this, too, disappeared after complete arrest of the seizures. The clinical, CT scan, and other investigatory fi

Autopsy findings in 35 cases of leprosy in Malaysia.

The findings of autopsies performed on 35 leprosy subjects in the University Hospital, Kuala Lumpur, between January 1981 and December 1985 are presented. This is the first report based on autopsy findings from Malaysia. The patients were elderly subjects with a mean age of 74 years. Sixty-six percent had lepromatous leprosy. None had active skin lesions. The most common cause of death was pyogenic infection, particularly bronchopneumonia. Tuberculosis was noted in 25% of the cases. The other important causes of death included cardiac and renal failure. Renal lesions were evident in 71% of the

Unusual presentation of neurological tuberculosis. A case report.

A patient is described whose disease caused difficulty in diagnosis and indicated that neurological tuberculosis, like other extrapulmonary forms of this disease, may be associated with little or no evidence of pulmonary involvement. In a population where tuberculosis is common, antituberculosis drugs should be considered for any patient with undiagnosed illness--especially of the central nervous system--pending investigation. A search of the English-language literature did not reveal a similar report.

Lack of Mycobacterium leprae-specific uptake in Schwann cells.

Among mycobacteria, Mycobacterium leprae have a unique property to infect peripheral nerves, which is the cause of a variety of debilities seen in leprosy. The possibility of selective uptake of M. leprae by Schwann cells was studied using a rat Schwannoma cell line 33B and rat sciatic nerve-derived Schwann cells. M. leprae were phagocytosed by 33B cells but so also were seven other mycobacteria ("Mycobacterium w," BCG, M. tuberculosis H37Rv, M. nonchromogenicum, M. vaccae, ICRC bacillus, and M. smegmatis) which do not involve peripheral nerves. All three mycobacteria tested (M. leprae, M. tub

Tuberculous meningitis--clinical and laboratory review of 100 patients.

In developing countries tuberculous meningitis is a difficult infection to differentiate from other central nervous system (CNS) infections. This paper presents the history, physical findings, laboratory data, and clinical course of 100 patients who were admitted to a special ward and had CSF cultures positive for Mycobacterium tuberculosis. Fifty-four patients were comatose when admitted and 76 had meningeal signs. Mean admission CSF values were WBC 531, glucose 23 mg/dl, and protein 166 mg/dl. Only two CSF AFB smears were positive. Sixty-one percent of the chest X-rays taken were consistent

Sensitivity and specificity of enzyme-linked immunosorbent assay in the detection of antigen in tuberculous meningitis cerebrospinal fluids.

A sandwich enzyme-linked immunosorbent assay was developed for its potential utility in the detection of antigen in the cerebrospinal fluid of patients with tuberculous meningitis. Cerebrospinal fluids examined included those from untreated (group Ia) and treated (group Ib) Mycobacterium tuberculosis meningitis, nonseptic central nervous conditions (group II) such as epilepsy, viral meningitis, and tetany, and nonmycobacterial septic meningitis (group III). The average levels of antigens determined and percent positive specimens, respectively, for each group were (group): Ia, 1.8 micrograms/ml

A controlled trial of six-month and nine-month regimens of chemotherapy in patients undergoing radical surgery for tuberculosis of the spine in Hong Kong. Tenth report of the Medical Research Council Working Party on Tuberculosis of the Spine.

Sixty patients in Hong Kong with a diagnosis of tuberculosis of the thoracic, thoraco-lumbar or lumbar spine were allocated at random to either a 6-month (6M) or a 9-month (9M) regimen of daily isoniazid plus rifampicin, supplemented in all patients by streptomycin twice a week for the first 6 months. All patients had the modified 'Hong Kong' operation of radical resection of the lesion and insertion of autologous bone grafts. After exclusions, the main analyses of this report concern observations for 3 years on 51 patients (25 6M, 26 9M). The clinical and radiographic condition of the two ser

Fine structure of cellular and vascular reaction in brain tuberculomas: a model for phagocytosis in the CNS.

The fine structure of cells of the mononuclear phagocyte series (MPS) and a few other cells with phagocytic capacity, has been critically evaluated, mainly from an electronmicroscopic examination of the reactive border zone of 11 human brain tuberculomas, which provide ideal material for the study of macrophages. Most of them appeared to be blood monocyte-derived epithelioid cells of various forms and stages. The cytoplasm of these cells showed either more rough ER representing protein synthesising activity; or more frequently, phagosomes, phagolysosomes, dense bodies or empty vacuoles, repres

Neuropathologic observations in acquired immunodeficiency syndrome (AIDS).

Neuropathologic changes are described in eight fatal cases of the acquired immunodeficiency syndrome (AIDS), including four subjects who were i.v. drug abusers (two women, two men), two Haitian men, one bisexual man, and one women who had no known risk factors for AIDS. All cases had significant infections of the brain, with examples of bacterial, fungal, protozoal, and viral disease in the group. Combined infections were observed in three cases, including one case of previously unreported central nervous system (CNS) abscess due to Nocardia (combined with Salmonella). The most frequent CNS pa

Neurological manifestations of the acquired immunodeficiency syndrome (AIDS): experience at UCSF and review of the literature.

In this review of the acquired immunodeficiency syndrome (AIDS), the authors have evaluated a total of 352 homosexual patients with AIDS or generalized lymphadenopathy managed at the University of California, San Francisco (UCSF), between 1979 and 1984. Of an initial unselected group of 318 patients, 124 (39%) were neurologically symptomatic, and one-third already had their neurological complaints at the time of presentation. An additional 210 AIDS patients with neurological symptoms have been reported in the literature. Thus, a total of 366 neurologically symptomatic patients with AIDS or lym

Magnetic resonance imaging in infections of the central nervous system.

The magnetic resonance (MR) images in 24 patients with infections of the central nervous system (CNS) were reviewed, and the use of a paramagnetic contrast agent (Gd-DTPA) in two cases is reported. The clinical diagnoses in the patients, aged neonate to 71 years old, comprised meningitis, meningitis and subdural empyema, meningoencephalitis, encephalitis, and single or multiple cerebral abscesses. A new sign of CNS infection, the pial-ependymal line, is described. Other consequences of infection, such as atrophy, delayed myelination, periventricular cerebrospinal fluid extravasation, changes i

Therapeutic uses of trace elements.

The properties of trace elements which feature in their therapeutic activity are: binding to macromolecules (enzymes, nucleic acids, etc.) with disturbance of biological function, and interaction with other elements. These properties, particularly the binding to large molecules, are far from specific, an observation which is reflected in the very wide range of diseases in which trace elements are employed. While metal compounds have been administered for several centuries, the scientific basis for treatment with trace elements began with the use of gold compounds, initially in patients with tu

Atypical spinal tuberculosis.

The familiar picture of spinal tuberculosis is one of destruction of adjacent vertebral bodies and of the intervening disc. There are, however, other patients without these radiographic changes and with no clinical deformity who present with symptoms and signs of compression of the spinal cord or cauda equina. These patients fall into two different groups: those with tuberculosis of the neural arch; and those with extra-osseous extradural tuberculosis. Both may require laminectomy, but whereas the first has bony involvement and a cold abscess, the second has neither.
